Intraoperative Monitoring of the Diaphragm Function During Cervical Dorsal Root Entry Zone-Lesioning Illustration With a Case Report.

BACKGROUND AND IMPORTANCE: Microsurgical DREZotomy (MDT) involves lesioning the dorsal root entry zone at the cervical spinal level, carrying a potential risk of iatrogenic spinal cord injury. Misidentifying the C4 root can lead to impaired diaphragmatic contractions and respiratory compromise. This report is the first MDT procedure in our Central Asian region and introduces the simultaneous analysis of 3 ventilator waveforms-capnography, pressure time, and flow time-as a novel, rapid-response intraoperative neurophysiological monitoring for definitive C4 root identification. CLINICAL PRESENT

C5 Palsy After Cervical Decompression: A 10-Year Single-Institution Study Highlighting Early Onset, High Bilaterality, and Poor Recovery.

INTRODUCTION: C5 palsy is a recognized complication following cervical decompression procedures, presenting as new-onset deltoid or biceps weakness. Despite extensive investigation, its mechanisms remain debated, involving root traction, foraminal stenosis, and possible cord-level ischemia-reperfusion injury. Reported incidences vary widely, and recovery outcomes differ across populations, making institutional data valuable. STUDY: This retrospective observational study evaluated postoperative C5 palsy over a 10-year period in patients undergoing cervical decompression. All cases were assesse

Accuracy and safety of navigated pedicle screw insertion in cervical spine fractures.

BACKGROUND: Pedicle screw (PS) fixation provides superior biomechanical stability compared with lateral mass screw (LMS) fixation for unstable cervical spine fractures (CS-Fx) but is associated with a risk of neurovascular injury. Navigation systems have improved PS placement accuracy, although most published studies remain small and underpowered to assess rare complications. OBJECTIVE: To evaluate the accuracy and safety of navigation-assisted PS fixation for unstable CS-Fx in a population-based cohort. METHODS: All consecutive patients with unstable CS-Fx who underwent navigated PS fixatio

Intraoperative Ultrasound for Post-lesion Confirmation in Cervical DREZ Surgery: Technical Feasibility and Clinical Outcomes.

BACKGROUND: Microsurgical dorsal root entry zone (DREZ) lesioning is an established treatment for refractory pain following brachial plexus avulsion. However, lesion depth and trajectory are critical technical variables that may influence safety and outcomes. We report our single-center experience using intraoperative ultrasound for systematic post-lesion confirmation during cervical DREZ lesioning. METHODS: Between January 2016 and March 2025, 23 consecutive patients with complete or partial brachial plexus avulsion and severe neuropathic pain (VAS ≥6) refractory to optimized pharmacological

Optimum Sonoanatomical Approach for Selective Cervical Nerve Root Block: Evaluation of Three Different Needle Pathways in Cadavers and Volunteers.

OBJECTIVE: To identify the safest approach for selective cervical spinal nerve block (SCSNB) using sonoanatomic evaluation of spinal nerves (SNs) and adjacent vascular structures. DESIGN: This study included 50 healthy volunteers and 7 cadavers. In volunteers, C5-C7 SNs were evaluated using ultrasound. Three virtual needle trajectories posterior-to-anterior (P-A), anterior-to-posterior (A-P), and lateral-to-medial (L-M) were assessed within a region of interest. Cadaveric dissections examined morphometric relationships between the vertebral artery (VA), ascending cervical artery (ACA), deep c

Predicting cervical muscle forces in fighter pilots: a comparison of shear wave elastography and PID controllers using finite element analysis.

PURPOSE: Helmet loads have been identified as a source of neck pain in fighter pilots. To minimize risk for neck pain, the effect of helmets on cervical spine biomechanics must be understood. In-vivo methods are difficult and invasive, so finite element modeling (FEM) techniques have been utilized recently to understand neck pathology development. Accurate muscle forces are crucial for physiologically relevant results. Therefore, this study evaluated the relationship between helmet loads and cervical muscle forces using two FEM muscle activation strategies. METHODS: A validated cervical spine

Cervical nerve root contribution to diaphragm motor innervation revisited via intraoperative clinical and neurophysiological observations.

OBJECTIVE: Classical descriptions state that diaphragmatic motor innervation arises predominantly from the fourth cervical nerve root (C4), with additional contributions from C3 and C5. However, clinical evidence supporting the functional involvement of C3 and C5 remains limited. Study aimed to reassess cervical root contribution to diaphragmatic innervation through intraoperative observations. METHODS: Eighteen consecutive patients undergoing dorsal root entry zone (DREZ) lesioning for refractory pain in upper-limb mostly due to brachial plexus injury were included. The procedure allowed int

Exoscope-assisted far-lateral approach for a retro-odontoid pseudotumor in the lateral position without fusion: a technical case report.

BACKGROUND: Retro-odontoid pseudotumor is a soft tissue mass that compresses cervicomedullary neural structures at the craniovertebral junction, causing severe neurological deficits. Optimal management remains controversial. We report exoscope-assisted intradural far-lateral resection of a retro-odontoid pseudotumor without fusion using the lateral position. CASE DESCRIPTION: A 69-year-old male presented with progressive motor weakness and gait disturbance. MRI showed a retro-odontoid pseudotumor with severe spinal cord compression and intramedullary high signal intensity at C1-C2. Radiograph

Surgical Nuances for Safeguarding Anomalous Vertebral Artery during Atlantoaxial/Occipital-C2 Fixation in Complex CV Junction Pathologies: Operative Video Article.

Nearly one-fifth of craniovertebral junction (CVJ) pathologies are associated with anomalous vertebral arteries (VAs), posing a significant surgical challenge. The VA can be damaged during dissection or screw placement. Eight patients with CVJ pathology and anomalous VA underwent instrumented fusion (C1-C2/O-C2). A comprehensive preoperative radiological evaluation with intraoperative biplanar fluoroscopy and Doppler was utilized for VA localization and screw placement. A postoperative computed tomography (CT) cervical spine with VA angiography was performed to assess the VA status and screw p

Biportal Endoscopic Posterior Cervical Foraminotomy.

BACKGROUND: Cervical spine pathologies causing nerve-root compression are treated with use of decompression through a variety of techniques. Although several operative management options exist, cervical foraminotomy may be indicated in patients with isolated, unilateral cervical radiculopathy. Biportal endoscopic cervical foraminotomy allows decompression with minimal soft-tissue disturbance and excellent visualization. DESCRIPTION: The procedure is performed with the patient in a prone position with routine neuromonitoring. Intraoperative fluoroscopy is utilized to identify the operative lev

The vertebral artery loops: a systematic review and meta-analysis.

PURPOSE: Vertebral artery loops (VALs) are focal deviations of the vertebral artery course that may produce pulsatile nerve-root compression and substantially increase the risk of iatrogenic vascular injury during cervical spine procedures. Although individually uncommon, they are a recognised "hidden danger" whose recognition changes preoperative planning. We aimed to estimate the pooled prevalence of VALs, to explain the wide variation in reported figures, and to translate these data into a practical framework for preoperative decision-making in cervical spine surgery. METHODS: A PRISMA-gui

Case report of an occipital neuralgia caused by a vertebral artery loop: surgical strategies from a rare neurovascular conflict.

BACKGROUND: Occipital neuralgia secondary to cervical nerve root compression is most commonly associated with degenerative or post-traumatic foraminal stenosis. In contrast, neurovascular conflict caused by an aberrant vertebral artery loop represents a rare and underrecognized etiology. Optimal surgical management remains variable, with multiple surgical techniques with risks and level of invasiveness. CASE DESCRIPTION: A 74-year-old woman presented with a five-year history of severe, progressively worsening left-sided occipital neuralgia refractory to conservative treatment. The pain was ch

Risk factors for C5 nerve root palsy following posterior cervical spinal canal expansion and reconstruction: a systematic review and meta-analysis.

OBJECTIVE: C5 nerve root palsy is the most common complication following posterior cervical spinal canal expansion and reconstruction, significantly impacting patients’ quality of life. Although several systematic reviews and meta-analyses have explored the risk factors of C5 nerve root palsy, inconsistencies remain in key conclusions such as the role of age and C4-5 intervertebral foramen stenosis, and new clinical evidence published in recent years has not been integrated. This meta-analysis aims to evaluate the incidence and risk factors of C5 nerve root palsy after posterior cervical spina

C2 nerve root sectioning in C1-C2 arthrodesis: a single-surgeon series of 31 patients.

PURPOSE: C1-C2 instability can result from trauma, degenerative changes, neoplasms, or congenital abnormalities. Atlantoaxial stabilization is a complex surgical procedure due to the unique anatomy of C1 and C2 and the wide range of motion of this segment. This study evaluates the safety and utility of C2 nerve root sectioning to facilitate visualization of the C1 screw entry point, reducing the risk of intraoperative neurological and vascular injury. METHODS: We retrospectively reviewed a single-surgeon (CM) series of 31 patients undergoing C1-C2 arthrodesis between 2009 and 2025, with intra

Role of T2 pedicle subtraction osteotomy in rigid cervicothoracic sagittal imbalance correction: A case series with technical details.

BACKGROUND: Correction of rigid cervicothoracic sagittal imbalance is technically demanding, especially in patients with prior cervical fusion or ankylosing spondylitis. Three-column osteotomies at C6-T1 can achieve deformity correction but carry risks of vertebral artery and cervical nerve root injury. T2 pedicle subtraction osteotomy (PSO) may provide comparable correction with a more favorable neurovascular risk profile. We evaluated radiographic outcomes, durability of correction, and safety of T2 PSO for rigid cervicothoracic deformity. MATERIALS AND METHODS: Nineteen consecutive patient

Spinal root magnetic stimulation: evaluating the limitations of a less invasive method to assess voluntary activation of the respiratory muscles.

Spinal root magnetic stimulation can be used to assess diaphragm voluntary activation (VA), but its validity during inspiratory efforts remains unclear. In addition, its use to evaluate abdominal muscle VA during expiratory efforts has not been explored. This study investigates spinal root magnetic stimulation as a method to assess VA of inspiratory and expiratory muscles. In 13 healthy participants (8 female, 18-40 yr), magnetic stimulation was delivered over the cervical spine (C6) and lower thoracic spine (T10) to activate the diaphragm (n = 11) during inspiratory efforts and the abdominal

Posterior Cervical Fixation: Accuracy and Safety of Free-Hand Inserted Screws.

BACKGROUND AND OBJECTIVES: Posterior fixation of the cervical spine (PFC) is commonly performed with rod-connected screws inserted in the vertebras. Because of the many sensitive structures located adjacent to the vertebras, computer-assisted navigation is often used with PFCs. We assessed safety and efficacy of free-hand placed PFCs. METHODS: We retrospectively identified all the patients in our institution between Januray 2006 and December 2016 who underwent a primary PFC for any indication. The postoperative images and medical records were evaluated for possible clinical failures, which wa
