Peroneal nerve injury associated with sports-related knee injury.

OBJECT: This study analyzes 84 cases of peroneal nerve injuries associated with sports-related knee injuries and their surgical outcome and management. METHODS: The authors retrospectively reviewed the cases of peroneal nerve injury associated with sports between the years 1970 and 2010. Each patient was evaluated for injury mechanism, preoperative neurological status, electrophysiological studies, lesion type, and operative technique (neurolysis and graft repair). Preoperative status of injury was evaluated by using a grading system published by the senior authors. All lesions in continuity …

Multiligament Knee Injury: Injury Patterns, Outcomes, and Gait Analysis.

Multiligament knee injuries remain a challenge for the treating surgeon and little is known about the injury patterns and factors determining clinical and gait outcomes after multiligament knee reconstruction (MLKR). This article aims to identify specificities of this complex injury, in terms of demographics, mechanisms of injury, injury pattern, and associated lesions. The time frame between injury and surgery and distribution of repair versus reconstruction procedures have been analyzed. This study reports functional and clinical outcomes, providing an overview of expected results. Using mot…

Traumatic knee injury healthcare pathways and outcomes: the Australian Knee Injury Inception Cohort Study (KIICS) protocol.

Decision-making for the optimal management of traumatic knee injuries can be challenging. Clinical trials reveal only small differences between surgical and non-surgical approaches, while patient and clinician biases, as well as healthcare access issues, may also influence management. Little is known about the real-world healthcare pathways for patients with a knee injury, as well as the person- and/or healthcare-related factors that influence management strategies and outcomes. The Australian Knee Injury Inception Cohort Study (KIICS) aims to: (1) describe healthcare pathways following acute …

Investigating the Ligament Involvement in High-Energy and Polytraumatic Multiligament Knee Injuries Compared With Low-Energy or Isolated Injuries.

BACKGROUND: Multiligament knee injuries (MLKIs) due to high-energy (HE) or polytraumatic (PT) mechanisms result in worse patient outcomes after surgery versus low-energy (LE) or nonpolytraumatic (NPT) mechanisms. There remains a paucity of research in the literature investigating MLKIs resulting from these mechanisms of injury. PURPOSE: To characterize differences in ligamentous injury/surgery patterns between different mechanism groups: HE versus LE and PT versus NPT. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: This is a retrospective chart review of consecutive sur…

Kaplan fiber injuries in the anterior cruciate ligament-injured knee: current concepts.

Kaplan fibers (KFs) are the deep femoral attachments of the iliotibial band and are increasingly recognized as important secondary restraints to anterolateral rotatory instability in the anterior cruciate ligament (ACL)-injured knee. However, their clinical significance, magnetic resonance imaging (MRI) diagnosis, natural history after ACL reconstruction, and indications for surgical treatment remain incompletely defined. This current concepts review summarizes contemporary evidence on KF anatomy, biomechanics, imaging, healing potential, clinical outcomes, and surgical management. Current evi

Biochemical and cytokine environment in skeletally immature patients after traumatic knee injuries.

Traumatic knee injuries in skeletally immature patients occur in a unique biological context shaped by growth, high tissue turnover, and distinct immune-repair dynamics. Beyond structural damage, acute post-traumatic changes in synovial fluid and cartilage metabolism can set the trajectory toward persistent symptoms and post-traumatic osteoarthritis. In this review, we synthesize recent evidence on the biochemical and cytokine milieu following common pediatric and adolescent knee injuries, including Anterior Cruciate Ligament tears, meniscal lesions, and osteochondral trauma. We highlight patt…

Patient-Reported Outcomes of Bicruciate Multiligament Versus Single Cruciate Multiligament Knee Injuries.

BACKGROUND: Multiligament knee injuries (MLKIs) are heterogeneous, and bicruciate knee ligament injuries are considered a serious form of this injury. The current literature tends not to distinguish between single and bicruciate MLKI when reporting outcomes. PURPOSE: To investigate patient-reported outcomes after surgical treatment of MLKI comparing single cruciate MLKI with bicruciate MLKI. The secondary aim was to investigate the influence of different factors on patient-reported outcomes after surgery. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: This study was designed as …

Management of Multiligament Knee Injuries Using Anatomic Autograft Reconstructions: A Case Series.

While proven effective management of multiligament knee injury (MLKI) using allograft is often reported, it has shown an increased risk for graft failure compared with autograft and raises availability and cost issues. Osseous stock preservation and tunnel convergence avoidance led us to develop a compromise innovative surgical procedure using only ipsilateral autograft for anatomic reconstruction of Schenck III-L or higher MLKI. We report the description and early outcomes of our initial experience. Our strategy consisted of an anatomic single-bundle posterior cruciate ligament reconstruction

[Complex knee injuries in football : Management from injury to return to competition].

BACKGROUND: Knee joint injuries are a regular and serious injury in football, often resulting in a long period of absence for players and are, therefore, a significant disadvantage for clubs. The various structures of the knee joint, such as ligaments, meniscus or cartilage, are exposed to the risk of injury due to different sport-specific situations in football and require different and specific therapeutic approaches for their adequate healing. TREATMENT: Both surgical and conservative treatment measures have been well investigated scientifically, especially for knee joint injuries in footb

Comparative Success of Repair Versus Reconstruction for Knee Extra-articular Ligament Injuries.

BACKGROUND: Surgical techniques and associated outcomes in treating acute and chronic extra-articular ligament knee injuries are in evolution, and there is question as to whether repair or reconstruction is optimal. PURPOSE/HYPOTHESIS: The purpose of this study was to compare the subsequent surgery rate between surgical repair versus reconstruction for all extra-articular ligament injuries of the knee utilizing a large database. Our hypothesis was that overall surgical repair of both lateral and medial extra-articular knee injuries would have a higher revision rate than those treated by recon…

The Impact of Race, Insurance, and Socioeconomic Factors on Pediatric Knee Injuries.

Disparities persist in pediatric sports medicine along the lines of race, ethnicity, insurance status, and other demographic factors. In the context of knee injuries such as anterior cruciate ligament (ACL) ruptures, meniscus tears, and tibial spine fractures, these inequalities affect evaluation, treatment, and outcomes. The long-term effects can be far-reaching, including sports and physical activity participation, comorbid chronic disease, and socio-emotional health. Further research is needed to more concretely identify the etiology of these disparities so that effective, equitable care is…

Evaluation and Surgical Management of Knee Osteochondritis Dissecans and Adolescent Cartilage Injuries.

Osteochondritis dissecans and cartilage injuries of the knee are among the most challenging clinical entities that pediatric sports medicine specialists encounter. As a weight-bearing joint with significant long-term implications on mobility and overall physical health, the knee also happens to be the most injured or adversely affected joint in preadolescent and adolescent athletes. However, cartilage injuries are unique among other musculoskeletal pathologies in children, in that the healing potential of articular cartilage tissue is limited, and minor injuries or small focal defects can have

Diagnostic and Management Strategies for Multiligament Knee Injuries: A Critical Analysis Review.

Multiligament knee injuries have a strong association with periarticular soft-tissue and neurovascular injuries, which must not be overlooked in the initial evaluation of the patient. Even though magnetic resonance imaging (MRI) is imperative for a complete evaluation of the damaged ligamentous knee restraints, stress radiography aids in establishing the functional consequence of the MRI findings and may assist in directing reconstruction. Although cruciate ligament tears are generally reconstructed, a combined repair-reconstruction approach is most useful for collateral ligaments and extra-ar…
