Incidence and outcomes of stage 3 acute kidney injury in the setting of pediatric trauma: a trauma quality improvement program analysis.

BACKGROUND: Acute Kidney Injury (AKI) increases morbidity and mortality risk, yet data in pediatric trauma are limited. This study evaluated the incidence of Kidney Disease: Improving Global Outcomes (KDIGO) stage 3 AKI, kidney replacement therapy (KRT) use, and associated risk factors in pediatric trauma patients. METHODS: Pediatric patients (< 18 years) in the Trauma Quality Improvement Program (TQIP) database from 2017-2023 were analyzed for stage 3 AKI. Multivariable logistic regression (MVLR) identified associations between AKI and adverse outcomes, adjusting for confounders. Serious inj

Iatrogenic Subglottic Stenosis in Mast Cell Activation Syndrome: A Sentinel Case and Narrative Review.

Subglottic stenosis (SGS) may arise from both mechanical and inflammatory airway injury. Patients with mast cell activation syndrome (MCAS) are uniquely vulnerable, due to both direct effects of systemic inflammation and indirect effects of repeated intubation for anaphylaxis that can converge to cause life-threatening airway compromise. We describe a sentinel case of a 34-year-old female with MCAS and multiple prior intubations who presented with progressive dyspnea. Flexible laryngoscopy revealed grade III SGS. She underwent urgent endoscopic intervention, including balloon dilation, carbon

Post-traumatic benign paroxysmal positional vertigo: mechanisms, clinical phenotypes, and a structured clinical pathway for management.

Post-traumatic benign paroxysmal positional vertigo (BPPV) is a common but frequently underrecognized cause of dizziness following trauma. Unlike idiopathic BPPV, trauma-related BPPV arises from diverse injury mechanisms and is often characterized by heterogeneous canal involvement, greater need for repeated treatment, and frequent coexistence with broader vestibular dysfunction. These features contribute to diagnostic delays and variable clinical outcomes, particularly in the trauma and emergency care settings. We conducted a structured literature search and synthesized clinical, epidemiologi

Impalement Injuries in Children: Patterns, Management, and Outcomes From a 10-Year Single-Center Review.

BACKGROUND: Impalement injuries in children, though rare, pose unique diagnostic and surgical challenges due to unpredictable trajectories and frequent involvement of multiple organ systems. Literature is limited to isolated reports and small series, with few comprehensive analyses from pediatric populations. METHODS: We conducted a retrospective review of pediatric impalement injuries managed at a tertiary children's hospital over 10 years (July 2015-June 2025). These patients were first encountered by the general surgery and critical care team in the trauma center initially and were subsequ

Rapid transfer to specialist orthopaedic ward reduces mortality in hip fracture patients, but several factors reduce the ability to achieve the < 4-hour target.

PURPOSE: Hip fractures are one of the most common fracture types in adults. Rapid admission to orthopaedic wards from ED (< 4 h) and optimal timing for surgery within 36 h, is linked with good patient outcomes. This study aims to identify factors that influence achievement of timely admission of elderly hip fracture patients to specialist wards at a UK tertiary centre. Additionally, it seeks to examine the impact of timely admission on 30-day mortality. METHODS: This retrospective, single-centre study of a cohort of 6,170 patients aged over 60 who were admitted to our hospital’s ED with hip fr

Factors associated with subjective cognitive complaints in former American football players.

OBJECTIVE: Subjective cognitive complaints (SCC) can precede cognitive decline and are associated with demographic, exposure, lifestyle, and psychological factors. Prevalences of SCC and their correlates in individuals with repetitive head impacts (RHI) are poorly understood. This study characterized SCC in former elite American football players by frequency, mood and behavioral correlates, concordance with informant reports, and associations with neuropsychological test performance, cerebrospinal fluid (CSF), and magnetic resonance imaging (MRI) markers of neurodegeneration. METHOD: Former A

ACR Appropriateness Criteria® Major Blunt Trauma: Update 2025.

Trauma remains the leading cause of mortality in the United States for those <45 years of age, and it is the fourth leading cause of death overall. Polytrauma is defined as an injury to at least two body parts, including the head, neck, chest, abdomen, pelvis, or an extremity, with any one or a combination of these injuries being potentially fatal. This document covers imaging of major blunt trauma or polytrauma resulting in multiple organ injuries. Burn injuries, and injuries to pediatric patients are excluded. The American College of Radiology Appropriateness Criteria are evidence-based guid

Retention or Retrieval: A dilemma in a Case of Pediatric Cervical Spine Penetrating Injury from an Air Gun Pellet.

INTRODUCTION: Airgun or air-rifle injuries are often perceived as minor injuries; however, they can cause significant morbidity, particularly in children. The cervical spine, with its vital neurovascular and airway structures around, is vulnerable to any penetrating injury, even to low-velocity projectiles. CASE REPORT: We report the case of a 9-year-old boy who sustained an accidental posterior cervical airgun injury at home. The child was neurologically intact, and imaging revealed a retained metallic pellet deeply embedded in the posterior aspect of the neck impacted within the edge of the

A rare case of self-inflicted throat incision in a patient with psychotic depressive disorder: A case report and literature review.

Suicide is a complex phenomenon triggered by multiple factors. Cut throat injuries as a method of suicide are rare compared with other methods. This method is most commonly observed in young adult males in developing countries and is often associated with economic hardship. In injuries resulting from suicide attempts, hesitation marks may be present, and these marks assist forensic medicine in determining whether an injury is suicidal. We deemed it appropriate to present the general characteristics of suicidal cut throat injuries and the unique nature of our case in this article. A 71-year-old

Vascular Malformation of the Temporomandibular Joint Area Causing Osteoarthropathy.

This recent report described a rare case of osteoarthropathy of the temporomandibular joint area caused by vascular malformation in the right TMJ region. The patient was a 62-year-old male with a 1-month history of sudden pain and clicking in the right TMJ during mouth opening and closing. Multiple imaging examinations revealed a benign soft tissue tumor with a cumulative condylar bone at the posterior edge of the right condyle neck. Due to the concealed location of the tumor, which is located between the condyle and the external auditory canal, and the aim to minimize damage to the facial ner

Epidural hematoma in a pediatric patient with Hutchinson-Gilford progeria syndrome: management considerations: a case report.

BACKGROUND: Hutchinson-Gilford progeria syndrome (HGPS) is an exceedingly rare genetic disorder characterized by accelerated aging and profound vascular fragility. While cerebrovascular accidents are common in this population, traumatic epidural hematomas (EDH) are clinical rarities, with only two pediatric cases previously documented. Currently, it is estimated that fewer than 200 children with HGPS are living worldwide. The primary objective of this report is to present a comprehensive multidisciplinary management strategy for traumatic EDH in a 15-year-old HGPS patient, highlighting the cri

Rupture of a Superior Thyroid Artery Pseudoaneurysm After Tracheostomy: A Case Report.

Minor bleeding after tracheostomy is common, whereas massive hemorrhage is rare and can be catastrophic. We report an 80-year-old man who underwent surgical tracheostomy for prolonged ventilatory support after multiple trauma. The thyroid isthmus was partially divided and reflected caudally to create a tracheal window at the second to third tracheal rings. Hemostasis of the divided edge was achieved with electrocautery (without suturing or vessel ligation), and no pulsatile bleeding or significant vessel was encountered intraoperatively. During recovery, delirium and agitation led to frequent

Development and evaluation of a multimodal feature-based predictive model for radiotherapy-induced oral mucositis in nasopharyngeal carcinoma.

BACKGROUND: Accurate prediction of radiation-induced oral mucositis is crucial for personalized treatment in head and neck cancer. However, developing robust predictive models utilizing high-dimensional multimodal data (CT imaging, dose distribution, and clinical features) remains challenging, particularly in cohorts with limited sample sizes. OBJECTIVE: This study aimed to rigorously evaluate and compare the multi-class predictive performance of traditional machine learning algorithms and deep learning architectures under a small-cohort setting. METHODS: Multimodal data from 108 patients we

Application of surgical gloves in cervical and scapular skin grafting surgery: A case report and literature review.

RATIONALE: Securing and protecting the transplanted skin grafts in the cervical and scapular regions posttransplantation is challenging. PATIENT CONCERNS: A 42-year-old male was admitted for the treatment of burns in the neck and scapular regions. Due to the large size of the burn wounds in these areas, combined with the fact that a tracheostomy and intubation had already been performed, securing the skin grafts was challenging. During the first surgery, a tie-over dressing fixation method was used, but the survival rate of the transplanted skin was suboptimal. DIAGNOSES: The patient was dia

Assessing the quality of AI-generated and physician-written discharge summaries: evaluation of an EHR-integrated tool in a Dutch academic hospital.

BACKGROUND: Large language models (LLMs) offer potential to reduce administrative burden in clinical care by generating discharge summaries. Most prior evaluations have been limited to drafts, small cohorts, or non-integrated settings. Robust validation of fully automated, EHR-integrated systems in real-world practice is lacking. METHODS: This study was conducted in April 2025 at a Dutch academic hospital. A total of 292 paired discharge summaries from multiple departments were evaluated, each consisting of a physician-written and an LLM-generated version. Summaries were independently assesse
