Intraoperative Ultrasound Guided Sling Adjustment During Transobturator Sling Placement: A Preliminary Pilot Report of the Technical Protocol and Feasibility.

OBJECTIVES: To describe a qualitative intraoperative ultrasound protocol for transobturator mid-urethral sling adjustment and assess technical feasibility. METHODS: This retrospective pilot series included 58 consecutive women with urodynamically confirmed stress urinary incontinence or stress-predominant mixed urinary incontinence undergoing outside-in transobturator sling placement. Sagittal and axial images guided gentle traction for a visible tape-urethra gap and tension release for urethral encroachment. Sling-arm oscillation supplemented assessment when acoustic shadowing limited visual

Association Between Dynamic Changes in the Sling-Pubis Gap and Urinary Symptoms After Tension-Free Vaginal Tape Surgery: Protocol for a Prospective Observational Pilot Study.

BACKGROUND: Stress urinary incontinence (SUI) is one of the most common pelvic floor disorders in women and is frequently treated surgically using midurethral sling procedures. Despite the high success rates of these procedures, postoperative voiding dysfunction remains a clinically relevant complication that may negatively affect patient satisfaction and quality of life. The underlying mechanisms are multifactorial and not completely understood. In particular, the interaction among sling position, urethral mobility, and pelvic floor dynamics during increases in intra-abdominal pressure may in

Case Report: Solitary fibrous tumor of the pelvis associated with Doege-Potter syndrome.

INTRODUCTION: Solitary fibrous tumor (SFT) is an uncommon mesenchymal spindle cell neoplasm (incidence ~1.4 per million). Although most SFT occur in the thoracic cavity, they can also arise in the retroperitoneum, pelvis, head, and neck. While the majority exhibit benign behavior, approximately 5% are associated with paraneoplastic syndromes, most notably Doege-Potter syndrome (DPS). DPS is characterized by non-islet cell tumor hypoglycemia (NICTH) resulting from tumor hypersecretion of high-molecular-weight insulin-like growth factor 2 (IGF2), which saturates insulin and IGF receptors causing

Individualizing Diagnosis and Management of Stress Urinary Incontinence in Neurogenic Lower Urinary Tract Dysfunction.

PURPOSE OF REVIEW: Stress urinary incontinence (SUI), defined as involuntary leakage with effort, exertion, sneezing, or coughing, remains a significant cause of morbidity. While well characterized in the general population, SUI in the setting of neurogenic lower urinary tract dysfunction (NLUTD) poses unique diagnostic and management challenges. This review outlines sex-specific epidemiology, diagnostic strategies, and evolving management approaches for SUI in NLUTD, emphasizing individualized and mechanism-based care. RECENT FINDINGS: Incontinence prevalence varies by neurologic diagnosis,

Functional outcomes and quality of life after vesicovaginal fistula repair: beyond anatomic closure.

PURPOSE OF REVIEW: Vesicovaginal fistula (VVF) repair has been historically measured by anatomical closure rates alone. This review summarizes recent evidence showing that successful treatment should also account for urinary continence, sexual function, psychological well-being, and social reintegration, highlighting a multidisciplinary approach to post-repair recovery. RECENT FINDINGS: Although anatomic closure rates exceed 80-95% across surgical approaches, post-obstetric fistula repair incontinence affects 16-55% of women even after successful anatomic closure. The importance of CSF distribution for target values and risk profiling for idiopathic Normal Pressure Hydrocephalus shunt surgery.

BACKGROUND AND PURPOSE: Idiopathic Normal Pressure Hydrocephalus is characterized by gait disturbance, cognitive impairment, and urinary incontinence. Symptom improvement can be achieved by shunt surgery, especially regarding gait difficulties. This study investigated whether CSF volumetry can define postoperative target values associated with clinical improvement and, secondly, predict potential non-response or overdrainage complications. MATERIALS AND METHODS: Forty-seven patients with iNPH were prospectively followed for three years after shunt surgery. Quantitative MRI was performed at ba

Developing a Risk-Adjustment Model for Stress Urinary Incontinence Procedures Using Australasian Pelvic Floor Procedure Registry Data.

INTRODUCTION AND HYPOTHESIS: Surgical outcomes for stress urinary incontinence (SUI) procedures are influenced by factors beyond the surgeon's control. Using Australasian Pelvic Floor Procedure Registry data, this study is aimed at developing a risk-adjustment model for equitable benchmarking of clinician-reported outcomes for female SUI procedures. METHODS: Logistic regression was used to create multivariate models for clinician-reported outcomes of primary SUI procedures. Risk factors such as age, body mass index (BMI), diabetic status, and symptoms of bladder disease were selected for adju

The relationship between endometrial cancer and urinary incontinence: systematic review.

BACKGROUND: The most common gynecological cancer in industrialized countries is endometrial cancer (EC), whose prevalence is growing and is associated with metabolic syndrome and obesity. Women's quality of life is negatively impacted by the common and upsetting disorder known as urinary incontinence (UI), particularly in postmenopausal and post-surgical populations. Despite plausible biological and iatrogenic connections between EC and UI-including shared risk factors such as obesity and the impact of hysterectomy on pelvic anatomy-the relationship remains inadequately synthesized in the lite

An incidental heterozygous ATP7B nonsense variant leading to a diagnostic pitfall for Wilson disease: a pediatric case report.

BACKGROUND: Wilson disease (WD) is an autosomal recessive disorder caused by pathogenic variants in ATP7B, resulting in impaired copper transport and progressive copper accumulation, most prominently affecting the liver. With the growing use of genetic testing, incidental detection of ATP7B pathogenic/likely pathogenic variants in individuals without typical hepatic phenotypes can prompt overdiagnosis and unnecessary investigations. This case highlights the importance of interpreting a single heterozygous ATP7B variant in the context of phenotype and a standardized biochemical diagnostic pathw

Female genital lichen sclerosus.

Female genital lichen sclerosus (FGLSc) is a chronic inflammatory scarring dermatosis that can lead to scarring and irreversible architectural change, sexual and urological dysfunction and malignancy. Although the aetiology is not completely understood, current evidence supports a multifactorial pathogenesis involving immune dysregulation, genetic susceptibility, epithelial stress and aberrant fibrogenesis, with emerging evidence for chronic irritation and occluded exposure to urine in susceptible individuals. Clinically, FGLSc presents with pruritus, soreness, dyspareunia and progressive scar
