Three-Point Bedside Congestion Score (IJV Collapsibility + IVC Collapsibility + Central Venous Pressure) in Elective Mitral Valve Surgery: A Prospective Observational Study.

BACKGROUND: Systemic venous congestion and right-ventricular (RV) dysfunction are key drivers of postoperative hemodynamic instability, prolonged ventilation, and acute kidney injury (AKI) after mitral valve surgery. Comprehensive echocardiography is informative but may be impractical in high-throughput pre-anesthesia workflows. We evaluated a pragmatic three-point bedside congestion score combining internal jugular vein (IJV) collapsibility, inferior vena cava (IVC) collapsibility, and invasively measured central venous pressure (CVP) for preoperative risk stratification. METHODS: In this pr

Laparoscopic Right Hemicolectomy Using A Cranial-First Approach: Technical Description and Early Outcomes.

There are various surgical approaches for laparoscopic radical right hemicolectomy. However, the Henle's trunk and middle colic vessels often reveal anatomical variations, and improper handling may lead to vascular injury. This protocol presents a "cranial-first" approach combined with complete mesocolic excision (CME) to attempt to overcome the aforementioned technical difficulties. 86 cases were collected from September 2023 to April 2026. First, the gastrocolic ligament was opened to expose the right gastroepiploic vessels and the pancreatic head. The colic branches of Henle's trunk and the

Carotid endarterectomy for postradiation carotid stenosis in patient with hypopharyngeal cancer: a case report.

BACKGROUND: Radiation therapy for head and neck tumors is associated with radiation-induced carotid injury, which increases the risk of carotid artery stenosis and cerebrovascular events. Despite the well-established risks, routine screening for these complications has not been incorporated into clinical practice. This case emphasizes that, despite severe fibrosis and adhesion in the neck tissues resulting from radiation therapy, a well-structured surgical evaluation can still lead to favorable outcomes. It highlights the importance of timely surgical intervention in such cases. CASE PRESENTA

Alterations in CSF Amyloid-β and Tau Biomarkers in Former College and Professional American Football Players: Findings from the DIAGNOSE CTE Research Project.

Chronic traumatic encephalopathy (CTE) is a neurodegenerative disease associated with exposure to repetitive head impacts (RHIs), characterized by tau tangles around small blood vessels at the depths of the sulci. Currently, CTE can be diagnosed only postmortem, but can present with an array of cognitive, behavioral, mood, and motor symptoms. However, traumatic brain injury is also associated with increased risk of Alzheimer's disease (AD) and may lead to comorbid neuropathology. Characterization of the in vivo biomarkers of CTE is a necessary next step to facilitate accurate diagnoses. We exa

Impact of systemic anticoagulation on traumatic cervical arterial repairs: An analysis of the PROspective Observational Vascular Injury Trial (PROOVIT) registry.

PURPOSE: Systemic anticoagulation (SAC) is standard in elective arterial repairs but controversial in trauma due to bleeding risks, with unclear benefits in this population. This is the first study to investigate SAC's impact on cervical artery repairs. METHODS: Using the PROOVIT database (2012-2023), we included all cervical arterial injuries (common carotid, internal/external carotid, or vertebral) who underwent surgical or endovascular repair, categorized by intraoperative SAC use. Primary outcomes included in-hospital complications; secondary outcomes were re-intervention, stroke, 24-h pa

A Single Concussion in Juvenile Mice Leads to Sex Specific Acute Cerebral Vascular Dysfunction and Blood-brain Border Dysfunction.

BACKGROUND: Traumatic brain injury (TBI) can induce alterations to the blood-brain border (BBB) that contributes to long-term neurological and behavioral deficits. The temporal progression of post-concussion BBB dysfunction during developmentally sensitive periods remains poorly understood. Therefore, we sought to characterize the temporal evolution of BBB disruption and cerebrovascular alterations acutely after concussion in juvenile mice. METHODS: Postnatal day 17 (PND17) C57BL/6J male and female mice were subjected to sham or single closed head injury with long-term disorders (CHILD). At 1

Therapeutic management of intracranial internal carotid artery blowout syndrome: an institutional case series and individual participant data meta-analysis.

OBJECTIVE: Carotid blowout syndrome is a constellation of clinical presentations involving injury to the extracranial carotid system. It is frequently secondary to postradiation effects or direct invasion from head and neck cancers and is often managed via endovascular therapy (EVT). However, intracranial internal carotid artery blowout syndrome (ICABS) is a less described entity, has no predefined management algorithms, and can be difficult to manage due to the inherent anatomical limitations of the intracranial carotid. The aim of this study was to perform an individual participant data meta

Blood-brain barrier disruption, traumatic encephalopathy, and cognitive decline in retired athletes.

Cerebrovascular disruption has been implicated in the pathophysiology of head trauma and chronic traumatic encephalopathy (CTE). However, the long-term consequences of repetitive head trauma on blood-brain barrier (BBB) integrity and its link to cognitive function remain unknown. Here, using dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI), we show that BBB disruption can be detected years after the retirement of combat and collision sports athletes (n = 47) from their respective sports. A subgroup of individuals (n = 17) with extensive BBB disruption displayed worse cognitive de

Postoperative radiotherapy and external carotid artery diameter: is there a link to osseous free flap osteoradionecrosis risk?

OBJECTIVES: Postoperative radiotherapy (PORT) is a cornerstone in the multimodal treatment of head and neck malignancies but may lead to osteoradionecrosis (ORN) inside healthy osseous free flaps. This study analyzes the role of external carotid artery (ECA) narrowing as a potential contributor to flap ORN, as prior evidence in this regard remains unclear. MATERIALS AND METHODS: This retrospective study analyzed 89 patients who received osseous free flaps and PORT between April 2017 and December 2024. ECA diameters were measured on pre- and post-radiotherapy computed tomographies. Applied radi
