Missing what's right under your nose: Failed appetitive and aversive audio-olfactory conditioning in humans.

The comparison of physiological mechanisms underlying appetitive and aversive conditioning is often challenging due to the involvement of stimuli from different modalities with potentially disparate effective mechanisms (e.g., pain stimuli versus monetary rewards). The olfactory system offers a unique opportunity to examine both types of conditioning in humans, as isointense odors can serve as comparably pleasant and unpleasant stimuli. To study physiological and behavioral responses during appetitive and aversive learning, we employed odors as unconditioned stimuli (US) in a within-subjects d

Assessment of remote digital auscultation for identifying significant aortic stenosis: A pilot study.

BACKGROUND: Aortic stenosis (AS) is the most common valvular disease in the Western world. Technological advances, such as the TytoCare™ device, have introduced remote telemedicine-based cardiac auscultation. Its utility in diagnosing and grading AS remains unexplored. This study aims to evaluate the reliability of the TytoCare™ for assessing AS. METHODS: Sixty patients were classified by standard echocardiogram into three groups: significant AS (at least moderate AS; n=20), non-significant AS (aortic sclerosis or mild AS; n=20) and controls (n=20). Heart sounds were self-recorded using the T

A dual branch feature extraction network for heart sound signal analysis.

The analysis of heart sound signals is critical in the early diagnosis of cardiovascular disease. However, the complexity and diversity of these signals pose significant challenges for accurate recognition. In this paper, we propose a novel heart sound dual-branch feature extraction network (HSDFE-Net) to address these challenges. The model first extracts both conventional audio features and bi-spectrum features from preprocessed heart sound signals, enabling comprehensive characterization of their linear spectral and nonlinear properties. Unlike conventional single-branch networks, HSDFE-Net

An Uncommon Cause of Respiratory Distress: Ruptured Tension Gastrothorax.

A gastrothorax occurs when the stomach becomes displaced into the thoracic cavity. Rarely, this gastrothorax can rupture, which can lead to respiratory distress, hemodynamic compromise, and cardiac arrest. Here, we discuss the case of a 69-year-old male who presented to the emergency department (ED) for evaluation of generalized weakness and shortness of breath without associated upper respiratory symptoms. On physical exam, the patient was hemodynamically unstable with absent lung sounds on the left. A portable chest X-ray (CXR) was obtained, which initially revealed concern for tension hydro

Haemorrhagic Pericardial Effusion Following Intrapleural Fibrinolysis Therapy With Alteplase/Dornase Alfa for Complicated Parapneumonic Effusion: A Case Report.

Standard treatment for complicated parapneumonic effusion is antibiotic therapy and drainage of the infected pleural fluid. Complicated effusions refractory to standard treatment require further interventions such as intrapleural fibrinolysis. This is effective for patients with loculated effusions as local fibrinolytics administered via a chest drain can break down the septations. Common complications include intrapleural bleeding, hypersensitivity, pain and discomfort with fibrinolytics, but no studies, to our knowledge, have reported pericardial haemorrhage. In this case report, we describe

A Forcecardiography dataset with simultaneous SCG, Heart Sounds, ECG, and Respiratory signals.

FOSTER is the first ever publicly available dataset of forcecardiography (FCG) signals with simultaneous recordings of conventional seismocardiography (SCG), phonocardiography (PCG), electrocardiography (ECG), and respiratory signals. The dataset contains recordings from 40 participants (20 males and 20 females) and aims to foster and facilitate research on non-invasive cardio-respiratory monitoring using mechanical sensors. All signals were acquired simultaneously to ensure precise temporal alignment for accurate analysis. Each recording lasts about 7 minutes and includes both long phases of

SpectroNet-LSTM: An interpretable deep learning approach to cardiac anomaly detection through heartbeat sound analysis.

Cardiac anomalies are severe and life-threatening, making early detection essential to reducing health risks and mortality. According to the European Society of Cardiology, over 13 million people suffer from heart valve diseases annually, often identified by heartbeat anomalies. Traditional diagnostic methods depend on specialized expertise and advanced equipment. This paper proposes SpectroNet-LSTM, an automated framework for detecting cardiac anomalies from a comprehensive dataset of heartbeat sound recordings. Mel-frequency cepstral coefficients (MFCCs) and spectrogram analysis are used to

A Rare Case of Isolated Left Pulmonary Agenesis in an Adult Male Patient.

Pulmonary agenesis is a rare congenital condition where there is no development of pulmonary tissue beyond the carina. One or both lungs may be involved, but bilateral involvement is not compatible with extra-uterine existence. If one lung is involved, it is most commonly the left side. However, right lung involvement is more frequently associated with other inborn anomalies and severity in presentation. Here, we report a 19-year-old male patient having complete agenesis of the left lung, presenting with recurrent episodes of nonspecific cough, without fever and dyspnea on exertion. His vitals

The role of C-reactive protein and albumin combined ındexes in predicting the need for ıntubation in acute heart failure: A prospective observational study.

OBJECTIVE: Heart failure is a clinical syndrome, caused by functional or structural dysfunction of the heart. Taking early decisions for interventional procedures and predicting the treatment process will be beneficial for patients and will shorten the waiting times for the emergency department. We evaluated the capability of C-reactive protein (CRP) and albumin combined indexes to predict the need for intubation in acute heart failure. METHODS: This study was designed prospectively. Patients with signs and symptoms such as weakness, shortness of breath, abnormal findings in lung sounds, jugu

Quantum LBP-driven heart sound analysis with quality assessment in real-world noisy environments.

BACKGROUND AND OBJECTIVE: Cardiovascular disease (CVD) is a major global health concern with increasing prevalence. While electrocardiography (ECG) and echocardiography offer high accuracy, their reliance on specialized equipment and trained personnel makes them costly and less accessible in rural areas. In contrast, the Phonocardiography (PCG) provides a more affordable alternative via capturing heart sound signals using a stethoscope. However, PCG analysis is often compromised by environmental noise. Most existing methods address this issue with denoising techniques, which can inadvertently

Acoustic Intensity as a Potential Indicator for Congestive Heart Failure Exacerbation: An Exploratory Pilot Study.

Background: Congestive heart failure (CHF) requires continuous monitoring, especially during exacerbation phases. Pulse oximetry, commonly used for critical patient surveillance, has shown diagnostic potential in acute heart failure. In this exploratory pilot study, we investigated oxygen saturation (SpO2) data and respiratory sounds from CHF patients to uncover their relationship and to assess the potential of respiratory sound intensity in telehealth or self-monitoring systems, with a view toward future predictive applications. Methods: The relationship between SpO2 and acoustic intensity wa

Incidental Detection of Situs Inversus Totalis During Preoperative Evaluation for Thyroid Cancer: A Case Report.

Situs inversus totalis (SIT) is an uncommon congenital condition in which the thoracic and abdominal organs are arranged in a mirror-image orientation. While often asymptomatic, undiagnosed SIT can complicate clinical evaluation, imaging interpretation, and surgical planning. We present the case of a 60-year-old woman with a palpable neck mass diagnosed as papillary thyroid carcinoma. During the preoperative assessment, a physical examination revealed right-sided heart sounds, and an electrocardiogram showed a right axis deviation with inverted P waves. Chest radiography and abdominal imaging

Improved peaks automatic detection algorithm of noisy quasi-periodic physiological signals.

The automatic multiscale-based peak detection (AMPD) method for quasi-periodic signals suffers from high computational complexity, substantial memory requirements, and occasional peak detection failures. To address these limitations, this paper proposes an Improved AMPD (IAMPD) algorithm that significantly enhances computational speed while maintaining strong noise robustness. The improvement in efficiency is attained through a frequency-informed scale constraint that narrows the search space using prior knowledge of the signal's frequency range, greatly reducing redundant computations. This i

Raw fetal PCG dataset contaminated with Mother's PCG.

This article presents a dataset of raw fetal phonocardiogram (fPCG) signals that naturally include contamination from maternal heart sounds. Recordings were conducted with eight pregnant participants, most of whom were at 36 weeks or later gestation, and one participant at 32 weeks. The data were collected in the Biomedical Instrumentation and Signal Processing Laboratory at Independent University, Bangladesh, using a simple non-invasive device: a stethoscope coupled with a microphone. At the same time, Doppler ultrasound and a pulse oximeter were used to obtain reference values of the fetal a
