Fragile X messenger ribonucleoprotein 1 (FMRP) is a multidomain RNA-binding protein highly expressed in neurons. It comprises a structured N-terminal region (NTR) containing five RNA/protein interaction domains and a large intrinsically disordered C-terminal region. A limited number of fragile X syndrome (FXS)-associated point mutations have been identified in the NTR, leading to the expression of mutated protein variants. Here, we show that the destabilizing effects of these mutations on the NTR region are less severe than those previously observed in the isolated domains, suggesting the pres

Human induced pluripotent stem cells (hiPSCs), together with CRISPR editing, are commonly used for studying neurodevelopmental disorders. Female iPSCs gradually lose the expression of the noncoding X chromosome inactive-specific transcript (XIST), leading to random X chromosome inactivation (XCI) erosion that affects both X-linked and autosomal gene expression. However, the effects of CRISPR editing and neural differentiation on XCI erosion and how this may confound a real-world analysis of differentially expressed genes (DEGs) are poorly understood. Here, we analyzed hundreds of RNA sequencin

Down syndrome is caused by trisomy 21. Far less common is trisomy 21 mosaicism, in which only a proportion of cells carry the chromosomal abnormality. In this case report, first-trimester combined screening and non-invasive prenatal testing (NIPT) showed an increased risk of trisomy 21. However, chromosomal microarray performed after amniocentesis was normal and confined placental mosaicism was deemed most likely. After birth, the child showed slight stigmata for Down syndrome, and a blood sample confirmed the presence of low-frequency mosaicism at 6 months of age.

Mitochondrial dysfunction is a core cellular abnormality in Down syndrome (DS), yet the mechanisms underlying impaired mitochondrial quality control remain poorly defined. Here, we provide an integrated characterization of the autophagy-mitophagy-lysosome axis in human trisomic fetal fibroblasts (DS-HFFs). We show that trisomic cells display a paradoxical combination of elevated basal autophagy/mitophagy-related markers and defective mitochondrial clearance, indicative of a saturated and inefficient quality control system. DS-HFFs exhibit increased steady-state levels of PINK1, PARKIN, OPTN, N

46,XX testicular disorder of sex development (DSD), also known as XX male syndrome, is a rare condition where an individual with a 46,XX karyotype develops a male phenotype. In 90% of cases, the syndrome results from the translocation of the SRY gene to the X chromosome or an autosome. The remaining 10% of individuals lack the SRY gene. We report a case of an SRY-negative 3-year-old boy with a 46,XX karyotype exhibiting normal male external genitalia. Array CGH analysis revealed a 723-kb duplication of the Xq27.1q27.2 region, encompassing the SOX3 gene. The duplication was tandem. An imbalance

The DYRK1A (Dual-Specificity Tyrosine Phosphorylation-Regulated Kinase 1A) gene is located on human chromosome 21 (HSA21) within the Down Syndrome Critical Region (DSCR). As a protein kinase, DYRK1A is involved in the regulation of the cell cycle, cell growth, transcription, and cellular communication. It also plays essential roles in embryonic and neuronal development, and in disease pathogenesis. Dysregulation of DYRK1A has been associated with several human disorders, including DYRK1A syndrome, caused by DYRK1A haploinsufficiency (i.e., one functional copy of the gene) and Down syndrome (DS

Fragile X-associated tremor ataxia syndrome (FXTAS) is a multi-faceted and progressive neurodegenerative disorder that manifests clinically with action tremor, gait ataxia, parkinsonism, peripheral neuropathy, and cognitive and psychiatric features. The disease is highly heterogeneous and there are no FDA approved medications for FXTAS as a specific indication. As such, treatment is symptomatic and must be tailored to the specific needs of a given patient. This review provides expert opinion-based treatment recommendations for the core and ancillary clinical features of FXTAS. Movement disorde

DiGeorge syndrome is associated with a wide spectrum of cardiovascular anomalies. We present a rare and anatomically complex arch anomaly in this context. A male newborn with prenatally diagnosed DiGeorge syndrome and a right aortic arch was suspected of having a left pulmonary artery arising from the ascending aorta. Postnatal multimodal imaging revealed a discontinuous left pulmonary artery originating anomalously from the left common carotid artery via a left-sided ductus arteriosus, which later developed significant stenosis. To preserve left pulmonary artery flow and promote its growth, p

Branchial cleft anomalies are the second most common congenital neck lesion. They present as cysts, sinuses, or fistulas arising from remnants of the pharyngeal apparatus; second arch anomalies predominate. The embryologic origin determines their anatomical relationships to nerves, vessels, and the pharynx and guides diagnosis and surgical planning. Typical findings are cervical swelling, cutaneous discharge, or recurrent infection. Third and fourth branchial anomalies may cause recurrent neck abscesses or acute suppurative thyroiditis, particularly in children. Ultrasonography is the primary

X-linked hypohidrotic ectodermal dysplasia (XLHED; Christ-Siemens-Touraine syndrome, ICD-10: Q82.4) is the most common type of ectodermal dysplasia, caused by genetic disorders of ectodermal derivatives. The condition significantly impacts patients' quality of life, presenting with a classic triad of hypohidrosis, hypotrichosis and hypodontia, which creates a risk of impaired thermoregulation and necessitates complex dental rehabilitation. The molecular basis of the disease lies in alterations of the EDA gene, which encodes ectodysplasin A. This protein belongs to the tumor necrosis factor (TN

Excessive telomere shortening contributes to pulmonary fibrosis (PF), with at least 30% of familial cases attributed to telomere biology disorders (TBDs). However, TBD-associated PF remains underdiagnosed because of unidentified disease-causing mutations and limited referral for telomere and genetic testing. Here, we combined telomeric restriction fragment analysis, telomere analysis by a long-read Nanopore sequencing method (NanoTelSeq), exome sequencing (ES), and optical genome mapping (OGM) to investigate the underlying cause of disease in a family affected by PF, microcephaly, and prematur

Chromosomal aberrations, including aneuploidies and structural rearrangements, are key drivers of cancer and numerous genetic disorders. Experimental modeling of these alterations is essential for understanding their role in disease initiation and progression. Historically, chromosome engineering relied on methods that lacked precision and could generate off-target genomic damage. Recent advances in genome editing have enabled the precise generation of defined chromosomal abnormalities in human cells. This review summarizes current approaches for modeling numerical and structural chromosomal a

PURPOSE OF REVIEW: Living donor liver transplantation (LDLT) increasingly depends on biologically related donors, particularly in Asia and the Middle East, where deceased donation remains limited. This review evaluates genetic risks in living donors and proposes a system-based approach to donor selection.
RECENT FINDINGS: Heterozygous carriers exhibit variable functional capacity determined by enzyme thresholds and epigenetic mechanisms such as X-chromosome lyonization. In ornithine transcarbamylase deficiency (OTCD), maternal donors may have insufficient enzymatic activity, resulting in dono

Background: Neuropeptide Y is a highly conserved neuropeptide involved in several physiological processes, including stress regulation, appetite control, and cognitive functioning. Emerging evidence suggests that alterations in neuropeptide Y signaling may contribute to neurodevelopmental and behavioural phenotypes. This study describes a family carrying a microduplication involving the neuropeptide Y gene on chromosome 7p15.3, with the aim of exploring its potential association with attention deficit/hyperactivity disorder and specific cognitive vulnerabilities, particularly in the memory dom
