A multimodal approach to Canine Osteoarthritis management: A state-of-the-art.

Canine osteoarthritis (OA) is a prevalent, progressive, and debilitating joint disorder characterized by cartilage degradation, synovial inflammation, and subchondral bone remodeling. This multifactorial disease leads to chronic pain and significant impairment of quality of life. As OA remains incurable, therapeutic strategies have shifted toward comprehensive multimodal management aimed at addressing the disease's complex pathophysiology. This review presents an evidence-based overview of current approaches, including weight management, rehabilitation, pharmacologic therapies (Non-Steroidal A

A composite endpoint for systemic sclerosis-associated interstitial lung disease: association with mortality in two clinical trial cohorts.

BACKGROUND: The forced vital capacity (FVC) is the most commonly used endpoint in registrational trials for systemic sclerosis-associated interstitial lung disease (SSc-ILD). However, the FVC has known methodological pitfalls, is affected by extra-pulmonary SSc manifestations and may not be clinically meaningful to patients. Combining individual outcomes into a composite endpoint is an attractive alternative for measuring treatment response and augmenting statistical efficiency in SSc-ILD trials. METHODS: We previously developed a composite endpoint for SSc-ILD using data from the Scleroderma

Mechanism of auranofin toxicity: Inhibition of tetrahydrobiopterin metabolism.

Auranofin is a gold-containing anti-inflammatory drug used for the treatment of rheumatoid arthritis. Recent studies indicate that auranofin targets cellular antioxidants including the thioredoxin system and the ubiquitin-proteasome system, which can result in cellular oxidative stress. In the present studies, we identified a novel mechanistic site of action of auranofin, namely tetrahydrobiopterin (BH4) biosynthesis. BH4 is an essential cofactor required for aromatic amino acid hydroxylases, nitric oxide synthase and alkylglycerol monooxygenase, enzymes that produce mediators important in reg

Thiazolidine derivative (LPSF/CR-35) modulates the production of IL-17A, IL-10, IL-4, TFN, CCL2 and CXCL8 in PBMC from systemic sclerosis patients.

BACKGROUND: Systemic sclerosis (SSc) is an autoimmune disease characterized by vasculopathy, fibrosis, and dysregulation of the immune system. The therapeutic options available for SSc have limited efficacy, and there is a clear need for new pharmacological alternatives. Thiazolidinediones (TZD) are molecules that have therapeutic potential for SSc due to their pharmacological properties. In this study, we aimed to evaluate the immunomodulatory activity of a new TZD analogue (LPSF/CR-35) in peripheral blood mononuclear cells (PBMC) from SSc patients. MATERIALS AND METHODS: PBMC response to LP

Therapeutical efficacy of immunobiotics in patients with newly diagnosed rheumatoid arthritis.

INTRODUCTION: Rheumatoid arthritis (RA) remission remains a key treatment goal, but remission rates vary. Emerging evidence suggests that gut microbiota modulation via probiotics may influence systemic inflammation and improve outcomes in RA. The objective was to evaluate the effects of adjunctive probiotic supplementation on clinical outcomes in patients with newly diagnosed RA receiving conventional disease-modifying antirheumatic drugs (cDMARDs) over 12 months. METHODOLOGY: In this randomized, placebo-controlled trial, 100 patients with newly diagnosed RA were assigned to receive either pr

Autoantibody landscape and functional role of anti-C-C motif chemokine receptor 8 autoantibodies in systemic sclerosis: post-hoc analysis of a B-cell depletion trial.

Systemic sclerosis (SSc) is an autoimmune disease marked by fibrosis and extensive autoantibody production. Although B-cell depletion with rituximab (RTX) has shown clinical benefit, predictive biomarkers of response remain elusive. Here, we apply proteome-wide autoantibody screening using wet protein arrays covering 13,455 human antigens in serum samples from participants of the randomized trial of RTX. We identify a significant elevation in the total autoantibody levels in SSc compared to healthy controls, with greater reductions post-treatment observed in RTX high responders than in low res

Novel therapies for axial spondyloarthritis and future directions.

This update explores emerging therapeutic strategies aimed at novel targets implicated in the pathogenesis of axSpA. Recent clinical trials of bimekizumab, a monoclonal antibody targeting both IL-17A and IL-17F, and janus-kinase inhibitors have demonstrated significant and sustained improvements in clinical and imaging outcomes, with a favorable safety profile and reduced rates of uveitis. Investigational agents targeting GM-CSF and MK2 have not demonstrated efficacy, but the targeting of autoreactive T cell clonotypes shared among individuals with axSpA using depleting antibodies to the varia

From ulcerative colitis to amyloid storm: a case-based review.

The most important complication of familial Mediterranean fever (FMF) are AA amyloidosis and amyloid nephropathy. Amyloid nephropathy typically follows a slow, indolent course. However, "amyloid storm" is a recently described acute condition, characterized by sudden onset of severe proteinuria and rapidly progressive renal failure with markedly acute-phase reactants, progressing over days to weeks into end-stage renal disease (ESRD). Amyloid storm has been rarely reported in the literature. Most cases have occurred in patients with known FMF-related AA amyloid nephropathy. However, it has also

Targeting mitochondrial oxidative stress: A novel therapeutic strategy for degenerative joint diseases (Review).

Degenerative joint diseases, such as osteoarthritis (OA), intervertebral disc degeneration (IVDD) and rheumatoid arthritis (RA), cause pain and disability worldwide. Globally, OA affects >500 million individuals, IVDD affects 40-60% of adults and RA affects 0.5-1% of the global population. Current treatments (such as non-steroidal anti-inflammatory drugs and corticosteroids for OA, conservative management and spinal surgery for IVDD, and disease-modifying anti-rheumatic drugs/biologics for RA) focus on symptom relief and inflammation control, but they do not prevent disease progression nor res

Disseminated Talaromyces marneffei infection in a patient with Ankylosing Spondylitis: a case report and review of the literature.

BACKGROUND: Ankylosing Spondylitis (AS) is a prevalent autoimmune disorder that necessitates the prolonged use of corticosteroids, non-steroidal anti-inflammatory drugs (NSAIDs), and disease-modifying antirheumatic drugs (DMARDs). Immunosuppressive medications are now commonly incorporated into treatment plans; however, this may increase patients' risk of severe infections. Infections caused by Talaromyces marneffei (TM) are rarely seen in patients with AS, yet if left untreated, they could result in severe morbidity and even death. CASE REPORT: This report presents a comprehensive case study

Utility of repeat MRI in assessing treatment non-response in axial spondyloarthritis: data from two large tertiary rheumatology centres.

INTRODUCTION: The role of MRI of the spine/sacroiliac joints to aid the diagnosis of axial spondyloarthritis (axSpA) is well established. Limited data, however, exist on the use of MRI to assess disease activity, resulting in current Assessment of SpondyloArthritis International Society (ASAS)/European Alliance of Associations for Rheumatology (EULAR) guidelines not recommending the use of MRI for this purpose. We aimed to assess the current use of MRI to assess disease activity and its impact on clinical decision making. METHODS: As part of a service evaluation, we identified patients with a

The treatment landscape in axial spondyloarthritis-do we need new therapies?

Despite major therapeutic advances, many patients with axial spondyloarthritis (axSpA) fail to achieve remission or even low disease activity with currently available advanced therapies. This highlights an urgent need to understand why treatment responses remain suboptimal and how management can be improved. Several mechanisms contribute to persistent symptoms. Misdiagnosis and overdiagnosis remain frequent, often due to nonspecific imaging findings being misattributed to axSpA. Among correctly diagnosed patients, a substantial proportion experience pain not directly driven by inflammation. No

Individualised treatment effect prediction: the benefit of initial biological treatment in early rheumatoid arthritis.

OBJECTIVES: Initiating biologics upfront in early disease-modifying antirheumatic drugs (DMARDs)-naïve rheumatoid arthritis (RA) may benefit selected patients more than starting with methotrexate (MTX) monotherapy. Current guidelines recommend using prognostic markers to define high-risk groups for early biological treatment. Predictive models combining these markers could better identify individuals likely to benefit. This study aimed to develop and validate models predicting the benefit of starting tocilizumab (TCZ) plus MTX over MTX alone in early DMARD-naïve RA. METHODS: We used data from

Pregnancy outcomes of non-tumour necrosis factor inhibitor biologic disease-modifying antirheumatic drugs among individuals with autoimmune diseases: a scoping review and proposed framework for reporting outcomes.

OBJECTIVES: Biologic disease-modifying antirheumatic drugs (bDMARDs) have transformed autoimmune disease treatment. Although the perinatal impacts of tumour necrosis factor inhibitors (TNFis) have been systematically synthesised, comparable evidence syntheses for non-TNFi bDMARDs are lacking. We conducted a scoping review to synthesise evidence on the impact of non-TNFi bDMARDs on pregnancy outcomes. METHODS: We searched Embase, MEDLINE, and CENTRAL databases in November 2023. We included studies among individuals with chronic autoimmune disease that examined non-TNFi bDMARD exposure in mothe

Which ASDAS cut-off corresponds best to treatment intensification in patients with axial spondyloarthritis in daily practice? A prospective study from a clinical registry.

To investigate which Axial Spondyloarthritis Disease Activity Score (ASDAS) cut-off corresponds best with treatment intensification (TI) in daily practice in patients with axial spondyloarthritis (axSpA). Patients from the prospective SpA-Net registry with axSpA and ≥ 1 ASDAS measurement in 2016–2022 were included. TI was defined as (1) increasing dose/frequency of current drug, (2) switching drug(s) or (3) adding drug(s); all due to inefficacy of current treatment and only considering anti-inflammatory drugs. Patients could contribute multiple observations. Receiver operating characteristic a

Rituximab-induced Acute Thrombocytopenia in Systemic Sclerosis: A Case Report of Two Cases.

Rituximab is frequently used to treat hematological and autoimmune diseases. Rituximab-induced acute thrombocytopenia (RIAT) is a rare adverse event associated with the use of this drug. In this report, we describe two cases of RIAT in patients with systemic sclerosis. Severe thrombocytopenia was observed in both patients following the second rituximab infusion. In addition, mild leukopenia, hypocomplementemia, and systemic inflammation were observed. Most previous reports have described RIAT in patients with hematological malignancies, but patients with autoimmune diseases may also develop th

Assessment of drug levels as a biomarker in rheumatoid arthritis.

In this viewpoint, we focused on 2 examples of the assessments of drug levels as a biomarker in rheumatoid arthritis (RA), based on our experiences in longitudinal studies and clinical trials. Among a long list of potential biomarkers in rheumatology, the measurements of drug levels for the efficacy and toxicity of the drugs have been extensively investigated. One example is the levels of methotrexate (MTX)-polyglutamates (PGs), and the second one is that of biological disease-modifying antirheumatic drugs (DMARDs) in patients with RA. Recently, meta-analyses and population pharmacokinetics-ph
