Dose matters: haploinsufficiency in osteogenesis imperfecta.

Osteogenesis imperfecta is a common genetic disorder of syndromic bone fragility manifesting in types with variable skeletal and extraskeletal severity. Over the past decades, the deforming types have almost exclusively received all scientific attention, leaving large knowledge gaps about the prevalent osteogenesis imperfecta type 1. However, studies within the past 5 years reveal serious unrecognized aspects of disease burden in this large patient population that contrast with osteogenesis imperfecta type 1's widely adopted classification as mild osteogenesis imperfecta. These patients presen

Diagnostic familiarity with osteochondroma among primary physicians: a retrospective analysis of 195 referred cases.

INTRODUCTION: Osteochondroma or exostosis is the most common benign bone tumor and is typically diagnosed based on its characteristic radiographic features. While it is generally familiar to orthopaedic surgeons, the level of diagnostic familiarity among primary physicians remains unclear. MATERIALS AND METHODS: This retrospective study reviewed 195 patients diagnosed with osteochondroma/exostosis who were referred to a tertiary orthopaedic oncology center. Inclusion criteria included radiological confirmation of osteochondroma/exostosis and the availability of referral documents. Diagnostic a

Co-Occurrence of Achondroplasia and 16p11.2 Deletion Syndrome in a Child With Group 4 Medulloblastoma: Expanding the Spectrum of Dual Genetic Diagnoses.

Achondroplasia (OMIM #100800), the most common skeletal dysplasia, results from the recurrent FGFR3 c.1138G > A (p.Gly380Arg) gain-of-function variant, which constitutively activates fibroblast growth factor receptor 3 signaling and impairs endochondral ossification. The 16p11.2 microdeletion (OMIM #611913) is a recurrent ~600 kb copy-number variant associated with developmental delay, autism spectrum disorder, epilepsy, and obesity, encompassing dosage-sensitive genes including MAPK3, KCTD13, and PRRT2. Recent genome-wide analyses have linked 16p11.2 deletion to a 13.9-fold increased risk of

Airway Involvement in Conradi-Hünermann-Happle Syndrome: A Novel Clinical Manifestation.

We report the first documented case of airway involvement in Conradi-Hünermann-Happle syndrome (CDPX2), an X-linked dominant form of chondrodysplasia punctata caused by pathogenic variants in EBP. A 2-month-old female with genetically confirmed CDPX2 developed severe subglottic stenosis and persistent respiratory distress requiring CPAP; cross-sectional imaging demonstrated extensive laryngotracheal calcifications. Serial endoscopic balloon dilations successfully restored airway patency, enabled weaning from respiratory support, and avoided tracheostomy, expanding the recognized clinical spect

Phenotype Variations in a Family with Various Rearrangements in the Locus of the SHOX Gene.

The SHOX gene is located on both sex chromosomes, X and Y, within the pseudoautosomal region 1 (PAR1). Gross deletions at the SHOX locus lead to protein insufficiency and are manifested by growth disorders such as Leri-Weill dyschondrosteosis (LWD), Langer mesomelic dysplasia (LMD), and idiopathic short stature (ISS). In cases of the SHOX gene duplication, the phenotype may range from tall to short stature and LWD. This study describes a family with various SHOX locus alterations and diverse phenotypic manifestations. The proband inherited both deletion and duplication in the SHOX locus from h

The Impact of Genetics on Craniofacial Dysplasias and Consequent Oral Malformations-Integrative Review.

BACKGROUND/OBJECTIVES: Diseases affecting the craniofacial skeleton are normally associated with disturbances in the regulation of cellular differentiation, the development of bone structures, and changes in bone density and ossification. Thus, the objective of this integrative review is to evaluate the published scientific literature from the last 8 years concerning the impact of genetics on some craniofacial dysplasias. Our aim covers the identification of oral cavity alterations to those dysplasias, through the most common orofacial manifestations. Three dysplasias were selected to be part

Computed Tomography Analysis of Craniofacial Features in Japanese Patients With Cleidocranial Dysplasia.

Cleidocranial dysplasia is a congenital malformation syndrome characterized by skeletal and dental abnormalities as well as distinctive craniofacial features. Most previous reports have relied on two-dimensional radiographs, and no comprehensive three-dimensional investigations have been conducted. This study aimed to provide a detailed overview of craniofacial features in patients with cleidocranial dysplasia using computed tomography. Craniofacial characteristics of 11 Japanese patients were assessed using three-dimensional computed tomography reconstructions. Qualitative evaluations were pe

[Research advance on the clinical phenotypes and molecular genetic mechanisms of Microcephalic primordial dwarfism].

Primordial dwarfism (PD) refers to a group of monogenic genetic disorders characterized by intrauterine growth restriction (IUGR) and severe, persistent postnatal growth retardation. These diseases have been associated with variants of multiple genes whose products are mainly involved in critical cellular biological processes such as maintenance of genomic stability, DNA damage repair, mRNA splicing regulation, and centrosome function. Variants of such genes can directly impair cell proliferation and developmental potential. With the widespread application of molecular genetic technologies suc

Co-Occurrence of Osteogenesis Imperfecta Type III and Chronic Abruption-Oligohydramnios Sequence: A Case Report Suggesting a Possible Role of Type I Collagen Fragility.

Pregnancy complicated by severe osteogenesis imperfecta (OI) is rare, and chronic abruption-oligohydramnios sequence (CAOS) is an uncommon obstetric disorder characterized by persistent bleeding and oligohydramnios without membrane rupture. To our knowledge, this is the first report describing the coexistence of type III OI and CAOS. A 24-year-old woman with OI developed recurrent bleeding early in pregnancy. Genetic testing revealed a novel COL1A1 splice-site variant (c.1876-2A>C). Ultrasonography showed a circumferential subchorionic hematoma and progressive fluid loss, leading to CAOS diagn

Report of the favorable pregnancy outcomes in an FKBP10-related Bruck syndrome case and a narrative review of pregnancy in severe osteogenesis imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a genetic syndrome that affects skeletal and connective tissues. OI type XI, also known as Bruck syndrome, is an extremely rare autosomal recessive disorder characterized by symptoms of severe OI, along with additional features. Despite the fact that patients with severe OI can survive into adulthood and have preserved fertility, only a few case reports have been published about pregnancy complications in Bruck syndrome. CASE REPORT AND NARRATIVE REVIEW: We report a case of maternal FKBP10-related Bruck syndrome, along with details of antenatal care,

Deep-Intronic Variant in RUNX2 Causing Pseudo-Exon Inclusion in a Family With Cleidocranial Dysplasia.

A deep-intronic single nucleotide variant in RUNX2 causes the characteristic clinical features of cleidocranial dysplasia (CCD) in a family via pseudo-exon inclusion into the mRNA. The pseudo-exon contains a premature stop codon and triggers mRNA decay, which results in RUNX2 haploinsufficiency, the known disease mechanism.

Case Report: Successful management of refractory SAPHO syndrome with guselkumab-upadacitinib combination.

SAPHO syndrome is a rare chronic aseptic inflammatory disorder characterized by the concurrence of osteoarticular inflammation and cutaneous lesions. The intricate mechanism network, coupled with issues such as paradoxical reactions, often results in suboptimal therapeutic outcomes. To the best of our knowledge, this is the first report of refractory SAPHO syndrome achieving clinical remission with the combination of guselkumab (an IL-23 inhibitor) and upadacitinib (a JAK inhibitor). A 38-year-old male with refractory SAPHO experienced complete pain resolution within 2 weeks and significant im

Impaired mitophagy contributes to osteogenesis and mineralization disorders in fibrous dysplasia.

Fibrous dysplasia (FD) is a bone mesenchymal stromal cells (BMSCs)-derived disorder caused by GNAS gene mutation, characterized by excessive fibrous tissue proliferation in bone and the formation of immature trabecular bone. Although impaired osteogenesis of BMSCs is central to FD pathogenesis, the underlying mechanism remains largely elusive. Here we demonstrate that hyperactivation of the cAMP-PRKA/PKA signaling axis disrupts mitochondrial homeostasis through impaired mitophagy, ultimately leading to diminished amorphous calcium phosphate (ACP) secretion and consequent mineralization failure
