BACKGROUND: Cystic echinococcosis (CE) is a parasitic zoonosis, caused by the larval stage of Echinococcus granulosus, typically forming cysts in the liver. If untreated, CE can be life-threatening. Albendazole remains the standard medical therapy, but surgical intervention is indicated in selected cases.
METHODOLOGY/PRINCIPAL FINDINGS: We conducted a retrospective, multicenter outcome analysis in Switzerland of surgically treated cystic echinococcosis from 2004 to 2024. Patients with CE stages 1-3b undergoing pericystectomy, endocystectomy, or non-resectional procedures were included. The pr

OBJECTIVES: Hepatic cystic echinococcosis (hepatic CE.) remains endemic in many regions, with severe complications and even death. Current treatments face limitations in efficacy and accessibility, while microwave ablation (MWA) represents a promising minimally invasive alternative, but real-world multicenter evidence is lacking.
METHODS: This retrospective multicenter study included patients with imaging-confirmed viable hepatic CE (WHO-IWGE CE1-CE3) who underwent ultrasound-guided aspiration-assisted percutaneous MWA at three tertiary hospitals in China between 2016 and 2024. Primary outcom

BACKGROUND: Bronchobiliary fistula (BBF) is a rare acquired communication between the biliary system and the bronchial tree. Biliptysis (bile-stained sputum) is the most characteristic clinical clue and should prompt evaluation for underlying hepatobiliary pathology, particularly in endemic regions where echinococcosis remains prevalent. CASE PRESENTATION: A 35-year-old Arab male presented with recurrent biliptysis, right upper quadrant pain, nausea/vomiting, reduced appetite, and mild exertional dyspnea. He had a history of hepatic hydatid cyst surgery 18 years earlier followed by albendazole

Alveolar echinococcosis (AE) caused by Echinococcus multilocularis, is a rare but potentially fatal zoonotic infection. AE most commonly presents with multiple cysts in the liver, may spread to other organs through infiltration or metastasis and can mimic malignancy with fatal consequences if not diagnosed early. Its occurrence in childhood is extremely rare and simultaneous hepatic and pulmonary involvement has been reported only in a few cases. This report aimed to discuss a pediatric AE case with concurrent hepatic and pulmonary involvement, initially misdiagnosed as malignancy, in the ligh

BACKGROUND: The C-C motif chemokine ligand 2 (CCL2)-C-C chemokine receptor type 2 (CCR2) signaling axis is critically involved in angiogenesis and cellular invasion; however, the therapeutic potential of its targeted blockade on alveolar echinococcosis (AE) remains largely unexplored. This study aimed to determine whether RS504393, a selective CCR2 antagonist, can inhibit the progression of AE by blocking the CCL2-CCR2 axis and modulating its downstream pathogenic mechanisms.
METHODS: In this study, bioinformatics analysis of the GSE124362 dataset was combined with molecular docking simulatio

INTRODUCTION: To explore the distribution characteristics of sympathetic and vagal nerves in hepatic alveolar echinococcosis (AE). By analyzing pathological specimens from patients, we focused on the differential distribution of nerves in lesions, perilesional zones, and normal liver tissues and revealed their correlation with clinical local staging, hepatic fibrosis and regenerative processes. METHODS: Pathological specimens from 74 patients diagnosed with hepatic AE were analyzed. The differential distribution of sympathetic and vagal nerves was examined in three distinct tissue regions: the

PURPOSE: Hepatopulmonary hydatidosis (HPH) is a clinically relevant presentation of hepatic cystic echinococcosis (CE) in which pulmonary involvement is present at the time of diagnosis. Reliable identification of patients at risk remains challenging, and indiscriminate thoracic imaging may lead to unnecessary investigations. This study aimed to identify hepatic predictors associated with concomitant pulmonary involvement and to develop a simple risk stratification model to support selective thoracic imaging.
METHODS: We conducted a retrospective cohort study of patients with confirmed hepati

Hydatid cyst disease is a parasitic disease induced by Echinococcus granulosus, which is still endemic in northern African countries. We report the case of a late-discovered pulmonary hydatid cyst in a four-year-old boy revealed by recent dyspnoea. The chest X-ray showed a complete white-out of the left hemithorax with mediastinal deviation, and the thoracoabdominal CT scan showed a giant pulmonary hydatid cyst encompassing the entire left lung associated with a hepatic cyst. An emergent superior pulmonary lobe resection was performed under deep general anaesthesia and selective intubation to

Cystobiliary communication (CBC) is a common complication of hepatic hydatid cysts, increasing surgical complexity and postoperative morbidity. Reliable preoperative prediction and intraoperative detection of occult CBC remain challenging. We retrospectively analyzed 106 patients undergoing surgery for hepatic hydatid disease. Patients were classified as CBC or non-CBC based on intraoperative findings. Clinical, laboratory, radiologic, and operative variables were evaluated. Multivariate logistic regression identified independent predictors, and Receiver Operating Characteristic (ROC) analysis

OBJECTIVES: Hepatic cystic echinococcosis remains endemic in many regions, and surgery is the mainstay of treatment. However, postoperative recurrence remains a significant concern. This study aimed to identify predictors of recurrence after surgical management.
METHODS: We conducted a retrospective study including patients who underwent surgery for hepatic hydatid cysts between 2010 and 2022 in a tertiary center in Tunisia. Clinical, radiological, and operative variables were analyzed. Recurrence-free survival (RFS) was estimated using the Kaplan-Meier method and compared with the log-rank t

INTRODUCTION: Waldenström macroglobulinaemia is a lymphoplasmacytic lymphoma producing monoclonal immunoglobulin M (IgM), and both the disease and its treatment can cause immunosuppression. Alveolar echinococcosis is a parasitic infection that primarily involves the liver but may present with extrahepatic manifestations.
CASE DESCRIPTION: We report the case of a patient who presented with chylothorax and non-specific hepatic lesions and was then diagnosed with Waldenström macroglobulinaemia. In addition to this diagnosis, alveolar echinococcosis was subsequently confirmed by liver biopsy and

Multilocular (alveolar) hepatic echinococcosis is an uncommon parasitic disease that may closely resemble malignant hepatic tumors because of its infiltrative growth pattern and complex internal architecture. The condition is rare in India and is therefore frequently misdiagnosed, particularly in pediatric patients. Early and accurate diagnosis is essential, as treatment strategies differ markedly from those of hepatic malignancies and pyogenic infections. We report the case of a 13-year-old boy who presented with a four-month history of intermittent fever and right upper abdominal pain. Initi

Percutaneous treatments (PTs) are established minimally invasive options for the management of hepatic cystic echinococcosis (HCE). Conventional techniques include puncture, aspiration, injection, and re-aspiration (PAIR), which relies on scolicidal agents, and puncture, aspiration, injection, and drainage (PAID), which uses prolonged catheter drainage to evacuate cyst contents. In this study, we describe our experience with a modified PAIR technique for the treatment of hepatic cystic echinococcosis. Eight patients with 19 cysts were initially included, and outcomes were analyzed in 6 patient

BACKGROUND: Alveolar echinococcosis (AE), caused by Echinococcus multilocularis, is a lethal zoonotic disease. Population screening for early diagnosis is crucial for effective treatment, and novel diagnostic antigens are needed for diagnostic purposes.
METHODS: E. multilocularis serine protease inhibitor 1 (EmSPI-1) was cloned and expressed in Escherichia coli. Quantitative real-time PCR (qRT-PCR) and Western blotting were used to assess gene expression at different life-cycle stages of the tapeworm. The diagnostic potential of recombinant EmSPI-1 (rEmSPI-1) was evaluated by enzyme-linked im

Background: Cystic echinococcosis (CE) is a chronic zoonotic parasitic disease with a significant impact on public health in endemic regions. The liver is the most frequently affected organ, and ultrasound-based surveys are considered a reliable tool for detecting asymptomatic infections. As population-based data specifically addressing hepatic CE prevalence in Türkiye remain limited, we aimed to assess the prevalence of liver cystic echinococcosis in Türkiye using only ultrasound-based surveys. Methods: A systematic review was conducted, in accordance with the PRISMA guidelines, to estimate t

INTRODUCTION: Hydatid disease is a zoonotic parasitic infection caused by the larval stage of Echinococcus species. While hepatic and pulmonary involvement is common, primary peritoneal hydatidosis is exceedingly rare, representing less than 2% of intra-abdominal cases. Due to its rarity and nonspecific presentation, primary peritoneal hydatidosis may mimic other intra-abdominal cystic lesions, making diagnosis challenging.
CASE PRESENTATION: We report a 28-year-old male with no prior hepatic or pulmonary hydatid disease who presented with progressive abdominal distension and lower abdominal

Cystic Echinococcosis (CE) is a major helminthic zoonosis caused by the larval stage of Echinococcus granulosus, particularly highly endemic in several Mediterranean regions, including Algeria. Humans are accidental hosts, while animals such as livestock act as intermediate hosts. This study aimed to update the fragmented epidemiological picture of CE in Algeria, by combining the knowledge on cysts fertility, and protoscolex viability with molecular strains assignment. In particular, the genetic typing of Echinococcus granulosus from humans (n = 16), cattle (n = 15), sheep (n = 15), and goat (
