Situs Inversus Totalis: A Case Report and Literature Review.

Situs Inversus Totalis (SIT) is a rare congenital anomaly characterized by the complete mirror-image transposition of the thoracoabdominal viscera. Although typically it has a favorable prognosis, SIT can be associated with ciliopathy-related disorders, most notably Primary Ciliary Dyskinesia (PCD). Consequently, challenges persist in the prenatal detection and subsequent genetic counseling for this condition. We present the case of a fetus diagnosed with Situs Inversus Totalis (SIT) via routine prenatal ultrasound and subsequent fetal echocardiography. Chromosomal Microarray (CMA) results wer

Dual renal pathologies in tuberous sclerosis complex: bilateral renal angiomyolipomas and concurrent left renal cell carcinoma.

A female patient in the 70s presented with bilateral flank pain and haematuria. Evaluation confirmed tuberous sclerosis complex (TSC) with cutaneous findings, a giant renal angiomyolipoma (AML) in the patient's left kidney with a bleeding pseudoaneurysm, and a large mass in the right kidney containing distinct AML and high-grade renal cell carcinoma tumours. Situs inversus totalis was noted. Transarterial embolisation controlled the haemorrhage from the left kidney, followed by open partial nephrectomy on the right kidney to address the malignancy while preserving renal function. Everolimus wa

Anaesthetic management of a paediatric patient with situs inversus totalis and congenitally corrected transposition of the great arteries undergoing adenotonsillectomy.

Situs inversus totalis (SIT) with congenitally corrected transposition of the great arteries (ccTGA) represents a rare and complex congenital combination that significantly complicates anaesthetic management. We report the anaesthetic management of an early childhood (2-5 years), 13 kg child diagnosed with SIT and ccTGA undergoing elective adenotonsillectomy. Individualised perioperative strategies were implemented, including modified ECG monitoring, arrhythmia preparedness and explicit mg/kg drug dosing. Intraoperative management was uneventful, and the patient had a stable recovery. This cas

Association of Single-Nucleotide Polymorphisms in Sweet Taste Perception and Intake Genes with Primary Ciliary Dyskinesia and Its Clinical Phenotypes.

Primary ciliary dyskinesia (PCD) is a congenital motile ciliopathy causing impaired mucociliary clearance and characterized by recurrent respiratory infections affecting both the upper and lower airways. Several genes involved in taste perception pathways are expressed in extraoral tissues and have recently emerged as regulators of airway immune responses. This study aimed to (1) analyze potential correlations between PCD clinical manifestations and (2) investigate whether genetic variants within sweet signaling genes (SweetG) could be associated with PCD features. A total of 17 SNPs in nine S

The Clinico-radiological Perspective of Kartagener's Syndrome.

Kartagener's syndrome is a rare autosomal recessive genetic disorder, a subset of primary ciliary dyskinesia. It comprises the triad of situs inversus totalis, bronchiectasis, and chronic paranasal sinusitis. The incidence is 1:30,000 live births, while the male: female ratio is 1:1. Most patients of Kartagener's syndrome present in early childhood with common symptoms of frequent colds, chronic cough with excessive production of purulent sputum, and recurrent lower respiratory tract infections affecting respiratory functions. Kartagener's syndrome commonly presents as infertility in males and

Breaking Anatomical Barriers: Drainage of a Pancreatic Pseudocyst in Situs Inversus-A Rare Coincidence.

BACKGROUND: Pancreatic pseudocysts are a frequent complication of pancreatitis, usually managed by endoscopic ultrasound (EUS)-guided drainage. Rare anatomical variants such as situs inversus may complicate endoscopic management. CASE HISTORY: A 45-year-old male with alcohol-related pancreatitis presented with early satiety and an epigastric bulge. Imaging revealed situs inversus with a large pseudocyst compressing the stomach. Endoscopic ultrasound-guided drainage was not feasible due to the unstable position in reversed anatomy. After unsuccessful attempts in left lateral and prone position

Ccdc57 regulates cilia and left-right patterning in Xenopus.

During embryogenesis, the establishment of left-right (LR) asymmetry depends on directional fluid flow generated by motile cilia within the left-right organizer (LRO). Disruption of this process can lead to laterality disorders such as situs inversus, heterotaxy, and congenital heart defects. Here, we identify CCDC57 as a regulator of ciliary function and LR patterning. Depletion of ccdc57 via morpholino oligonucleotides (MOs) led to abnormal cilia in the multiciliated cells of the embryonic epidermis of Xenopus. Additionally, LR markers, dand5 and pitx2c were misexpressed resulting in defects

Laparoscopic Sleeve Gastrectomy in a Patient With Situs Inversus: A Case Report.

Laparoscopic sleeve gastrectomy (LSG) is the most commonly performed bariatric surgery worldwide and is an effective treatment for obesity. Situs inversus (SI) is an uncommon congenital abnormality characterised by the anatomical location of organs within the body; it presents a rare technical challenge for bariatric surgeons due to the cognitive dissonance associated with operating on patients with mirror-image anatomy. Though even rarer, some of these patients may present with features of heterotaxy, which is defined by the abnormal arrangement of organs that comprises variants such as polys

Splenic Torsion in a Patient With Situs Inversus Totalis and Polysplenia: Challenging Diagnosis and Treatment of a Rare Case.

Splenic torsion is a rare condition caused by the twisting of the splenic pedicle, potentially leading to parenchymal infarction. We report a case of splenic torsion in a young patient with situs inversus totalis, isolated polysplenia, and a wandering spleen, without other cardiothoracic or digestive anomalies. An 18-year-old female, already known for situs inversus totalis, presented in the emergency department of a regional hospital with sudden right abdominal pain associated with vomiting. Laboratory tests showed leukocytosis and elevated C-reactive protein. Abdominal CT confirmed the situs

Novel Compound Heterozygous CFAP53 Variants in a Fetus With Situs Inversus Totalis: A Case Report.

Heterotaxy defects can include morphological deviations that affect left-right symmetry. In this case report, we observed novel compound heterozygous CFAP53variants that affect laterality. Whole exome sequencing (WES) was performed during pregnancy. Prenatal exome sequencing is particularly valuable for fetuses with laterality defects, given the increasingly high diagnostic success observed in this category of morphological abnormalities. Molecular testing plays an important role in genetic counselling, allowing parents to make decisions about pregnancy, calculating risk, and often predicting

First Case of Laparoscopic Cholecystectomy in a Patient with Situs Inversus Totalis in Ethiopia.

Situs inversus totalis (SIT) is a rare genetic condition that presents a significant challenge for laparoscopic surgery because of its mirror-image anatomy. While reported globally, this is the first documented case of laparoscopic cholecystectomy (LC) for symptomatic cholelithiasis in a patient with SIT in Ethiopia. A 43-year-old female with known SIT presented to the hospital with a 5-year history of dyspepsia and left upper quadrant abdominal pain. Ultrasound confirmed cholelithiasis. After a thorough preoperative workup to rule out associated syndromes, she underwent elective LC. A modifie

Loss-of-function variants in ODAD1 disrupt ODA docking and induce actin cytoskeletal remodeling in primary ciliary dyskinesia.

Primary ciliary dyskinesia (PCD) is a genetically heterogeneous disorder characterized by defective mucociliary clearance due to impaired motile cilia function. Outer dynein arm docking complex subunit 1 (ODAD1) encodes a structural component of the outer dynein arm docking complex (ODA-DC), which is crucial for effective ciliary beating. However, the full spectrum of ODAD1-associated pathogenesis remains incompletely defined. Here, we identified one novel homozygous loss-of-function ODAD1 variant, c.705_706insGCAG, and a recurrent homozygous splicing variant, c.-41-2A > C, in seven unrelated

Intralobar pulmonary sequestration in situs inversus: A rare presentation with recurrent hemoptysis.

BACKGROUND: Bronchopulmonary sequestration (BPS) is a rare congenital malformation of lung tissue characterized by the absence of normal bronchial connection and systemic arterial supply [1,2]. Situs inversus totalis (SIT) is an independent congenital anomaly in which thoracic and abdominal organs are reversed in mirror-image fashion [4]. The coexistence of BPS and SIT is exceedingly rare [9]. CASE PRESENTATION: We present a 51-year-old woman with SIT and BPS who developed massive hemoptysis. She had undergone prior embolization for intralobar sequestration (ILS) three years earlier. CT angio

Value of the Non-enhanced Cardiac Computed Tomography Scan Analysis in Determination of the Appropriate Coronary CT Angiography Scan Protocol.

OBJECTIVE: Dextrocardia refers to the right-sided positioning of the heart during embryonic development and may occur in isolation or in association with visceral malposition or other malformations. However, few studies have investigated this condition. This study aimed to determine the incidence of cardiac and non-cardiac malformations, as well as to analyze long-term follow-up and survival outcomes in patients with dextrocardia. METHODS: This was a retrospective chart review of dextrocardia cases at King Faisal Specialist Hospital & Research Centre in Riyadh, Saudi Arabia from April 22, 197

Two Distinct Clinical Presentations of Primary Ciliary Dyskinesia (PCD): Diagnostic Utility of Whole-Exome Sequencing in a Genetically Heterogeneous Disorder.

Primary ciliary dyskinesia (PCD) is a rare, genetically heterogeneous disorder with variable clinical presentation. In cases where traditional diagnostic tools such as transmission electron microscopy (TEM) or nasal nitric oxide (nNO) measurement are inconclusive/unavailable, molecular diagnostics via whole-exome sequencing (WES) may provide essential insights. In this case report, we present two male infants with diverse phenotypes and genotypes. 1st patient was ultimately diagnosed with PCD thanks to WES, while 2nd patient was strongly suggestive of PCD. The 1st patient portrayed in his clin

Structural and functional brain asymmetry in relation to heterogeneous causes of situs inversus totalis.

Various aspects of brain organization differ between the left and right hemispheres. Clues to the developmental origins of these asymmetries may be gained through associations with situs inversus totalis (SIT), a rare condition in which visceral organs are reversed on the left-right axis. In the largest previous brain imaging analyses of SIT (38 cases, 38 controls from Belgium), typical functional asymmetries such as left-hemisphere language dominance were mostly unaltered, but various aspects of asymmetrical cerebral structure - petalia, bending, and posterior venous anatomy—were often revers
