AIM: Surgical repair of large congenital diaphragmatic hernia (CDH) remains a challenge. Although many different procedures are described, none has become a 'gold standard'. This study aimed to evaluate long-term results of the single muscle flap repair to avoid using a prosthetic patch.
MATERIALS AND METHODS: After institutional review board approval (#15.92), we conducted a retrospective study including all neonates who underwent single transversus abdominis muscle flap repair as first-line treatment for large CDH between 1995 and 2023 in our institution. Survivors were assessed clinically

BACKGROUND: Patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and comorbid asthma have more severe disease and are difficult to treat. However, the molecular endotypes associated with CRSwNP with comorbid asthma (CRSwNP + AS) are not clear. This study aimed to investigate the characteristics of type 2 inflammation and the molecular signatures associated with CRSwNP + AS.
METHODS: A total of 195 subjects; including 65 CRSwNP + AS patients, 99 CRSwNP-alone patients, and 31 healthy control subjects; were enrolled in the study. Nasal tissues from patients with CRSwNP + AS, CRSwNP-al

BACKGROUND: In congenital diaphragmatic hernia (CDH), the size of the diaphragmatic defect is a key determinant of lung hypoplasia, pulmonary hypertension, survival and complexity of postnatal surgical repair. Current prenatal prognostic assessment relies on indirect surrogates of disease severity-such as lung volume measurements and liver herniation-that do not directly quantify the anatomical defect. Direct prenatal assessment of diaphragmatic defect size remains largely unexplored, with limited evidence available.
OBJECTIVE: To introduce the Defect-to-HEmithorax Ratio (DHER), a novel prena

INTRODUCTION: To evaluate the association between longitudinal changes in Observed-to-Expected Lung-to-Head Ratio (O/E LHR) measured by ultrasound and postnatal survival in fetuses with Congenital Diaphragmatic Hernia (CDH).
METHODS: A total of 74 pregnant women carrying fetuses with CDH were retrospectively analyzed. The change in O/E LHR (ΔO/E LHR) was assessed as a continuous variable using univariate and multivariate logistic regression analyses. Hernia lateralization and liver herniation were combined into a single categorical variable to account for their clinical and statistical correl

INTRODUCTION: Congenital diaphragmatic hernia (CDH) manifests as a result of incomplete closure of the diaphragm muscles during embryological development. Most cases are diagnosed during the antenatal period or may manifest during the first week of life. In this case, we have reported a child patient with a late-presenting left congenital diaphragmatic hernia with bowel obstruction diagnosed by an X-ray alone.
PRESENTATION OF CASE: An 8-year-old boy presented with severe generalized abdominal pain, associated with vomiting, constipation, and difficulty in breathing. On examination, the child

Congenital diaphragmatic hernias (CDHs), particularly those of the Bochdalek type, arise when the pleuroperitoneal canal fails to close during fetal development, most often affecting the left posterolateral aspect of the diaphragm. These hernias are typically identified in the neonatal period, making their appearance in adolescence quite rare. When they do occur later in life, they may present with a range of GI or respiratory symptoms, which can complicate diagnosis. A missed or delayed diagnosis, especially in cases involving visceral perforation, can significantly increase the risk of compl

BACKGROUND: Congenital diaphragmatic hernia (CDH) is typically diagnosed in the prenatal or immediate postnatal period. Late-presenting CDH is rare and can have varied, nonspecific symptoms. Upper gastrointestinal bleeding is an uncommon initial manifestation. CASE PRESENTATION: We present the case of an 11-month-old male infant with respiratory distress and coffee-ground emesis. Initial imaging revealed a large left-sided diaphragmatic hernia with mediastinal shift. Laparotomy showed herniation of stomach, small intestine, and spleen, with gastric ischemia and subcapsular splenic hematoma. Re

Transamniotic stem cell therapy (TRASCET) constitutes the least invasive approach to prenatal stem cell delivery described to date. This strategy is largely based on the recently discovered active transport of donor cells from the amniotic fluid to the placenta directly through the amnio-placental interface. From a simple amniocentesis, via the placenta, donor cells of different phenotypes can reach the fetal circulation and virtually any fetal anatomical site, including, and particularly, the fetal bone marrow. As a result, TRASCET has shown broad therapeutic potential in an assortment of ani

Effective sucking is important for shortening hospital stays in newborn infants. Several studies have demonstrated that oral support can improve sucking efficiency. However, the effects of oral support on sucking pressure have not been thoroughly investigated. In our previous study, we reported the equipment configuration and analysis method of sucking pressure. That method of measuring sucking pressure was applied to cheek support. In this study, four newborn infants with low birth weight, congenital diaphragmatic hernia, or multicystic encephalomalacia were enrolled. Their sucking pressures

Congenital diaphragmatic hernia (CDH) associated with neural tube defects is exceedingly rare. We report a female neonate (2550 g) born at 35 weeks (Apgar 2/4/5) with severe left-sided Bochdalek CDH, lumbosacral myelomeningocele, Chiari II malformation, and unilateral renal dysplasia. Maternal history revealed no folic acid supplementation. Despite intensive care and inhaled nitric oxide for persistent pulmonary hypertension, the patient died at 72 h. This case highlights the value of prenatal ultrasound in detecting multisystem anomalies and supports the concept of early embryologic mesoderma

RATIONALE: Neonates with congenital diaphragmatic hernia (CDH) frequently experience central airway abnormalities, including tracheobronchomalacia, which persist post-surgical repair. However, these complications often remain underdiagnosed due to reliance on symptomatic evaluation and limited use of bronchoscopy. Ultrashort echo time (UTE) magnetic resonance imaging (MRI) offers a non-invasive, three-dimensional method to assess airway dynamics and evaluate tracheobronchomalacia.
OBJECTIVES: To quantify changes in central airway morphology and dynamics before and after surgical diaphragmatic

BACKGROUND: Morgagni hernia is a rare type of congenital diaphragmatic hernia, and its prenatal diagnosis by ultrasound is extremely challenging, especially when combined with massive pericardial effusion, as it can be easily confused with other diaphragmatic hernias. This article reports a case of fetal Morgagni hernia definitively diagnosed by serial prenatal ultrasound and magnetic resonance imaging, followed by successful intrauterine pericardial drainage and postnatal surgical correction. It aims to emphasize the importance of standardized imaging follow-up for prenatal identification and

OBJECTIVE: We assessed gender based access to pediatric surgical care in Central and South Asia (CSA) by reviewing published clinical studies.
METHODS: A PRISMA-guided systematic review and meta-analysis of 99 pediatric surgical studies from CSA (2004-2023) included 10,508 children. Nineteen conditions grouped as acquired, congenital, or tumor related. After adjusting for minor sex based incidence differences using global and regional rates, we compared adjusted sex distributions with those in high-income countries.
RESULTS: Boys were overrepresented in acquired conditions compared with high

INTRODUCTION: In this study, we aimed to investigate the expression of nuclear Yes-associated protein (nYAP), a molecule involved in mechanotransduction, during fetal lung development in a novel ex vivo mouse model of congenital diaphragmatic hernia (CDH) lung hypoplasia.
METHODS: Pregnant CD1 mice were gavaged nitrofen at E8.5 to induce CDH lung hypoplasia. At E16.5, fetal lungs were harvested and cultured ex vivo for 72 h with or without tracheal occlusion (hypo-TO and hypo, respectively). Age-matched normal fetal lung controls (norm-TO and norm) were evaluated in parallel. Whole lungs at E

Morgagni-Larrey hernia is a rare condition, accounting for less than 5% of congenital diaphragmatic hernias, with a marked predominance on the right side. Left-sided localizations are encountered rarely due to the protection afforded by the heart and pericardium. We report the case of a 17-year-old patient admitted to the emergency department with acute abdominal pain and dyspnea. Chest X-ray revealed a left basithoracic opacity with air-fluid level, confirmed by thoracoabdominal-pelvic computed tomography as corresponding to a left sided Morgagni-Larrey hernia. The hernial contents were parti

BACKGROUND: Bochdalek hernia is a congenital posterolateral diaphragmatic defect that typically presents in infancy, yet some defects remain clinically silent and are first detected in adulthood. Adult cases may be incidental or present with nonspecific respiratory or gastrointestinal symptoms, and cross-sectional imaging is often required for definitive diagnosis.
CASE PRESENTATION: A 65-year-old woman presented with two months of fatigue and progressive exertional dyspnea without gastrointestinal complaints. Examination demonstrated decreased breath sounds and crackles at the left lung base

Adult Bochdalek hernia is a rare and often misdiagnosed congenital diaphragmatic anomaly that poses significant clinical risks due to the potential for visceral incarceration and strangulation. We report a case of a 20-year-old female patient with no prior medical history who presented with non-specific arrhythmias, an atypical clinical onset that led to the identification of a massive left-sided diaphragmatic hernia. Diagnostic imaging, specifically multidetector computed tomography, confirmed migration of the stomach, spleen, and bowel loops into the thoracic cavity, associated with pulmonar

Congenital diaphragmatic hernia (CDH) diagnosed in the first or early second trimester is exceedingly rare. We herein present a unique case of Emanuel syndrome identified through early sequential findings of increased nuchal translucency (NT) and CDH. A 34-year-old Japanese woman was referred at 14 weeks of gestation because of an elevated NT of 3.9 mm detected at 12 weeks, with no other markers of aneuploidy. At 15+1/7 weeks, an ultrasound revealed right-sided heart displacement, a tubular structure near the heart, and a cystic structure on the left, presumed to be the stomach and intestines,
