Experimental protocols for ex vivo lens stretching tests to investigate the biomechanics of the human accommodation apparatus.

PURPOSE: To explore alternative experimental protocols to investigate the biomechanical behavior of the crystalline lens and zonules using ex vivo stretching. METHODS: Radial stretching tests were conducted on the anterior segment (consisting of lens, zonules, ciliary body, and sclera) of four pairs of presbyopic human donor eyes. A simple mechanical model is used to describe the behavior of the anterior segment when tested in this way. Each pair of samples was initially stretched with the ciliary body intact. One sample was retested after cutting the ciliary body radially, and the other samp

Monovision-induced motion illusions in presbyopic and non-presbyopic populations.

Monovision is a common correction for presbyopia that focuses one eye at far distances and the other at near distances, resulting in an interocular difference in blur between the eyes. Because blur increases the speed of visual processing by a few milliseconds, these optical conditions can induce dramatic misperceptions of the distances and 3D directions of moving objects. To date, however, the illusion has been demonstrated in only non-presbyopic individuals. We analyze the prevalence of both the processing speed differences and the visibility of the resulting illusions in the presbyopic and …

The ageing lens.

The human lens grows by a process of epithelial cell division at its equator and the formation of generations of differentiated fibre cells. … ght aggregates, may account for the decreased deformability of the lens nucleus which characterises presbyopia. Additional factors include: the progressive increase in lens mass with age, changes in the point of insertion of the lens zonules, and a shortening of the radius of curvature of the anterior surface of the lens. Also with age, there is a fall in li …

Exploring the optimum step size for defocus curves.

PURPOSE: To evaluate the effect of reducing the number of visual acuity measurements made in a defocus curve on the quality of data quantified. SETTING: Midland Eye, Solihull, United Kingdom. DESIGN: Evaluation of a technique. METHODS: Defocus curves were constructed by measuring visual acuity on a distance logMAR letter chart, randomizing the test letters between lens presentations. The lens powers evaluated ranged between +1.50 diopters (D) and -5.00 D in 0.50 D steps, which were also presented in a randomized order. Defocus curves were measured binocularly with the Tecnis diffractive, Re

The horizontal dark oculomotor rest position.

BACKGROUND: This study sought to investigate whether eye dominance and age are related to the stimulus-free oculomotor resting state described via the dark disconjugate position (near or far), the dark conjugate position (left to right), and the near dissociated phoria. METHODS: Nineteen non-presbyopes and 25 presbyopes with normal binocular vision participated in two identical sessions. The left-eye and the right-eye positions were recorded using a video-based infrared eye tracker while the subjects were in total darkness. Dark disconjugate responses and dark conjugate responses were calcula

[Refractive surgery].

The surgical correction of refractive errors has gained widespread acceptance in the past 20 years, mainly through the introduction of the excimer laser. The excimer is used to ablate the cornea, renoving stromal tissue in the center (to correct myopia through flattening of the surface), or in the midperiphery (to steepen the cornea to correct hyperopia). Although excimer procedures dominate the field of refractive surgery, other approaches are also available. In the cornea itself, ring segments can be implanted, heat-induced coagulation effects can produce steepening, and cross-linking of the

Accommodative response in pre-presbyopes with visual impairment and its clinical implications.

PURPOSE: There are isolated reports that accommodative response is reduced in some populations with low vision. The purpose of this study was to measure accommodative response in a wider range of pre-presbyopes with visual impairment and to examine what factors may affect accommodation among the low vision population. METHODS: Accommodative responses for accommodative demands between 4 and 10 D were measured with dynamic retinoscopy in 21 subjects with low vision due to a variety of disorders and in 40 control subjects, aged 3 to 35 years. The control subjects were divided into age groups of

[Seeing in the dark: limited vision in the aged as a prototypical "environmentally relevant" loss in competence].

In the conceptual part of this article, the existential dimension of experiencing a dramatic visual loss in late life is highlighted. Three theoretical avenues are proposed to approach age-related vision loss: 1) a stress and burden perspective; 2) a coping perspective, 3) an environmental psychology perspective. The empirical study is based on 84 elderly subjects suffering from vision loss (42 visually impaired, 42 blind), a comparison group of 42 subjects with mobility impairments, and a control group of 42 healthy subjects (particularly not impaired in vision or mobility). In all impaired g

Diminished foveal sensitivity may predict the development of advanced age-related macular degeneration.

Visual function testing was performed on one eye with drusen from each of 18 elderly patients in 1984. Eleven patients had advanced age-related macular degeneration (AMD) in the fellow eye, and seven had only bilateral drusen. These patients were all followed prospectively (median, 45 months), at which time one eye had a new vessel membrane, three eyes had pigment epithelial detachments, and one eye had geographic atrophy. Only two of these five eyes had AMD-related visual loss in the fellow eye in 1984. The degree of loss of foveal dark-adapted sensitivity in 1984 predicted which patients dev

Analysis of a progressive addition lens population.

Analysis of 98 progressive addition lens (PAL) wearers was undertaken using both mailed questionnaires and by assessing patient records. Statistical analysis was undertaken to investigate the nature and dependent variables of a PAL population. The PAL patients were from the Aston University undergraduate optometric clinic. Social survey techniques, such as the use of personalized mail, first-class stamps, and follow-up letters, were employed to produce a high return rate for the questionnaire. The number of returned questionnaires reached 89.9% of those originally sent which may be considered

Predicting Accommodative Response Using Paraxial Schematic Eye Models.

PURPOSE: Previous ultrasound biomicroscopy (UBM) studies showed that accommodative optical response (AOR) can be predicted from accommodative biometric changes in a young and a pre-presbyopic population from linear relationships between accommodative optical and biometric changes, with a standard deviation of less than 0.55D. Here, paraxial schematic eyes (SE) were constructed from measured accommodative ocular biometry parameters to see if predictions are improved. METHODS: Measured ocular biometry (OCT, A-scan, and UBM) parameters from 24 young and 24 pre-presbyopic subjects were used to co

Bassoonists' medical problems-current state of knowledge.

Specific musical instruments can be a source of physical problems to their players. Based on reviews of the literature and personal experience, this paper summarizes current knowledge of problems affecting musicians who play instruments in the bassoon family (including the bassoon, contrabassoon, and several other instruments). Prevalence rates are higher in reports of surveys (ranging up to 86%), compared to clinical reports of patients seen and treated. Significant risk factors include young age, small body size, female gender, and use of large instruments. Problems unique to bassoonists are
