Association between immunological diseases and their similar clinical manifestations.

We report on a 30-year-old female patient, with biological mitral valve prosthesis due to symptomatic mitral stenosis and a history of acute myocardial infarction and generalized tonic-clonic seizure episodes, visual hallucinations, cerebral thromboembolic events and, at present, chorea and acute carditis. The patient was diagnosed with active rheumatic fever (RF), systemic lupus erythematosus (SLE) and Antiphospholipid syndrome (APS). The combination of three unusual diagnoses in the same patient makes this a unique case, modifying patient treatment and prognosis.

Near-Infrared Tandem Organic Photodiodes for Future Application in Artificial Retinal Implants.

Photovoltaic retinal prostheses show great potential to restore sight in patients suffering from degenerative eye diseases by electrical stimulation of the surviving neurons in the retinal network. Herein, organic photodiodes (OPDs) sensitive to near-infrared (NIR) light are evaluated as photovoltaic pixels for future application in retinal prostheses. Single-junction and tandem OPDs are compared. In the latter, two nominally identical single-junction cells are processed on top of each other, effectively doubling the open-circuit voltage (V OC ). Both single-junction and tandem OPD micropixels

Dermal Fat Graft for Preventing Extrusion in Tibial Bone Osteo-Keratoprosthesis Exposure.

PURPOSE: Tibial bone keratoprostheses are used in situations where conventional corneal transplantation is not viable. However, the prognosis is often guarded in cases of ocular surface diseases, such as autoimmune conditions or chemical burns, because of the risk of tissue necrosis and subsequent prosthesis extrusion. In this study, we describe an innovative approach using a dermal fat graft to manage keratoprosthesis (KPro) exposure and avoid extrusion. METHODS: We report the case of a man with a history of bilateral chemical burns caused by caustic soda, who underwent tibial bone keratopro

Clinico-Radiological Outcome of Single-Level and Hybrid Total Disc Replacement with Spineart Baguera®-C for Cervical Myeloradiculopathy: Minimum 2-Year Follow-Up Study in Indian Population.

CONTEXT: Cervical radiculopathy and myelopathy is one of the most frequent ailments encountered by spine surgeon. Motion-preserving surgeries in cervical spine is a standard of care due to its certain advantages such as biomechanical anatomical conformity, reduced chances of adjacent segment degeneration, and revision surgeries. While there is abundant data from some centers, data from developing countries are still limited. AIMS: The aim was to study the clinico-radiological outcome of single-level and hybrid total disc replacement (TDR) with Spineart Baguera®-C cervical prosthesis for cervi

Uncemented three-dimensional-printed prosthetic replacement for giant cell tumor of distal radius: a new design of prosthesis and surgical techniques.

INTRODUCTION: Currently, it is challenging to treat giant cell tumor (GCT) of distal radius. For Campanacci grade III or recurrent GCTs, en bloc resection has been accepted as a better treatment option. Although numerous methods are available for reconstruction, all of them have some limitations in joint function and complications. In this study, our aims were to treat the GCT of distal radius with uncemented three-dimensional (3D)-printed prosthesis and to present and evaluate the surgical techniques and short-term outcomes. METHODS: Between September 2015 and March 2017, 11 patients with di

Analysis of outcomes after revision replacement of failed total temporomandibular joint prostheses.

End-stage disease of the temporomandibular joint (TMJ) can be managed successfully with alloplastic total replacements, but these can fail because of allergy, infection, wear, fracture, and heterotopic development of bone. We prospectively reviewed the outcome data of all patients who required revision of previously placed replacement joints between 2004 and 2016. Data included pain and diet scores using 100mm visual analogue scales (VAS), and interincisal distance recorded before, and at six weeks, six months and 12 months after operation. The reasons for failure and the number of previous pr

Budd-Chiari syndrome. Report of a case, successfully treated with a meso-caval shunt.

A young woman with the Budd-Chiari syndrome is described. About 2 months after the onset of her illness, a mesocaval shunt with a 14-mm Gore-Tex prosthesis was performed. She recovered rapidly after the operation and returned to work 5 weeks postoperatively. Two years after the operation, hepatic veins were clearly visualized by ultrasound. The impact of early portosystemic shunting on prognosis in these cases is discussed and the value of modern ultrasonography is emphasized.

Guided Training with Visual Feedback can Improve Direct Electromyography Control of Prosthetic Ankles and Sit-to-Stand Performance: A Case Study.

Direct electromyography (EMG)-controlled prostheses can allow people with amputation (PWA) to directly control their prosthetic ankle. This versatile control can assist with a variety of activities of daily life (ADL), including rising from and sitting down into a chair (i.e., sit-to-stand (S2S)). However, direct EMG control may be limited by a PWA's ability to activate their residual muscles. Physical therapy training with visual feedback may allow PWA to better utilize direct EMGcontrolled prostheses. One PWA (transtibial) completed four training sessions with physical therapy guidance and v

Hip Osteoarthritis Associated with Prominent Synovitis Revealed by 68Ga-FAPI PET/CT.

A 56-year-old patient was initially diagnosed with aseptic loosening of a left acetabular prosthesis based on 68Ga-FAPI. Half a year after revision, this patient developed right hip pain and a subsequent 68Ga-FAPI was performed revealing increased tracer uptake in the right hip, leading to a diagnosis of synovitis-induced osteoarthritis. This case highlights the utility of 68Ga-FAPI-PET/CT in visualizing synovial activity and underscores the potential role of synovitis in the pathogenesis of osteoarthritis.

Endoscopic Stapes Surgery.

The management of stapes fixation using transcanal endoscopic ear surgery is a viable alternative approach. The advantages of transcanal endoscopic ear surgery are improved visualization and illumination of the stapes and oval window regardless of the size and geometry of the external auditory canal. Footplate work and prosthesis placement pose significant but surmountable challenges using transcanal endoscopic ear surgery secondary to the lack of depth perception. Several recent studies demonstrate the efficacy and safety of transcanal endoscopic ear surgery.

Human-in-the-loop optimization of visual prosthetic stimulation.

Objective. Retinal prostheses are a promising strategy to restore sight to patients with retinal degenerative diseases. These devices compensate for the loss of photoreceptors by electrically stimulating neurons in the retina. Currently, the visual function that can be recovered with such devices is very limited. This is due, in part, to current spread, unintended axonal activation, and the limited resolution of existing devices. Here we show, using a recent model of prosthetic vision, that optimizing how visual stimuli are encoded by the device can help overcome some of these limitations, lea

Vestibular ablation and a semicircular canal prosthesis affect postural stability during head turns.

In our study, we examined postural stability during head turns for two rhesus monkeys: one animal study contrasted normal and mild bilateral vestibular ablation and a second animal study contrasted severe bilateral vestibular ablation with and without prosthetic stimulation. The monkeys freely stood, unrestrained on a balance platform and made voluntary head turns between visual targets. To quantify each animals' posture, motions of the head and trunk, as well as torque about the body's center of mass, were measured. In the mildly ablated animal, we observed less foretrunk sway in comparison w

Towards real-time cardiovascular magnetic resonance guided transarterial CoreValve implantation: in vivo evaluation in swine.

BACKGROUND: Real-time cardiovascular magnetic resonance (rtCMR) is considered attractive for guiding TAVI. Owing to an unlimited scan plane orientation and an unsurpassed soft-tissue contrast with simultaneous device visualization, rtCMR is presumed to allow safe device navigation and to offer optimal orientation for precise axial positioning. We sought to evaluate the preclinical feasibility of rtCMR-guided transarterial aortic valve implatation (TAVI) using the nitinol-based Medtronic CoreValve bioprosthesis. METHODS: rtCMR-guided transfemoral (n = 2) and transsubclavian (n = 6) TAVI was pe
