An unusual case of radiocarpal dislocation in a 33-year-old man is reported. He was treated with closed reduction and plaster immobilization with a good functional result.

The association of congenital camptodactyly, familial arthropathy and coxa vara is a rare but recognized clinical entity. The radiological manifestations were reviewed in five patients. In the hips, coxa vara, short broad femoral necks and intraosseous cysts were demonstrated in all patients. Abnormal modeling of the acetabulum, increased joint space, effusion, small iliac wings and intraosseous herniation of fluid were found in four out of five patients. A family showing dominant inheritance of microcephaly, short stature, congenital dislocation of the hips and dysmorphic features is described. Affected individuals have malar hypoplasia, prominent nasal root, beaked nose, short philtrum and simple ears. This facial appearance is very similar to that in the family reported by Bawle and Horton in 1989.

Ligamentous injuries to multiple interphalangeal joints of an extremity have seldom been reported. We treated a patient with multiple ligamentous injuries in the little finger. Manual reduction of the distal interphalangeal (DIP) joint and surgical repair of the radial collateral ligament were performed to correct multidimensional joint instability and to allow for rehabilitation.

A child with multiple congenital abnormalities and a de novo interstitial deletion of the long arm of chromosome 17 is reported. This is the third case reported with this chromosome abnormality. The three cases present a peculiar phenotype, which is probably specific to the deletion.

Multiple injuries to the hand are relatively common in basketball. This article explains the importance of an early and thorough examination, preferably by a hand specialist. It is important to obtain a history of prior injury during the assessment because treatment and prognosis may be very different. Delayed treatment often yields an inferior result and lost time. If an injury is not treated initially, it may require operative treatment or even be untreatable.

Case notes of 20 patients who presented with grinding machine injury to the hand from April 1984 to March 1990 were analysed for pattern of injury, method of management and outcome of treatment. Thirteen cases were seen in the last three years of this period (the SAP era) compared to seven cases in first three years of the period (the pre-SAP era). Historical, physical, cognitive, and cytogenetic data were documented in 105 heterozygous fragile X [fra(X)] females and 90 controls in a prospective fashion. For comparisons, we divided heterozygotes and controls into those with cognitive impairment (IQ less than 85) and normal IQ (IQ greater than or equal to 85). An old dorsal lunate dislocation with associated multiple extensor tendon ruptures is described. Treatment consisted of proximal row carpectomy and transfer of the extensor indicis proprius to the distal stumps of the ruptured extensor tendons to the long, ring, and small fingers.

During a 9-year period, we treated 13 patients with major fractures of bones of the distal carpal row creating instability of the corresponding carpometacarpal joints. Diagnosis of the injury was made early in only eight patients whose treatment consisted of open reduction, pin or screw fixation, and immobilization from 6 to 8 weeks. They showed no complications and had an excellent functional recovery. Diagnosis in five (38%) patients was delayed 1 week to 8 months after the accident. At initial presentation, symptoms included pain, loss of grip strength, and mild limitation of finger motion.

We report the case of a 23-year-old man who fell from a third-floor window and injured his left hand. Physical examination and radiographic evaluation revealed open dorsal metacarpophalangeal dislocation of four fingers. The index finger dislocation was reduced in the emergency department, and the other three fingers responded to closed reduction during surgery. The patient made an uneventful recovery without complications. Metacarpophalangeal dislocations are unusual, with multiple dislocations rarely described.
