Concussion symptom clusters and changes in symptom reporting and severity following multiple concussions in male professional rugby league players.

BACKGROUND: Symptom reporting remains an important part of the diagnosis and management of concussion. Understanding the patterns of symptoms and symptom clusters in athletes with multiple concussions may be clinically valuable. The purpose of this study was to characterise acute symptom reporting in players with multiple concussions. METHODS: National Rugby League (NRL) players who have sustained multiple concussions between the 2017 and 2022 seasons were included in the study. Data included the symptoms reported on the Sport Concussion Assessment Tool - 5th Edition (SCAT5) during the Head I

Mahi tahi (working together) and the lessons learned from implementating a concussion management pathway for community rugby in New Zealand: Haerenga (our journey).

Concussion guidelines are widely available, but implementing them consistently in community sport remains difficult. Common barriers include limited access to medical care, reliance on volunteers, fragmented communication, and sporting cultures that may discourage reporting. In response, New Zealand Rugby developed and refined over multiple seasons, a community Concussion Management Pathway (CMP), to operationalise World Rugby's principles of 'Recognise and Remove' in real-world community settings. This editorial reflects on lessons learned from the design, implementation, and iterative refine

Physical examination of the cervical spine should be conducted in female athletes with a history of concussion.

BACKGROUND: Female athletes may be at increased risk of injury following sports-related concussion (SRC), even after symptom resolution. Residual cervical spine impairments have been proposed as a potential contributor; however, these have not been well characterised in female athletes. This study aimed to examine whether a history of SRC was associated with clinical assessments of the cervical spine in female athletes. METHODS: In this cross-sectional observational study, 156 elite and pre-elite female athletes from water polo, soccer, diving, rugby-7s and cycling/BMX were recruited. History

Metabolomics in Mild Traumatic Brain Injury: A Scoping Review.

Mild traumatic brain injury (mTBI) accounts for around 80-90% of TBI cases, is increasing in incidence, and represents a major public health concern. Despite its high incidence, mTBI lacks reliable molecular biomarkers for diagnosis or prognosis. Metabolomics, the profiling of small-molecule metabolites (≤1 kDa), has emerged as a promising approach to characterize acute neurometabolic perturbations in mTBI and identify candidate biomarkers. However, metabolomic changes in mTBI have not yet been comprehensively characterized. Following the Preferred Reporting Items for Systematic Reviews and Me

The impact of an app-delivered mindfulness-based intervention on resting-state functional connectivity in pediatric concussion: a pilot study.

RATIONALE: Concussion disrupts brain connectivity, and effective treatments remain limited. Mindfulness-based interventions (MBIs) may influence functional connectivity (FC) of networks affected by concussion, but pediatric trials are lacking. This study investigated whether an app-delivered MBI produced greater FC changes than a sham condition across the default mode (DMN), salience (SN), and central executive (CEN) networks. METHODS: This randomized clinical trial (NCT05105802) recruited participants aged 12.00-17.99 years from the emergency department or interdisciplinary concussion clinic

Estimating Linear and Rotational Head Kinematics in Ice Hockey from a Single Helmet-Mounted IMU Using Vector Autoregressive Models.

PURPOSE: Accurate estimation of head kinematics is critical for assessing brain injury risk in contact sports. This study introduces a Vector AutoRegressive (VAR) framework to jointly model linear acceleration and angular velocity during head impacts, capturing the inherent coupling between translational and rotational dynamics. METHODS: Controlled impacts were performed on a helmeted Hybrid III 50th percentile male headform using a pendulum impactor across multiple locations (front, front-oblique, side, rear-oblique) and three impact intensities (30°, 50°, 70° pendulum angles). Helmet-mounte

Primary caregiver employment status is associated with traumatic brain injury in children in the USA.

BACKGROUND: Traumatic brain injury (TBI) is a common injury in children. Previous literature has demonstrated that TBI may be associated with supervision level. We hypothesised that primary caregiver employment would be associated with child TBI. METHODS: A retrospective cross-sectional study was performed for children aged 0-17 using the National Survey of Children's Health (NSCH) 2018-2019. The NSCH contains survey data on children's health completed by adult caregivers from randomly selected households across the USA. We compared current TBI prevalence between children from households of d

Incidents and patterns of commotio cordis among athletes in the USA from 1982 to 2023.

INTRODUCTION: Commotio cordis is a rare event that occurs following blunt, non-penetrating trauma to the chest, precipitating a ventricular arrhythmia. Commotio cordis requires immediate medical attention through cardiopulmonary resuscitation and defibrillation, often resulting in death. Commotio cordis is most common condition among young male athletes. The purpose of this study was to describe the incidents and patterns of commotio cordis among young athletes participating in organised sports in the USA from academic years 1982-1983 through 2022-2023. METHODS: This was a retrospective, desc

A Geometry-Aware Bio-Inspired Sparse Reservoir With Online Learning.

The forensic physician is often faced with the diagnosis of concussion or mild traumatic brain injury (mTBI). The earlier concept of cerebral concussion as a functional and fully reversible clinical entity must be considered obsolete, as concussion can lead to demonstrable structural changes in the brain. It is therefore recommended that the use of the term 'concussion' be abandoned as soon as possible. However, as these terms are still used in clinical practice in many places and there is no clear criterion to distinguish concussion from mTBI, it is pragmatic to consider these terms as synony

Can a novel computer vision-based framework detect head-on-head impacts during a rugby league tackle?

BACKGROUND: Head-on-head impacts are a risk factor for concussion, which is a concern for sports. Computer vision frameworks may provide an automated process to identify head-on-head impacts, although this has not been applied or evaluated in rugby. METHODS: This study developed and evaluated a novel computer vision framework to automatically classify head-on-head and non-head-on-head impacts. Tackle events from professional rugby league matches were coded as either head-on-head or non-head-on-head impacts. These included non-televised standard-definition and televised high-definition video c

Biomarkers for acute mild brain injury (BAMBI): optimising GFAP/UCH-L1 thresholds to support CT stewardship in a NICE-guided mild-traumatic-brain-injury cohort.

BACKGROUND: NICE head injury guidance prioritises sensitivity but results in high CT utilisation and low diagnostic yield, contributing to Emergency Department (ED) crowding, particularly among older adults after falls. GFAP/UCH-L1 blood biomarkers may support safer CT stewardship, although limited specificity in older adults may reduce clinical impact. OBJECTIVE: To evaluate plasma GFAP/UCH-L1 against CT findings in a Major Trauma Centre ED cohort and assess whether age-stratified interpretation improves clinical utility while preserving rule-out performance. METHODS: Adults with suspected
