BACKGROUND AND AIMS: MEK1/2 inhibitors are increasingly used in neurofibromatosis type 1 (NF1) and can cause cancer therapy-related cardiac dysfunction (CTRCD). Cardiotoxicity rates of up to 12% have been reported in combination therapy, and up to 7% in monotherapy trials. Cardiac monitoring is recommended, but data on trametinib in NF1 remain limited. Therefore, we evaluated the incidence, timing, and clinical characteristics of CTRCD in adults with NF1 receiving trametinib monotherapy.
METHODS: The phase II single-arm TRAIN trial included NF1 patients receiving trametinib 2 mg daily. Serial

Von Hippel-Lindau disease (vHL) predisposes to tumor development, mainly clear cell renal carcinoma and hemangioblastoma. The underlying cause is germline variants in the VHL gene, with tumorigenesis thought to require additional somatic 'second-hit' events that most commonly include loss of 3p. However, the precise mechanisms of vHL-related tumor development remain incompletely understood. Genomic investigations of familial hemangioblastoma may help elucidate the early steps of tumorigenesis and contribute to improved disease prediction, biomarker discovery, and therapeutic strategies. BACKGROUND: Randomized controlled trials are the gold standard for demonstrating treatment efficacy. When infeasible, external control arm (ECA) analysis is an effective way to interpret treatment arm results. We developed an ECA for a single-arm trial (LS-004) for a HIF-2α inhibitor (belzutifan) in 61 patients with VHL renal cell carcinoma (RCC) to help interpret results. With a median follow-up of 37.8 months in LS-004, the ORR was 64% (95% CI = 50.6 to 75.8); median time to surgery was not reached.
METHODS: The ECA was developed using natural history study data for VHL RCC patients undergo

The neurofibromin 1 (NF1) splice-site mutation c.61-2A>G (rs1131691100) is a rare, pathogenic, autosomal dominant variant that disrupts NF1 tumor-suppressor function, causing neurofibromatosis type 1 (NF1). Its pathogenic mechanism is poorly understood, and the potential for personalized therapeutic genome editing remains unknown due to the absence of a standard framework for investigating splicing disorders. Here, we performed a comprehensive multi-omics analysis of a de novo c.61-2A>G case from South Korea, integrating short- and long-read whole genome sequencing, whole transcriptome sequenc

Sturge-Weber syndrome (SWS) is a rare and congenitally acquired neurocutaneous disorder characterized by port wine stain, leptomeningeal angiomas with or without glaucoma. Clinical presentation includes focal motor epilepsy as the first symptom. They may also develop migraines, cognitive impairment, blindness secondary to glaucoma, and transient stroke-like episodes. We report a case of a 60-year-old patient with SWS with known leptomeningeal angiomatosis presenting with aphasia in the setting of left middle cerebral artery narrowing who received intravenous tissue plasminogen activator (IV tP

BACKGROUND: Smartphone ecological momentary assessment (Smart EMA) has the potential to address limitations in standardised cross-sectional self-report assessments. Young adults with rare genetic conditions, including Tuberous Sclerosis Complex (TSC), may further benefit from Smart EMA as it may help overcome associated cognitive difficulties in self-report and mental health stigma. Framework analysis was used to explore the hypothetical acceptability of the use of Smart EMA in monitoring the daily experiences of young adults with TSC. RESULTS: 14 young adults with TSC (Female = 9, age range 1

Schwann cells (SC) are the principal glia in the peripheral nervous system, supporting normal peripheral nerve function, particularly in the myelination of motor and sensory axons of the peripheral nerves. In Neurofibromatosis Type 1 (NF1), disruptions in normal SC function can lead to decreased motor function, sensory capability, and neuropathic pain. There is no cure for NF1 and any available treatments are for symptom management such as surgical removal and drug therapies that do not always have consistent or favorable outcomes. As a result, there has been an increased interest in understan

BACKGROUND: Tuberous sclerosis complex (TSC) is a rare genetic disorder caused by pathogenic variants in the TSC1 or TSC2 genes. Apart from multisystem physical manifestations, most individuals with TSC experience TSC-associated neuropsychiatric disorders (TAND). Little is known about how TAND severity changes over time and what factors may predict these changes. Preliminary data suggest the presence of differential TAND severity trajectories. Caregiver well-being may act as a mediator of TAND severity, and a well-being intervention designed for caregivers of children with developmental disabi

AIM: Neurofibromatosis type 1 (NF1) is a common monogenic disorder with diverse clinical manifestations. Imaging is essential for diagnosis and follow-up yet international data reveal wide variability in imaging practices. This study aimed to evaluate current NF1 imaging practices across Romania, identify gaps in standardisation, and assess needs for alignment with international guidelines.
METHODS: A national, cross-sectional, questionnaire-based survey was conducted between March and May 2025 among Romanian imaging centres involved in NF1 diagnostics. Nineteen centres, both public and priva

Neurofibromatosis (NF) is a hereditary disease characterized by the formation and growth of benign tumors of neuroectodermal origin. There are three main nosological forms: NF1, NF2, NF3. It is believed that the disease debuts in childhood and significantly reduces the life expectancy of patients. The main cause of death in middle age are cardiovascular diseases and malignant neoplasms. There are isolated publications on NF in patients over 60 years old. The relationship between NF and cardiorenal syndrome has not been sufficiently studied. The development of vasculitis with vasculopathy is de

Neurofibromatosis type 1 (NF1) patients are predisposed to develop plexiform neurofibromas (PNFs). By cross-comparison of RNA sequencing and RUNX1-CHIP sequencing data on mouse PNFs, we found that transcripts encoding the NF1-interacting p97/valosin-containing protein (VCP) gene are overexpressed in PNFs. Co-immunoprecipitation confirmed that VCP bounded to neurofibromin. Western blot and immunostaining confirmed VCP overexpression in both mouse and human PNFs. Treatment of primary mouse PNF Schwann cells with CB-5083, a p97/VCP inhibitor, led to accumulation of poly-ubiquitinated proteins and

BACKGROUND: Plexiform neurofibromas (PNs) are observed in up to 50% of patients with neurofibromatosis type 1 (NF1). Trametinib has been used widely to treat PNs but limited data has been reported on its efficacy within a clinical trial. We conducted a phase 2 trial with trametinib for patients with NF1 and inoperable PNs.
METHODS: Patients with NF1 and inoperable PNs aged ≥1 month to ≤25 years when starting trametinib were eligible. Patients received trametinib once daily orally continuously for a maximum of 18 cycles of 28 days. Patients completed patient-reported outcome measures. Response

Neurofibromin, the protein product of the neurofibromatosis type 1 (NF1) gene, requires the direct binding interaction with SPRED to negatively regulate the RAS-MAPK pathway. Although the region of neurofibromin that stimulates the intrinsic GTPase activity of RAS represents only a small percentage of the entire protein, a large degree of the NF1 structural domains and their correlating mechanistic functions remain elusive. Here, we demonstrate RAS-independent biochemical and signaling functions regulated by the coordinate control of NF1 and SPRED1/2. Utilizing CRISPR-Cas9 methods to ablate NF

BACKGROUND AND OBJECTIVE: Renal cell carcinoma (RCC) is a common manifestation in Von-Hippel Lindau (VHL) syndrome, accounting for almost 30% of the patients. However, since VHL syndrome is relatively rare, there are no clear guidelines regarding the approach to VHL patients with RCC. This article aims to provide evidence-based insights for diagnosis, treatment, and surveillance in VHL patients with RCC.
METHODS: The articles indexed between 1977 and 2025 in PubMed was searched. Only articles in English were included.
RESULTS: Renal cysts are quite common in VHL patients and these cysts pred

BACKGROUND: Monogenic conditions, such as neurofibromatosis type 1 (NF1), provide a unique opportunity to study autism within a known molecular pathway. This study estimated the prevalence of autism in a large international NF1 cohort and examined how autistic traits relate to cognitive, behavioral, and adaptive functioning.
METHODS: In this cross-sectional study, 202 children with NF1 and 106 typically developing controls (aged 3-15 years) completed standardized cognitive testing and parent-reported behavioral measures. NF1 participants scoring ≥ 60 on the Social Responsiveness Scale-2 were

BACKGROUND: The pharmaceutical formulation of highly purified cannabidiol (CBD; Epidiolex) is approved by the the US Food and Drug Administration for the treatment of seizures associated with Lennox-Gastaut syndrome, Dravet syndrome, and tuberous sclerosis in patients one year of age and older. Among the large volume of drug-resistant cases treated at pediatric Level 4 Epilepsy Centers, many patients fall outside of these categories but are still prescribed CBD for off label use. This study sought to characterize the real-world experience with CBD of a large single pediatric comprehensive epil

Von Hippel-Lindau disease (VHLD) is a rare autosomal dominant disease, occuring in ~ 1 in 35,000 individuals. Overall, individuals with inherited mutations in the VHL tumor suppressor gene are predisposed to a variety of cancer, including high frequencies of clear cell renal cell carcinoma (ccRCC), pancreatic neuroendocrine tumors, and hemangioblastomas, as well as benign cystic conditions and other cancers. The degree of risk for each of these pathological conditions depends on the location and severity of the inherited germline mutation, and the specific VHL protein interactions and function
