ST elevation myocardial infarction, or not - This is the question.

We present an artefactual ECG created by a digital ECG-recording machine, caused by deletion of the first 80 ms of the QRS complex immediately following the pacing spike in a patient with complete atrio-ventricular block, biventricular pacing and chronic atrial fibrillation. The artefact was mistaken for inferior ST elevation myocardial infarction and the patient underwent unnecessary urgent coronary angiogram. We are not aware of this particular artefact pattern being previously reported in the literature.

The influence of scar on the spatio-temporal relationship between electrical and mechanical activation in heart failure patients.

AIMS: The aim of this study was to determine the relationship between electrical and mechanical activation in heart failure (HF) patients and whether electromechanical coupling is affected by scar. METHODS AND RESULTS: Seventy HF patients referred for cardiac resynchronization therapy or biological therapy underwent endocardial anatomo-electromechanical mapping (AEMM) and delayed-enhancement magnetic resonance (CMR) scans. Area strain and activation times were derived from AEMM data, allowing to correlate mechanical and electrical activation in time and space with unprecedented accuracy. Spec…

Left bundle branch block-induced cardiomyopathy: a diagnostic proposal for a poorly explored pathological entity.

Despite being increasingly recognized as a specific disease, at the present time left bundle branch block (LBBB)-induced cardiomyopathy is neither formally included among unclassified cardiomyopathies nor among the acquired/non-genetic forms of dilated cardiomyopathy (DCM). Currently, a post-hoc diagnosis of LBBB-induced cardiomyopathy is possible when evaluating patients' response to cardiac resynchronization therapy (CRT). However, an early detection of a LBBB-induced cardiomyopathy could have significant clinical and therapeutic implications. Patients with the aforementioned form of dyssync…

The 43rd International Society for Computerized Electrocardiology ECG initiative for the automated detection of strict left bundle branch block.

The presence of left bundle branch block (LBBB) is an important predictor of benefit from cardiac resynchronization therapy (CRT). New "strict" electrocardiographic (ECG) criteria for LBBB have been shown to better predict benefit from CRT. The "strict" LBBB criteria include: QRS duration ≥140 ms (men) or ≥130 ms (women), QS- or rS-configurations of the QRS complex in leads V1 and V2, and mid-QRS notching or slurring in ≥2 of leads V1, V2, V5, V6, I and aVL. The "strict" LBBB criteria are not regularly used and most hospital automated ECG systems and physicians still use more conventional LBBB…

[Right ventricular septal stimulation would produce similar bi-ventricular dyssynchrony as does apical stimulation in patients with normal ejection fraction].

OBJECTIVE: To determine in patients with normal ejection fraction, undergoing permanent VVI pacing, if medial septal stimulation has lower dyssynchrony than apical stimulation assessed by echocardiography. METHOD: A prospective trial, 19 patients>70 years old, scheduled for VVI pacemaker implantation for complete degenerative atrioventricular block, ventricular frequency<50beat per minute and ejection fraction≥45%. Patients with atrial fibrillation, heart failure, left bundle branch block and QRS durations longer than 120milliseconds in surface electrocardiogram with sinus rhythm were exclude

Implantation of left ventricular epicardial leads in cardiosurgical patients with impaired cardiac function--a worthwhile procedure in concomitant surgical interventions?

BACKGROUND: Cardiac resynchronization therapy (CRT) by means of multisite biventricular pacing is an effective therapeutic option for the treatment of severe heart failure. The present study estimates how many open heart-surgery patients could benefit from the implantation of permanent left ventricular (LV) pacing leads. After routine preoperative screening, epicardial electrodes were implanted in selected patients. Lead performance and outcomes were investigated. METHODS: Primarily, 1059 patients were retrospectively investigated with regard to LV function, left bundle branch block and QRS d…

[Early warning system for pulmonary fluid status monitoring in terminal heart failure].

HISTORY AND ADMISSION FINDINGS: A 38-year-old man was admitted for increasing dyspnea, nausea and emesis during the preceding year. Clinical … condary to dilated cardiomyopathy was initiated without significant improvement in clinical status. Cardiac resynchronization therapy (CRT) was therefore performed with implantation of a biventricular pacer with defibrillator (CRT-D) and a fluid status monitoring system (OptiVol). Subsequently the patient was able to increase his physical activity to NYHA class II and was followed …

Lower rate limit for pacing by cardiac resynchronization defibrillators: Should lower rate programming be reconsidered?

BACKGROUND: No real-world large database associates lower rate limit (LRL) programming and survival of subjects with cardiac resynchronization therapy-defibrillators (CRT-Ds). OBJECTIVE: The purpose of this study was to test the hypothesis that lower LRL programming is independently associated with survival, and that LRL and heart rate score (HrSc) are associated. METHODS: All dual-chamber CRT-D devices in the Remote Patient Monitoring (RPM) ALTITUDE database (2006-2011) were queried. Baseline HrSc was defined as the percentage of all atrial sensed and paced beats in the tallest 10-beat hist…

Bipolar left ventricular pacing is associated with significant reduction in heart failure or death in CRT-D patients with LBBB.

BACKGROUND: There are limited data on the significance of left ventricular (LV) lead pacing polarity to predict clinical outcomes. OBJECTIVES: We aimed to determine the association between the LV lead pacing polarity for heart failure (HF) or death and ventricular tachyarrhythmias (VTA) in patients enrolled in MADIT-CRT (Multicenter Automatic Defibrillator Implantation Trial with Cardiac Resynchronization Therapy), receiving a cardiac resynchronization therapy device with implanted cardioverter-defibrillator (CRT-D). METHODS: We retrospectively analyzed LV pacing polarity. Patients with LV b…

The left ventricular lead electrical delay predicts response to cardiac resynchronisation therapy.

Up to one-third of patients who undergo cardiac resynchronisation therapy (CRT) are not responders. To identify potential responders to CRT may be sometimes difficult and time-consuming. Forty-five patients who had undergone CRT implantation for standard indications were evaluated. Electrical left ventricular (LV) lead location was assessed by left ventricular activation time (LVAT), LV lead electrical delay (LVLED), and RV-LV interlead electrical delay (RVsense-LVsense). Anatomic LV pacing location was assessed as basal or mid-ventricular between 3:00 to 5:00 (traditionally optimal site), and

Complications arising from cardiac implantable electrophysiological devices: review of epidemiology, pathogenesis and prevention for the clinician.

Cardiac implantable electronic devices (CIEDs) which include devices such as permanent pacemakers, implantable cardioverter-defibrillators and cardiac resynchronization therapy devices are well established therapies that have been shown to improve quality of life and prolong survival in appropriately selected patients, and are being increasing utilized across the world. However, CIED implantations are invasive procedures and a thorough understanding of the potential complications is required to prevent deleterious patient outcomes. In this review, we will discuss frequent complications (infect…

Incidence, predictors, and procedural results of upgrade to resynchronization therapy: the RAFT upgrade substudy.

BACKGROUND: The resynchronization-defibrillation for ambulatory heart failure trial (RAFT) study demonstrated that adding cardiac resynchronization therapy (CRT) in selected patients requiring de novo implantable cardiac defibrillators (ICD) reduced mortality as compared with ICD therapy alone, despite an increase in procedure-related adverse events. Data are lacking regarding the management of patients with ICD therapy who develop an indication for CRT upgrade. METHODS AND RESULTS: Participating RAFT centers provided data regarding de novo CRT-D (CRT with ICD) implant, upgrade to CRT-D durin…

Re-evaluating the electro-vectorcardiographic criteria for left bundle branch block.

The criteria for left bundle branch block have gained growing interest in the last few years. In this overview, we discuss diagnostic and prognostic aspects of different criteria. It was already shown that stricter criteria, including longer QRS duration and slurring/notching of the QRS, better identify responders to cardiac resynchronization therapy. We also include aspects of ST/T concordance and discordance and vectorcardiography, which could further improve in the fine-tuning of the left bundle branch criteria.
