Multimodal Novel Recurrent Laryngeal Nerve Monitoring Techniques in Anterior Cervical Spine Surgery: Initial Experience and Proof of Concept.

Injury to the recurrent laryngeal nerve (RLN) during anterior cervical spine surgery is an uncommon yet impactful sequela. This study evaluates the feasibility and intraoperative behavior of laryngeal adductor reflex (LAR) and motor-evoked potentials with vocalis recordings (MEP-VC) during anterior cervical spine surgery. LAR and MEP-VC were employed for 13 patients undergoing anterior cervical spine surgery. LAR alerts were issued when clinically correlated reductions in amplitude, typically ≥20% in previously stable, non-habituating recordings, and/or temporal dispersion of the LAR waveform

Oxaliplatin-chemotherapy induced cochlear synaptopathy.

Oxaliplatin is widely used as a first-line and reference anticancer agent for advanced colorectal cancer. However, it frequently causes peripheral neuropathy. It is generally accepted that oxaliplatin is significantly less ototoxic than the other platinum-based antineoplastic drugs, cisplatin and carboplatin. This study aimed to evaluate potential hearing damage induced by oxaliplatin in young male CBA/JRj mice. Mice received intraperitoneal injections of oxaliplatin (3 mg/kg) twice weekly for 3 weeks, for a cumulative dose of 18 mg/kg. Auditory function was assessed using electrophysiological

Axonal protection by nicotinamide riboside with the SIRT1-AMPK pathway in TNF-induced optic nerve damage.

BACKGROUND: Nicotinamide riboside (NR) is an NAD+ precursor and is reported to upregulate NAD+ levels in several tissues. We previously demonstrated that NR exhibited protection against axon loss induced by tumor necrosis factor (TNF) with upregulation of SIRT1 and enhancement of autophagy. We recently found that NR upregulated phosphorylated adenosine monophosphate-activated protein kinase (p-AMPK) protein in the optic nerve. The present study aimed to examine whether a SIRT1 inhibitor prevents axonal protection by NR and alters p-AMPK expression. METHODS AND RESULTS: Rats were given intravit

Self-amplifying RNA therapy encoding CNTF with disulfiram co-delivery promotes optic nerve repair through microglial pyroptosis inhibition and RGC axonal regeneration.

BACKGROUND: Traumatic optic neuropathy (TON) is a devastating cause of irreversible vision loss for which no effective treatment currently exists. Its poor prognosis stems from two major challenges: the limited regenerative capacity of retinal ganglion cells (RGCs) and the hostile, inflammation-driven environment that follows injury. RESULTS: In this work, using transcriptomic bioinformatic and histopathological analysis, we discovered that mechanical trauma and subsequent neuroinflammation trigger microglial pyroptosis through the NLRP3/CASP1/GSDMD pathway. This process amplifies inflammator

VIsceral sheath targeted approach for left recurrent laryngeal nerve lymphadenectomy during robot-assisted minimally invasive esophagectomy (VISTA approach).

BACKGROUND: Lymphadenectomy along the left recurrent laryngeal nerve (RLN) is essential for curative esophagectomy but carries a high risk of RLN palsy, which impairs postoperative quality of life. Despite advances in robot-assisted minimally invasive esophagectomy (RAMIE), safe and radical dissection around the left RLN remains technically demanding. To achieve both oncological completeness and nerve preservation, we developed a novel Visceral sheath targeted approach (VISTA) based on precise anatomical understanding of the visceral and alar fasciae. METHODS: This retrospective observational

Iatrogenic Nerve Injuries in Head and Neck Surgeries: A Systematic Review of Mechanisms, Outcomes, and Prevention Strategies.

Iatrogenic nerve injury in head and neck surgery remains a substantial yet potentially preventable source of morbidity across endocrine, otolaryngologic, and related procedures. This systematic review synthesized evidence to identify the nerves most frequently affected, delineate operative mechanisms of injury, describe the clinical course, and evaluate preventive strategies. Traction on visually intact nerves emerged as the predominant mechanism, with additional contributions from thermal injury, compression, ischemia, and entrapment by suture or clip. The recurrent laryngeal, facial, trigemi

Mitochondria transplantation preserves retinal ganglion cells and promotes CNS axonal regeneration.

Mitochondrial dysfunction is a central driver of retinal ganglion cell (RGC) loss in glaucoma and other forms of optic neuropathies, leading to irreversible blindness. Here, we demonstrate that replenishing the mitochondrial pool through exogenous mitochondrial transplantation ("mitotherapy") in adult mice not only preserves neuronal survival but also promotes regenerative competence in the central nervous system (CNS). In aging or injured RGCs, we identified profound deficits in mitochondrial biogenesis, fission-fusion balance, and mitophagy. Transplantation of functional mitochondria in in v

Otopathology of Facial Nerve and Chorda Tympani Traumatic Neuromas.

HYPOTHESIS: Traumatic neuromas of the facial nerve are rare lesions that can mimic other tumors of the temporal bone and might be consequential to injuries of common otologic surgeries. Literature is scarce in describing these lesions in the temporal bone. The aim of this study is to expand the histologic understanding of these lesions. BACKGROUND: Traumatic neuromas are benign proliferations of neural structures after total or partial sectioning, representing what seems to be a reparative attempt. In otologic surgeries, the facial nerve and its branch chorda tympani are at risk of iatrogenic

Otoprotective effect of MnTBAP in cisplatin-induced hearing loss.

OBJECTIVE: Cisplatin, a life-saving chemotherapeutic drug, causes ototoxicity. Although sodium thiosulfate is used to prevent ototoxicity in pediatric patients, no other intervention has been approved for clinical use against cisplatin-induced hearing loss. Hence, there is an urgent need to identify drugs that prevent cisplatin ototoxicity. METHODS: CBA/J mice were treated with cisplatin (3 mg/kg, i.p., daily for 5 days), and MnTBAP (10 mg/kg, i.p., daily for 8 days) was used to inhibit cisplatin-induced ototoxicity. Quantitative CT Imaging Features Associated With High Risk of Elevated Intracranial Pressure in Children Under 2 Years of Age With Craniosynostosis: A Patient-Control Study.

OBJECTIVE: Elevated intracranial pressure (ICP) is an important complication of craniosynostosis, and early identification may help optimize surgical planning and treatment. Although invasive ICP monitoring remains the reference standard, its routine use in infants and young children is limited by procedural risks. This study aimed to evaluate whether quantitative computed tomography (CT) parameters could serve as noninvasive imaging markers associated with a high risk of elevated ICP in children under 2 years of age with craniosynostosis, compared with age- and sex-matched controls. METHODS:

Postinfectious polyneuritis cranialis: A case report.

Polyneuritis cranialis is characterized by the simultaneous or sequential inflammation of multiple cranial nerves, which may occur unilaterally or bilaterally. Although it is often related to infection, its exact etiology remains unclear. Due to its nonspecific clinical manifestations, diagnosis typically relies on the exclusion of other conditions. Herein, we report a case of postinfectious polyneuritis cranialis. The patient presented to our hospital with restricted mouth opening, dysphagia, coughing while drinking, dysarthria, and posterior neck pain following a finger injury. Laboratory te

Fate of intrastromal injected adipose-derived mesenchymal stem cell and its effects on corneal nerve regeneration: An in vivo confocal microscopy study.

Corneal nerve damage reduces corneal sensitivity and compromises ocular surface health. This study investigated the fate of adipose-derived mesenchymal stem cells (AD-MSCs) following intrastromal injection and evaluated their effects on nerve regeneration in a rabbit model of corneal nerve injury. AD-MSCs were first characterized for MSC surface markers using immunocytochemistry staining (IHC), as well as for multipotency with trilineage and neurogenic differentiation assays. The cells expressed CD73, CD90, and CD105, and successfully differentiated into chondrogenic, osteogenic, adipogenic, a

Gastroesophageal reflux-induced cough: the positive feedback loop of microaspiration and neurogenic inflammation.

Gastroesophageal reflux-related cough (GERC) is a common etiology of chronic cough, characterized by a complex pathogenesis. The limited efficacy of conventional acid suppression therapy suggests the presence of critical mechanisms that extend beyond mere gastric acid irritation. This review systematically explores the interaction between the two core pathways-"microaspiration" and "neurogenic inflammation"-in the context of GERC, aiming to elucidate their synergistic pathogenic mechanisms. On one hand, the gastric contents involved in microaspiration (such as gastric acid and pepsin) directly
