Cardiac resynchronization therapy for the treatment of sleep apnoea: a meta-analysis.

AIMS: Sleep apnoea (SA) is a common problem among congestive heart failure (CHF) patients. Evidence has shown that cardiac resynchronization therapy (CRT) reduces morbidity and mortality associated with CHF. The aim of this paper was to review studies evaluating the reduction of the Apnoea-Hypopnoea Index (AHI) in patients with SA after treatment with CRT and to perform a meta-analysis to estimate the true effect of CRT on SA. METHODS AND RESULTS: A systematic electronic literature search was conducted in Medline and Embase to identify studies reporting on the effects of CRT on SA. A hand sea…

Cardiac resynchronization therapy upgrade in a patient with dextrocardia and situs inversus.

Cardiac resynchronization therapy in patients with congenital heart disease can be technically challenging. We report a case of upgrade of an implantable cardioverter defibrillator to a resynchronization device, in a patient with dextrocardia and situs inversus. The procedure was successfully performed without complication, using a conventional approach and standard equipment.

Cardiac resynchronization therapy for CHF. The rewards and risks of biventricular pacing.

Patients with advanced heart failure continue to experience high morbidity and mortality despite recent progress made with the use of such drugs as beta-blockers and angiotensin-aldosterone inhibitors. Cardiac transplantation has severe limitations due to the short supply of organs and the ineligibility of most CHF patients for this therapy. Approved heart-assist devices are cumbersome and costly. Therefore, these devices are currently used mainly in tertiary care centers in a limited number of patients. CRT has been rapidly evolving as a viable and beneficial therapy that is universally appli

Longer-term effects of cardiac resynchronization therapy on mortality in heart failure [the CArdiac REsynchronization-Heart Failure (CARE-HF) trial extension phase].

AIMS: The CArdiac REsynchronization-Heart Failure study randomized patients with left ventricular ejection fraction < or =35%, markers of cardiac dyssynchrony, and persistent moderate or severe symptoms of heart failure despite pharmacological therapy, to implantation of a cardiac resynchronization therapy (CRT) device or not. The main study observed substantial benefits on morbidity and mortality during a mean follow-up of 29.4 months [median 29.6, interquartile range (IQR) 23.6-34.6]. Prior to study closure, an extension phase lasting a further 8 months (allowing time for data analysis and p…

Cardiac Inflammation Impedes Response to Cardiac Resynchronization Therapy in Patients With Idiopathic Dilated Cardiomyopathy.

BACKGROUND: Cardiac resynchronization therapy (CRT) is an established therapy in patients with dilated cardiomyopathy (DCM) and conduction disorders. Still, one-third of the patients with DCM do not respond to CRT. This study aims to depict the underlying cardiac pathophysiological processes of nonresponse to CRT in patients with DCM using endomyocardial biopsies. METHODS: Within the Maastricht and Innsbruck registries of patients with DCM, 99 patients underwent endomyocardial biopsies before CRT implantation, with histological quantification of fibrosis and inflammation, where inflammation w…

Cardiac Implantable Electronic Device Therapy: Permanent Pacemakers, Implantable Cardioverter Defibrillators, and Cardiac Resynchronization Devices.

Cardiac implantable electronic devices (CIEDs) provide lifesaving therapy for the treatment of bradyarrhythmias, ventricular tachyarrhythmias, and advanced systolic heart failure. Advances in CIED therapy have expanded the number of patients receiving permanent pacemakers, implantable cardioverter defibrillators, and cardiac resynchronization therapy devices. These devices improve quality of life and, in many cases, reduce mortality. However, limitations remain in the management of patients who require CIED therapy. This article provides a broad overview of CIED therapy in the management of th…

Left ventricular systolic torsion correlates global cardiac performance during dyssynchrony and cardiac resynchronization therapy.

Left ventricular (LV) systolic torsion is a primary mechanism contributing to stroke volume (SV). We hypothesized that change in LV torsion parallels changes in global systolic performance during dyssynchrony and cardiac resynchronization therapy (CRT). Seven anesthetized open chest dogs had LV pressure-volume relationship. Apical, basal, and mid-LV cross-sectional echocardiographic images were studied by speckle tracking analysis. Right atrial (RA) pacing served as control. Right ventricular (RV) pacing simulated left bundle branch block. Simultaneous RV-LV free wall and RV-LV apex pacing (CR…

Influence of different atrioventricular and interventricular delays on cardiac output during cardiac resynchronization therapy.

Restoration of the atrioventricular (AVD) and interventricular (VVD) delays increases the hemodynamic benefit conferred by biventricular (BiV) stimulation. This study compared the effects of different AVD and VVD on cardiac output (CO) during three stimulation modes: BiV-LV = left ventricle (LV) preceding right ventricle (RV) by 4 ms; BiV-RV = RV preceding LV by 4 ms; LVP = single-site LV pacing. We studied 19 patients with chronic heart failure due to ischemic or idiopathic dilated cardiomyopathy, QRS >/= 150 ms, mean LV end-diastolic diameter = 78 +/- 7 mm, and mean LV ejection fraction = 21

The Association of Electrical Delay on Hemodynamic Response With Cardiac Resynchronization Therapy.

BACKGROUND: The effect of electrical delay at the left ventricular (LV) pacing site on acute hemodynamic response with cardiac resynchronization therapy (CRT) has been investigated only in small observational studies. OBJECTIVES: This study evaluates the impact of electrical delay, as assessed by interventricular (right ventricular-LV]) interval, on the acute hemodynamic response to CRT in a large, diverse multicenter cohort. METHODS: A total of 144 patients in 3 prospective studies, the PATH-CHF (Pacing Therapies in Congestive Heart Failure) I and II and the CRTAVO (CRT Optimization Algorit…

Strain-based staging as a unifying concept in cardiac resynchronization therapy.

AIMS: Left bundle branch block (LBBB) septal strain patterns are associated with left ventricular (LV) reverse remodelling after cardiac resynchronization therapy (CRT). However, their prognostic value and impact in patients undergoing CRT, as well as in CRT-eligible patients receiving conservative treatment, remains underexplored. This study aimed to validate the prognostic significance of LBBB strain patterns and elucidate the mechanisms underlying CRT response by evaluating their interaction with myocardial scar and clinical outcomes. METHODS AND RESULTS: In this multicentre study, 267 CRT…

Vectorcardiographic QRS area as a predictor of response to cardiac resynchronization therapy.

Cardiac resynchronization therapy (CRT) is a good treatment for heart failure accompanied by ventricular conduction abnormalities. Current ECG criteria in international guidelines seem to be suboptimal to select heart failure patients for CRT. The criteria QRS duration and left bundle branch block (LBBB) QRS morphology insufficiently detect left ventricular activation delay, which is required for benefit from CRT. Additionally, there are various definitions for LBBB, in which each one has a different association with CRT benefit and is prone to subjective interpretation. Recent studies have sh…

Clinical impact of MultiPoint pacing in responders to cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy demonstrated benefits in heart failure. However, only 60-70% are responders and only 22% are super-responders. MultiPoint pacing (MPP) improves structural remodeling, but data in responder patients is scarce. METHODS: A prospective, randomized study of the efficacy of MPP was conducted in patients who were CRT responders after 6 months of bi-ventricular (BiV) therapy. At 6 months, responder patients (LV end-systolic volume [LVESV] reduction ≥15%) were randomized to either continued BiV therapy or to MPP programmed with wide anatomical separation ≥…

Outcomes of Cardiac Resynchronization Therapy in Patients with Hypothyroidism and Heart Failure.

BACKGROUND: Hypothyroidism is known to be associated with adverse clinical outcomes in heart failure. The association between hypothyroidism and cardiac resynchronization therapy outcomes in patients with severe heart failure is not clear. METHODS: The study included 1316 patients who received cardiac resynchronization therapy between 2002 and 2015. Baseline demographics and cardiac resynchronization therapy outcomes, including left ventricular ejection fraction, New York Heart Association class, appropriate implantable cardioverter-defibrillator therapy, and all-cause mortality, were collect…

Mortality and cardiac resynchronization therapy with or without defibrillation in primary prevention.

AIMS: Cardiac resynchronization therapy with (CRTD) or without (CRTP) defibrillator is recommended in selected patient with systolic chronic heart failure and wide QRS. There is no guideline firmly indicating choice between CRTP and CRTD in primary prevention, particularly in older patients. METHODS AND RESULTS: Based on the French administrative hospital-discharge database, information was collected from 2010 to 2017 for all patients implanted with CRTP or CRTD in primary prevention. Outcome analyses were undertaken in the total study population and in propensity-matched samples. During foll…

Comparison of measures of ventricular delay on cardiac resynchronization therapy response.

BACKGROUND: Left ventricular (LV) pacing at sites of prolonged LV delay (QLV) or at long interventricular delay (right ventricle [RV]-LV) is strongly associated with cardiac resynchronization therapy (CRT) response. QLV and RV-LV have been independently evaluated, but little is known regarding the interrelationship between these measures or of delay to the RV. OBJECTIVE: The purpose of this study was to evaluate the relationship between measures of electrical delay on CRT response in the SMART-AV (SmartDelay Determined AV Optimization: A Comparison to Other AV Delay Methods Used in Cardiac Re…
