Psychotic Features in Myhre Syndrome: Evidence for Broader Neuropsychiatric Surveillance.

Myhre syndrome is a rare genetic disorder caused by pathogenic variants in SMAD4. The neurocognitive profile may include intellectual disability and developmental delay across a wide spectrum of domains. Four individuals with Myhre syndrome and psychosis and/or schizophrenia have been reported. We discuss three additional individuals (one briefly mentioned in a review of patients with Myhre syndrome) with SMAD4 pathogenic variants at codon Arg496Cys (two cases) and codon Ile500Val (one case). All had social challenges and variable developmental delays. They presented with acute-onset psychosis

Variants in CEP135 Cause Congenital Microcephaly and Primary Ciliary Dyskinesia.

INTRODUCTION: Primary ciliary dyskinesia (PCD), a disorder of motile ciliary dysfunction causing chronic respiratory infections, may also rarely present with aspects of non-motile ciliary dysfunction, including retinitis, central nervous system malformations, skeletal dysplasia, and sensorineural hearing loss. However, congenital microcephaly has not been previously associated with PCD. METHODS: We identified three children with congenital microcephaly and chronic respiratory disease consistent with PCD. Comprehensive PCD diagnostic testing and whole exome sequencing were performed. Character

Advances in PMP22 sequence variant-associated hereditary neuropathy with liability to pressure palsies: from gene dosage effects to mechanism-informed variant interpretation.

Hereditary neuropathy with liability to pressure palsies (HNPP) is an autosomal dominant neuropathy classically caused by a heterozygous deletion of the 17p11.2-p12 region encompassing PMP22. However, PMP22 sequence variants can produce HNPP-like or overlapping phenotypes, complicating variant interpretation. This narrative review synthesizes clinical, genetic, functional, and experimental evidence relevant to PMP22 sequence variant-associated HNPP. PMP22 sequence variants may cause quantitative loss of functional protein or qualitative impairment through defective folding, trafficking, membra

Correlation of Prolonged Sitting Time and Sitting Posture on Low Back Pain: A Cross-Sectional Study among Medical Students at Universitas Sumatera Utara.

BACKGROUND: Low Back Pain (LBP) is the most common musculoskeletal disorder in the world. Medical students, particularly those who use computers for study purposes, are more vulnerable to LBP due to prolonged sitting times and improper sitting posture during lectures. However, prior research showed that the prevalence of LBP among medical students appeared lower when they were enrolled during online pandemic learning. Hence, this research is performed to determine the correlation between the duration of sitting and sitting posture on the incidence of LBP among medical students in Universitas S

Unraveling the Mechanistic Spectrum of Myhre Syndrome: SMAD4 Signaling Disruption, Skeletal Phenotypes, and Translational Innovation.

Myhre syndrome (MYHRS) is an ultra-rare, progressive multisystem disorder caused by recurrent heterozygous missense variants in the SMAD4 gene, a central mediator of TGF-β and BMP signaling. Skeletal abnormalities-including postnatal short stature, brachydactyly, thickened calvarium, and craniofacial dysmorphism-are cardinal features, often accompanied by joint contractures and progressive soft tissue fibrosis. Extensive cellular and genetic evidence supports a gain-of-function (GoF) mechanism wherein mutant SMAD4 displays increased protein stability and prolonged nuclear localization, enhanci

A review of Hallermann-Streiff syndrome demonstrates clinical overlap with other conditions.

Hallermann-Streiff syndrome (HSS) is a rare genetic condition characterized by seven cardinal findings comprising congenital cataracts, microphthalmia, recognizable facial features, sparse hair with hypotrichosis, skin atrophy, dental anomalies, and short stature. Around 200 patients have been reported and almost all cases have been sporadic. Several reviews have described the clinical findings in patients reported to have HSS, but the lack of a genetic test or other biomarker has complicated an accurate delineation of the condition. Although HSS is suspected to have a genetic basis and non-re

Therapy for Myhre Syndrome: Goals, Misconceptions, and Current Agents.

Myhre Syndrome (MYHRS, MIM #139210) is a rare, multisystem connective tissue disorder caused by recurrent heterozygous gain-of-function pathogenic variants in the SMAD4 gene, a key player in TGF-β signaling and a regulator of extracellular matrix homeostasis. MYHRS is characterized by a progressive fibrotic phenotype affecting multiple organ systems, including the skeletal, cardiovascular, respiratory, and integumentary systems. MYHRS individuals often present with short stature, joint contractures, cardiac valve defects, subglottic stenosis, and skin thickening. Neurodevelopmental disorders,

What are the functional, esthetic, and psychosocial effects of orthognathic surgery on quality of life? A prospective observational study.

BACKGROUND: Dentofacial deformities are associated with functional, esthetic, and psychosocial impairments that negatively affect quality of life. Orthognathic surgery is used to correct these deformities; however, changes in postoperative quality-of-life may be influenced by both clinical and psychosocial factors. METHODS: This prospective observational study included 71 patients who underwent orthognathic surgery between June 2023 and June 2024. Oral health-related quality of life was assessed preoperatively, at 6 weeks postoperatively, and at 6 months postoperatively using the Oral Health

Selective Ultrasound Screening for Developmental Dysplasia of the Hip in At-Risk Infants With Normal Physical Examinations.

BACKGROUND: Screening for developmental dysplasia of the hip (DDH) remains variable, particularly for infants who present with known risk factors but have normal physical examinations. This study evaluated the diagnostic yield and management impact of selective ultrasonography screening in this patient population. MATERIALS AND METHODS: We analyzed data from all infants who underwent hip ultrasonography for at-risk screening at a single institution between 2018 and 2024. Patients with at-risk status had a normal physical examination and 1 or more DDH risk factors (ie, breech presentation, fir

Gastroschisis-Like Defect in a Crossbred Bovine Calf: Anatomical, Clinical and Surgical Perspectives.

Congenital abdominal wall defects with evisceration are relatively uncommon in bovine neonates. This report describes a gastroschisis-like anomaly in a one-day-old female crossbred calf, suspected to result from the prenatal rupture of an omphalocele. The condition was characterised by evisceration of the pyloric abomasum and a portion of the small intestine through a ventral abdominal wall defect (involving the umbilical ring) without a protective sac. Clinically, the calf presented with generalised weakness, lateral recumbency and an inability to suckle. Physical examination revealed hypothe

Radioulnar Synostosis After Isolated Undisplaced Distal Ulna Fracture: A Rare Case Report and Review of the Literature.

CASE: A 50-year-old man developed radioulnar synostosis and presented with restricted forearm rotation, 15 months after sustaining an undisplaced fracture of the ulna. Radiographs and computed tomography revealed a mature bony bridge between the forearm bones. Surgical excision of the heterotopic bone was performed, and bone wax was applied to mitigate hemorrhage and prevent recurrence. Postoperatively, the patient achieved near-complete pronation and supination and no recurrence was found at the end of 2 years of follow-up. CONCLUSION: Radioulnar synostosis is an infrequent complication asso

[The expanded concept of orthognathic surgery as a "five-in-one" mode involving appearance, occlusion, joints, airway and psychology].

The orthognathic surgery aims to treat dentofacial deformities, with the fundamental principle of addressing both appearance and function, and combining surgical and orthodontic treatments. This approach integrates orthodontics and orthognathic surgery to resolve issues related to facial appearance and occlusal relationships. With the development of socio-economic conditions and the widespread dissemination of scientific knowledge, patients' awareness and acceptance of orthognathic surgery have continuously increased. The accumulation of clinical experience has led scholars to realize that ort

Evans Calcaneal Osteotomy in Flexible Flatfoot Deformity: A Podiatry Institute Perspective.

The Evans calcaneal osteotomy is a powerful lateral column lengthening procedure that creates multiplanar correction in the flexible flatfoot deformity. The procedure was popularized within podiatric medicine and surgery by Dr James V. Ganley. The Podiatry Institute faculty modified the procedure over the years. It has stood the test of time as a reliable procedure with good bone healing and predictable radiographic correction. This article describes its history along with its indications, surgical technique, and possible complications.

Mastectomy and sentinel lymph node biopsy in a patient with left-sided breast cancer and Poland syndrome.

This case focuses on breast cancer surgery in the setting of Poland syndrome (PS), a rare congenital condition characterised by unilateral absence or hypoplasia of the pectoral muscles. The literature describing breast cancer in patients with PS is limited. We present a case of an elderly woman with left-sided PS in whom breast cancer was incidentally detected during routine surveillance imaging for malignant melanoma. Imaging and biopsy confirmed a small invasive carcinoma, and the patient underwent mastectomy with sentinel lymph node biopsy. Intraoperatively, the absence of the pectoralis ma

MRI severity of common extensor origin damage associated with prior glucocorticoid injection in chronic lateral epicondylitis: a retrospective multicenter analysis.

BACKGROUND: Lateral epicondylitis (LE) is a degenerative tendinopathy of the common extensor origin (CEO). Although local glucocorticoid injections are widely used for symptom relief, concerns exist regarding their potential adverse effects on tendon structure. This study aimed to investigate the association between prior glucocorticoid injection and magnetic resonance imaging (MRI)-based structural CEO damage in patients with chronic LE. METHODS: In this retrospective, multicenter cross-sectional study, 147 patients with clinically diagnosed LE (symptom duration ≥ 6 months) who underwent elb

Graph theory-based machine learning for automated fibromyalgia diagnosis using fMRI during an emotional task.

BACKGROUND: Fibromyalgia (FM) is a multifaceted chronic pain disorder presenting with pain throughout the musculoskeletal system, alongside chronic fatigue, sleep disturbances, and cognitive impairments. Current diagnostic criteria primarily rely on symptom questionnaires, with no specific laboratory markers or definitive tests, making diagnosis subjective and often delayed. Moreover, the overlap with other central pain syndromes further complicates accurate diagnosis. Neuroimaging studies, particularly functional magnetic resonance imaging (fMRI), have shown promise in elucidating brain abnorm

Three-dimensional spinal and pelvic posture in professional saxophone players.

OBJECTIVES: The psychophysical demands of playing a musical instrument at a professional level pose significant challenges to the musculoskeletal system. Maintaining a position adapted to the instrument for long hours, every day and for many years may result in future postural abnormalities and chronic health problems. This study evaluated the 3-dimensional spinal and pelvic posture of professional saxophone players compared to non-musician controls. MATERIAL AND METHODS: The study included 60 men aged 20-30 years: 30 professional saxophone players with an average playing experience of mean (

Condensates of the chromatin regulator ANKRD11 restrict hypertranscribed genes to safeguard development.

Genes that define cell identity are often highly transcribed, but how their activity is regulated to prevent pathological dysregulation remains elusive. Using a two-tiered genetic screen, we identify a network of genes regulating hypertranscribed loci, which is notably enriched for genes mutated in developmental disorders. Among these, ANKRD11, a chromatin regulator haploinsufficient in KBG syndrome, exerts progressively stronger repression on more highly transcribed genes. ANKRD11 enriches around hypertranscribed loci and forms biomolecular condensates via charge-block-patterned intrinsically
