Phylogeny and its clinical significance.

The creation of the universe, the origin of life, and the rise of the one celled organism to man are reviewed. The embryologic development of man recapitulates the entire phylogenesis. The evolution of the sense organs (ear and nose) and food and airway passages is discussed in detail. Finally, observations are made regarding phylogenetic development and its relation to human congenital malformations and other pathologic processes.

Congenital alar field defects: clinical and embryological observations.

Congenital malformations of the nasal ala may occur as isolated unilateral nasal alar defects or as part of more severe facial clefting syndromes. The embryologic significance of the isolated alar deformity seems separate from other craniofacial deformities and justified comment. Clinical examples of isolated nasal alar malformations are correlated with a study of 60 human embryos ranging from seven to 10 weeks gestation. These studies indicate that the nasal and alar developmental fields each have a separate critical time during which cells are most susceptible to altered development.

Kartagener's syndrome. Preliminary report on cilia structure, function, and upper airway symptoms.

Our study was designed to examine the motility and ultrastructure of cilia from the nose of patients with Kartagener's syndrome. Microphoto-oscillographic recording from single cells showed that the patients had in fact motile cilia, although the number was reduced. Asynchrony within the single cell was a more consistent feature. The first results of blind, quantitative microscopy showed the ultrastructural defects, described earlier, but the overlapping with a normal control group was considerable. Only one of nine patients had no dynein arms and completely immotile cilia; an observation whic

Holoprosencephaly in monozygotic twins - clinical and computer tomographic findings.

Of monozygotic male twins with holoprosencephaly, one showed by computer tomographic (CAT) scan a mild type with orbital hypotelorism, flat nose, bilateral cleft lip, and cleft palate associated with an intermaxillary rudiment. By CAT scan, the other boy had the alobar type with marked orbital hypotelorism, flat nose, absent nasal bridge, and median cleft lip. Lymphocyte chromosomes were apparently normal. A review of the literature shows a heterogeneous patient material in that most pertinent cases involved dizygotic twins, and some of these presumably had chromosome abnormalities.

[Trichorhinophalangeal syndrome (Giedion)].

Three cases of the Tricho-rhyno-phalangeal syndrome (TRF) are described. Two cases were in the same family with some affected membres (Camacho et al., 1978). The third case, of sporadic presentation, showed radiological signs of Perthes' disease. All cases presented the main clinic and radiologic alterations of the TRF syndrome: hypotrichia, pear-shaped nose and cone shaped epiphyses. Some of them presented other characteristic features such as: darwism, prominent nose, elonged philtrum, thin upper lip, baldness of the external 1/2 eyebrows, prominent ears of low implantation, shortening of so

Incomplete trisomy 22. I. Familial 11/22 translocation with 3:1 meiotic disjunction. Delineation of a common clinical picture and report of nine new cases from six families.

A syndrome due to 3:1 meiotic segregation of balanced 11/22 translocation is defined from nine personally observed patients and 22 cases from the literature with apparently the same aberration. Frequent findings include a characteristic face with deep-set eyes, flat nose, prominent upper lip, receding mandible and preauricular pits or tags, male genital hypoplasia, anal atresia or other anomalies of the anus, cleft palate, and congenital heart defect. Less frequent are severe reduction of the auricles, an additional pair of ribs, and hypoplasia of the diaphragm. Perinatal mortality is high. Gr

[A case of trisomy 19 mosaicism (author's transl)].

Trisomy-19 mosaicism was observed in lymphocytes of a newborn male with facial dysmorphism (upward slanting palpebral fissures, pointed nose, short upper lip, prominent antitragus), laryngeal stridor, and leucothrombopenia, and who died on day 13. Autopsy revealed no other major malformation but poor development of the cerebral sulci.

A new malformation syndrome of long palpebral fissures, large ears, depressed nasal tip, and skeletal anomalies associated with postnatal dwarfism and mental retardation.

Five unrelated patients with a previously unrecognized mental retardation malformation syndrome are presented. Clinical features common to them include moderate mental retardation, postnatal dwarfism, susceptibility to infection in infancy, and peculiar craniofacial dysmorphia characterized by long palpebral fissures, high-arched and abnormal eyebrows, heavy and long eyelashes, large ears, short nasal septum and/or depressed nasal tip, and cleft palate. Other anomalies are stubby fingers, deformed vertebra and other bone and joint anomalies, and abnormal dermatoglyphics. The absence of familia

Coloboma, congenital heart disease, and choanal atresia with multiple anomalies: CHARGE association.

We report 21 patients with choanal atresia or ocular coloboma or both who have certain other associated anomalies, including congenital heart disease, postnatal growth deficiency, mental retardation and/or CNS anomalies, microphallus and cryptorchidism, and ear anomalies and/or deafness. Facial palsy, micrognathia, cleft palate, and swallowing difficulties were also common. It has not been possible to define a single etiology or a syndrome in these patients. We propose the mnemonic CHARGE (C-coloboma, H-heart disease, A-atresia choanae, R-retarded growth and retarded development and/or CNS ano

A clinical syndrome associated with 5p duplication and 9p deletion.

We have evaluated a sister and brother with a similar pattern of malformations and death in early childhood associated with partial duplication chromosome 5p and possibly deletion of 9p. The father and the brother and several paternal relatives are carriers of the balanced translocation t(5;9) (p13;p22). The malformations which the two have in common are: prominent forehead, flat nasal bridge, long thin fingers, bilateral equinovarus deformity of the feet, diaphragmatic hernia and kidney malformations. The children died at ages 4 months and 27 months, the latter showing marked psychomotor reta

[Partial deletion of the short arm of chromosome 3. Report of a case (author's transl)].

A patient with partial deletion 3p is reported. Only one other patient has been previously reported in the literature. Features the two share include microcephaly, failure to thrive, ptosis, symophrys, a broad and flat nose, a prominent maxilla, an over developed chin, and an increased number of loops on the finger tips. These observations are compared in type and countertype with known observations of trisomy 3p and of 3q duplication/3p deletion.

Medical genetics for the otorhinolaryngologist.

Genetic disorders involving the ear, nose and throat can be looked at in essentially the same way that one would view other genetic problems. It is important to differentiate genetic disorders from those due to environmental influences. This may be difficult on clinical grounds, since similar clinical features may be produced by different environmental factors or by different genes or groups of genes. When the cause of the disorder can be established, the risks involved in a family having further offspring usually can be determined reasonably readily. In the uncharacterized situations, empiric

Duplication of region 2q31 leads to 2qter in a family with 2/9 translocation.

A balanced carrier mother with a t(2;9) (q31;p24) had a son with partial duplication of the long arm of chromosome 2(q31 leads to qter). A comparison of this child with other cases from the literature suggests that hypertelorism, a short nose, a broad nasal bridge, low-set ears, micrognathia, clinodactyly, a short neck and muscular hypotonia are characteristic phenotypic features of individuals having a duplication of 2q31 leads to qter.

The spectrum of frontonasal dysplasia in an inbred pedigree.

An inbred pedigree is described in which three members were affected with FND (Frontonasal Dysplasia). Two of these individuals were products of a consanguineous mating with an inbreeding coefficient of F = 0.0391. The third affected individual (propositus), was born to a marriage in which the coefficient of inbreeding was 0.0742. The mother of the propositus, whose inbreeding coefficient was 0.0625, had borderline hypertelorism and a broad nose. Several other members of the pedigree who had hypertelorism were products of consanguineous matings. The presence of consanguinity in all individuals

Incontinentia pigmenti achromians (Ito).

A patient with incontinentia pigmenti achromians was reported. She displayed other abnormalities such as a saddle nose, malformed auricles, growth retardation, convulsions and abnormal EEG. Histopathologically, a partial decrease in melanin granules in the basal layer and the presence of melanocytes which showed a weak dopa reaction were the main findings. The hypofunction of melanin production was detected by electron microscopy.

Congenital myopia and retinal detachment.

Retinal detachment is unusual in children and is often associated with other serious ocular abnormalities. In more than 25 children varying in age from 3 to 16 years in whom the common ocular complication was congenital myopia, associated features included facial abnormalities and other changes often associated with familial and inherited conditions. A very high incidence of giant retinal breaks was found and these were associated with considerable difficulty in treatment.

The immotile cilia syndrome: phase contrast light microscopy, scanning and transmission electron microscopy.

In the immotile cilia syndrome, transmission electron microscopy of the cilia shows abnormalities in the arrangement of the central pairs of tubules and in the dynein arms of the peripheral tubules, or in the radial spokes, We studied four nonrelated children, 9/12, 5, 6, and 6 years old, with situs inversus and a history of chronic sinusitis and bronchitis (Kartagener's syndrome) and four children in the same age group and with the same history, but without situs inversus. Under the phase contrast microscope no motile cilia were seen in the four patients with Kartagener's syndrome and in two

The immotile-cilia syndrome in a newborn infant.

A newborn infant was suspected to have the immotile-cilia syndrome. He had situs inversus, atelectasis and other pulmonary changes. A biopsy from the nasal mucosa at the age of 21 months confirmed the diagnosis and showed a lack of ciliary orientation and absent dynein arms. Chymotrypsin values, sweat test and most immunoglobulins were normal. The boy is now three years old. So far he has had rather mild respiratory symptoms. The importance of early diagnosis for management and prognosis is stressed.

Ring chromosome 10:46,XX,r(10)(p15 leads to q26).

Cytogenetic analysis of an 8-month-old Japanese girl with moderate retardation of physical development was performed and a ring chromosome 10, 46,XX,r(10)(p15→q26), was found. She had short stature, mildly stubby nose, antimongoloid slants, and moderately protruding ears. The extremities showed erythrocyanosis, oedema, and pigmentation. Lansky et al1 was the first to describe ring chromosome 10 with a Turner-like phenotype. Only two other cases2 3 have been reported so far and little is known about this syndrome. This is the report of a fourth case.

Experience with the Tajima procedure in primary and secondary repair in unilateral cleft lip nasal deformity.

The method of cleft lip nasal deformity repair described by Tajima and Maruyama appears to produce superior results compared to other methods we had tried. A modified technique of suture placement, aided by our special nasal retractor, greatly simplifies suture placement. In both primary and secondary cases, significant and maintained improvement has been achieved with what is a comparatively simple and standardized procedure.
