Sleep patterns among patients with chronic fatigue: A polysomnography-based study.

OBJECTIVES: The purpose of this study was to detect treatable sleep disorders among patients complaining of chronic fatigue by using sleep questionnaires and polysomnography. METHODS: Patients were referred to hospital for investigations and rehabilitation because of a suspected diagnosis of myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). The criteria for further referral to full-night polysomnography (PSG) were symptoms of excessive daytime sleepiness and/or tiredness in the questionnaires. RESULTS: Of a total of 381 patients, 78 (20.5%) patients underwent PSG: 66 women and 12 …

Use of Chest Wall Electromyography to Detect Respiratory Effort during Polysomnography.

STUDY OBJECTIVES: To evaluate the ability of chest wall EMG (CW-EMG) using surface electrodes to classify apneas as obstructive, mixed, or central compared to classification using dual channel uncalibrated respiratory inductance plethysmography (RIP). METHODS: CW-EMG was recorded from electrodes in the eighth intercostal space at the right mid-axillary line. Consecutive adult clinical sleep studies were retrospectively reviewed, and the first 60 studies with at least 10 obstructive and 10 mixed or central apneas and technically adequate tracings were selected. Four obstructive and six central

Event scoring in polysomnography: scoring arousals, respiratory events, and leg movements.

Polysomnographic events comprise a wide variety of phenomena, including episodes of apnea, episodes of hypopnea, leg movements, transient central nervous system arousals, and eye movements. The process of event scoring involves pattern recognition and provides a description of potentially pathophysiologic activity occurring during sleep. The rules for scoring sleep-related events continue to be developed. The rules are precise and must be followed with extreme care. When the polysomnogram is interpreted, the report must include a summary of event scoring and clinical correlation to the sleep com

Nonattended home automated continuous positive airway pressure titration: comparison with polysomnography.

OBJECTIVE: Automatic adjusting nasal continuous positive airway pressure titration (APAP) has been introduced as an alternative method of establishing pressures for patients with sleep apnea. The performance and accuracy of APAP in nonattended home environment are controversial. This study assessed APAP polysomnographic outcomes and accuracy in a nonattended home environment. STUDY DESIGN AND SETTING: We conducted a retrospective consecutive case series of 24 consecutive patients who had nonattended APAP and simultaneous full polysomnography (PSG). RESULTS: APAP was tolerated and reduced obs…

Using self-reported questionnaire data to prioritize OSA patients for polysomnography.

Many laboratories have large numbers of patients with suspected obstructive sleep apnea (OSA) waiting to be tested. We assessed the use of simple clinical data to detect those patients with an apnea index <20 (low AI) who could be studied less emergently. Using questionnaires completed by patients prior to evaluation, we collected data on 354 consecutive patients (281 males, 73 females; mean age 48.6 years) referred for OSA and assessed with polysomnography (PSG). The questionnaires included the Epworth sleepiness scale (ESS), height, weight, age, and a history of observed apnea. Analysis of r…

The role of polysomnography in Parkinson's disease: objective sleep assessment and clinical indications.

Sleep disorders (SDs) are highly prevalent in Parkinson's disease (PD), affecting up to 80% of patients during the disease and significantly contributing to impaired quality of life, cognitive decline, motor worsening, fall risk, and increased caregiver burden. Polysomnography (PSG) remains the gold standard for objective sleep assessment in PD and plays a central role in differential diagnosis and therapeutic planning. This review examines the spectrum of SDs identifiable by PSG, including rapid eye movement (REM) sleep behavior disorder (RBD), obstructive sleep apnea (OSA), periodic limb mov…

Normal polysomnography parameters in healthy children: a systematic review and meta-analysis.

INTRODUCTION: There is a lack of normative values for polysomnographic parameters in childhood. This study presents a comprehensive meta-analysis of paediatric polysomnography parameters scored using recent criteria to establish normative values adjusted for age and sex. METHODOLOGY: A systematic search was conducted in Web of Science and Scopus for studies that performed overnight polysomnography in healthy children and reported polysomnographic parameters scored using the American Academy of Sleep Medicine criteria (2007 or 2012). Children with previously diagnosed health conditions were ex…

Risk of Developing Seizures in Children With Abnormal EEG Findings During Polysomnography.

BACKGROUND: Polysomnography (PSG) utilizes abbreviated electroencephalogram (EEG) to stage sleep. The aim of this study was to determine whether epileptiform abnormalities on this limited EEG coverage correlated with abnormalities on routine EEG (rEEG) and an increased risk for seizures in children without a prior diagnosis of epilepsy. METHODS: A six-year retrospective chart review was performed assessing children with abnormalities on EEG during PSG. Children who underwent subsequent rEEG were included; children with a prior diagnosis of seizures were excluded. The main outcome measures wer…

Utility of split-night polysomnography in the diagnosis of upper airway resistance syndrome.

PURPOSE: Split-night polysomnography can both establish the diagnosis and titrate continuous positive airway pressure (CPAP) during a single study in patients with sleep-disordered breathing. We sought to determine if split-night polysomnography could be effectively used in upper airway resistance syndrome (UARS) without diminishing diagnostic accuracy or success of CPAP titration. METHODS: Consecutive patients diagnosed with UARS were included. Split-night studies were performed in patients meeting predefined criteria. We compared data between those undergoing traditional and split-night pol…

[Comparative use of MESAM IV and polysomnography in sleep-related respiratory disorders].

The high prevalence of sleep-related respiratory disturbances among the male population requires development of reliable outpatient measurement systems enabling the selection of patients to be admitted to the sleep laboratory. MESAM IV is such a system. It can record for up to 18 hours via four sensors a patient's snoring sounds, heart rate, oxygen saturation and posture. To test the reliability of the instrument we compared parallel night measurements by MESAM IV and polysomnography (PSG) via the respective Respiratory Disturbance Indices (RDI = number of apnoeas plus hypopnoeas per hour). Th…
