The association of congenital megaureter and ureteropelvic junction obstruction.

Megaureter and ureteropelvic junction obstruction are not uncommon urinary tract congenital abnormalities. While each has been known to be associated with other urogenital anomalies, their association together is rarely reported. This report will describe the diagnosis and management of such a case.

Duplication of the renal pelvis and ureter: associated anomalies and pathological conditions.

Thirty-one patients with anomalous and/or pathological conditions in the duplication of the renal pelvis and ureter were studied in order to analyze the urographic findings. A segmental defect evident in the visualization was caused by the abscess of one of the poles in the kidney. In patients with hypoplasia of one of the poles, the other normal pelvis extended to the neighboring segment, and consequently showed the same pattern as a simple pelvis and ureter on urography. The renal hypoplasia was apt to be combined with simple or ectopic ureterocele and ectopic ureteral opening. In patients w

Carcinoma at the site of ureterosigmoidostomy.

A 31-year-old woman who had undergone bilateral ureterosigmoidostomy at age five had a moderately differentiated adenocarcinoma at the anastomotic site that was not detected by preoperative studies or intraoperative palpation. The authors review other reports of anastomotic carcinomas occurring after ureterosigmoidostomy for benign disease and recommend a diagnostic protocol. Whenever the urine is diverted away from the sigmoidostomy (rediversion), the anastomotic site should be resected. Ureterosigmoidostomy should be undertaken only with the understanding that meticulous long-term follow-up

Autosomal dominant duplication of the renal collecting system, hearing loss, and external ear anomalies: a new syndrome?

We report two families in which propositi had severe bilateral sensorineural hearing loss, a preauricular pit or tag, and duplication of the ureters or bifid renal pelvices. Other relatives had one or more of these anomalies in a pattern suggesting autosomal dominant inheritance with reduced penetrance and variable expressivity. We suggest the term "branchio-oto-ureteral syndrome" to designate this condition.

Ectopic ureter with seminal vesicle insertion in an infant with tracheoesophageal fistula and possible adult polycystic kidney disease.

We report on a male newborn with a tracheoesophageal fistula and possible adult polycystic kidney disease as well as a single ectopic ureter with seminal vesicle insertion. There is an association of tracheoesophageal fistula, hindgut and sacral anomalies with ureteral ectopia but adult polycystic kidney disease has not been described. The diagnostic modalities currently available are discussed, as well as the implications of the association with other anomalies.

Chronic renal failure due to unilateral renal agenesis and total renal dysplasia (= aplasia). Report of three cases.

Three adult patients with unilateral renal agenesis/total dysplasia (= aplasia) and with an early chronic renal failure are presented. One patient had renal agenesis without ureter bud and ureteric ostium on one side, and reflux pyelonephritis on the other; one had small compact total renal dysplasia (= aplasia) on one side, while chronic uric acid nephropathy (chronic renal disease as a cause of gout) was diagnosed on the other; the third patient had a total large multicystic dysplasia on one side, and on the other a segmental large multicystic dysplasia. Radiological steps and radiodiagnosti

Congenital ureteral valves: report of 2 patients, including one with a solitary kidney and associated hypertension.

We report 2 cases of congenital ureteral valves treated successfully by excision of the involved segment and ureteroureterostomy. Hypertension in 1 patient and recurrent urinary tract infection in the other prompted initial evaluation. In the hypertensive patient renin collections from the vena cava and the renal vein of the solitary kidney were normal. Nevertheless, relief of obstruction has resolved the hypertension for 3 years. We review the literature regarding ureteral valves and discuss the mechanism by which unilateral hydronephrosis probably produces hypertension.

[Long-term observations following urinary drainage into the intact large intestine].

A report is given on 21 patients who have survived for at least 5 years after drainage of urine into the non-eliminated large intestine (Coffey-Mayo). Uretero-intestinal anastomosis was performed in 19 children, because of extrophy of the bladder, and in 2 women, following a malignant primary disease. In around half the patients there have been no complications during the post-operative observation period (on average 16.5 years), while new drainages had to be performed in the other half because of structures of the anastomoses. The patients have to adapt their daily lives to the "Coffey" and k

[Diagnosis and treatment of Wilms' tumor].

Recent advances in the diagnosis and treatment of Wilms' tumor are reviewed. Our study disclosed that principal clinical and pathological features of Wilms' tumor are as follows: (1) It occurs in young infants and children under 5 years of age; (2) It is frequently associated with other congenital anomalies; (3) Deletion of a portion of short arm of chromosome 11 can be closely related to the tumor genesis of nephroblastoma; (4) The occurrence of distant metastases is rare at diagnosis; (5) Differential diagnosis between Wilms' tumor and neuroblastoma is occasionally quite difficult; (6) There

Neoplasia and ureterosigmoidostomy: a colonoscopy survey.

Patients who have undergone implantation of ureters into the sigmoid colon (ureterosigmoidostomy) are known to be at high risk of developing cancer of the colon many years later. The operation is often performed in infancy for congenital abnormalities of the bladder, thus creating a long term surveillance problem. Six of 34 patients (17.6 per cent) who had undergone ureterosigmoidostomy were found at a screening fibresigmoidoscopy to have adenomas of the left colon or severe dysplasia of the stoma, a mean interval of 22 years after their urinary diversion. Four other patients had previously ha

A rare variant of ureteric ectopia: opening in vagina and vesico-uretero-vaginal communication.

A rare variant of ureteric ectopia, i.e. an opening in the vagina and a vesico-uretero-vaginal communication, is herein reported. This type of congenital anomaly, found in 2 females, has not been reported before. One patient had a free, 2-way communication between the bladder and the vagina via the lower segment of the right ectopic ureter. The other was found to have a one-way communication from the vagina to the bladder through the ectopic distal ureter. Surgical intervention was undertaken in both cases. It is suggested that vaginography after distending the bladder with air is important in

Nonrefluxing colonic conduit: efficiency and complications of ureterocolic anastomosis.

The efficiency of and complications in 65 cases of colonic conduit have been studied (64 cases of bladder exstrophy and 1 case of cloacal exstrophy). For the ureterocolic reimplantation, we employed the Leadbetter technique and a modified kelalis technique. The results after 1-12 years in 128 renoureteral units showed 28 complications (21 obstruction and 7 refluxing units). Other types of complications have been observed and reported in this work. The high incidence of obstruction was probably due to the long submucous tunnel in the ureterocolic reimplantation. We believe that the sigmoidal co

Surgical correction and rehabilitation for children with "Prune-belly" syndrome.

Until the recent past, patients born with congenital absence of the abdominal musculature, and associated deformity of the urinary collecting system, have often had an unhappy life and an early death. Twelve years ago, a treatment protocol was devised in our institution which included the following major steps: 1) bilateral tubeless pyelostomy immediately after birth; 2) serial evaluation of renal status during the first year of life; 3) laparotomy at one year of age with orchiopexy, reconstruction of the urinary collecting system and reconstruction of the abdominal wall; 4) serial electromyog

Ultrasound in the investigation of renal disease in infants and children.

The author reports the case histories of a number of neonates and children with renal disease to illustrate the use of grey-scale ultrasound in the investigation of such patients. In the series, ultrasound proved to be an excellent diagnostic tool in the investigation of masses of suspected renal origin, both in ascertaining their site of origin and in evaluating their nature. Other applications of diagnostic ultrasound included investigation of the kidney not visualised by urography, and suspected urinary tract obstruction, and renal or peri-renal infection. The characteristic ultrasound appe

Congenital female epispadias with incontinence.

Six cases are described of congenital female epispadias and incontinence. Of these patients 3, who were seen in the last 4 years, were made continent by a 1-stage operation using a combined perineal and transvesical approach. The procedure includes excising the abnormally wide roof of the distal urethra from below narrowing the urethral caliber and mons plasty. From above the upper urethra and bladder neck are narrowed. Ureters are reimplanted if they have reflux or to facilitate bladder neck narrowing. The 3 other cases are described in which this combined procedure could have been used, in r

Treatment and prevention of complications after extravesical antireflux technique.

An extravesical antireflux technique was performed in 300 consecutive patients who presented primary vesicoureteral reflux (VUR). The following results are reported: Elimination of VUR in 99% of the cases without complications. Early complications reported include: urinary retention, suppurative pyelonephritis and fistulae. Urinary retention occurred in 2 patients presenting other congenital abnormalities. Vesicocutaneous fistulae occurred in 2 patients due to obstruction of the catheter for vesical drainage; they closed spontaneously under adequate drainage of the bladder. Suppurative pyelone

Congenital cystic adenomatoid malformation in bilateral renal agenesis. Its mitigation of Potter's syndrome.

Potter's syndrome develops secondary to a deficiency of amniotic fluid, such as occurs in renal agenesis. Congenital cystic adenomatoid malformation (CCAM), on the other hand, is frequently accompanied by polyhydramnios. We describe a newborn with both renal agenesis and CCAM who had only mild features of Potter's syndrome. The pathogenesis of polyhydramnios in CCAM is discussed with regard to the ultrastructural findings of numerous type 2 pneumocytes lining the cysts. The association between CCAM and bilateral renal anomalies is emphasized.
