Unsynchronized nasal intermittent positive pressure versus nasal continuous positive airway pressure in preterm infants after extubation.

OBJECTIVE: To determine the effect of unsynchronized nasal intermittent positive pressure ventilation compared to continuous positive airway pressure in preterm infants after extubation. METHODS: A total of 67 premature infants who were <35 weeks gestation and/or <2000 g birth weight and received mechanical ventilation because of respiratory distress syndrome (RDS) were studied. Infants were randomized to receive either unsynchronized nasal intermittent positive pressure ventilation (NIPPV) with shortened endotracheal tube (Group 1) or nasal continuous positive airway pressure (NCPAP) with bi

Successful pregnancies for ventilator users.

This case series describes full-term pregnancies despite no autonomous ability to breathe due to poliomyelitis or ventilatory insufficiency due to severe kyphoscoliosis. Three women with postpoliomyelitis who were continuously dependent on noninvasive intermittent positive pressure ventilation and one woman who developed ventilatory insufficiency due to severe kyphoscoliosis became pregnant and delivered healthy, full-term babies. They had vital capacities of 240, 250, 280 (5% of normal), and 880 ml (14% of normal), respectively, when becoming pregnant. The up to continuous use of noninvasive

A new laryngeal mask prototype. Preliminary evaluation of seal pressures and glottic isolation.

A new prototype laryngeal mask airway is described which incorporates a second mask to isolate the upper oesophagus and a second dorsal cuff to increase the seal against the glottis. We have made a within-patient comparison of seal pressures between the prototype and standard laryngeal mask airway in 20 patients, and determined if the prototype facilitates functional isolation of the glottis and upper oesophagus. Leak pressure was found to be significantly higher for the prototype and equalled or exceeded 5.0 kPa in all patients (p < 0.001). The prototype laryngeal mask airway provided functio

Prolonged hyperventilation and intracranial pressure.

The effects of PaCO2 changes on intracranial pressure (ICP) often remain even after prolonged hyperventilation. Two representative cases are described where PaCO2 rises were directly related to serious rises in ICP after 4, 6, and 14 days of hyperventilation. This is common in head injuries and suggests that where hyperventilation is used to reduce ICP in the presence of brain swelling, withdrawal of the technique should be cautious.

Home mechanical ventilatory support in patients with restrictive ventilatory disorders: a 48-year experience.

UNLABELLED: We performed a retrospective analysis to the effects of negative pressure ventilation (NPV), tracheal intermittent positive pressure ventilation (TIPPV), and nasal intermittent positive pressure ventilation (NIPPV, volume or pressure-controlled ventilatory mode), in 114 patients with restrictive ventilatory disorders instituted in our hospital from 1956 until 2005. The patients were assigned on "ad hoc" basis to NPV, TIPPV, or NIPPV. All patients were subdivided in an idiopathic kyphoscoliosis group (IK, n=64), a post-poliomyelitis syndrome group (PP, n=30), or a miscellaneous grou

High-frequency oscillatory ventilation in children: A systematic review and meta-analysis.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) is an alternative mechanical ventilation mode proposed to reduce ventilator-induced lung injuries and improve clinical outcomes. The aim of this study was to determine the effects of HFOV compared to conventional mechanical ventilation (CMV) when used in children with hypoxemic respiratory failure. METHODS: The literature search was conducted to identify all studies published before December 2020. Eligible studies included a population aged between 28 days and 18 years old, presented original data from randomized controlled trials (RCT

[Research advances in the methods for weaning from high-frequency oscillatory ventilation in neonates].

Neonatal respiratory failure is a serious clinical illness commonly seen in the neonatal intensive care unit (NICU). Although clinicians want to maximize noninvasive respiratory support, some low-birth-weight preterm infants may require invasive respiratory support from the beginning. As an important respiratory management technique for the treatment of respiratory failure, high-frequency oscillatory ventilation (HFOV) allows gas exchange by rapid delivery at a tidal volume lower than or equal to anatomy death volume. Continuous distending pressure was applied to achieve uniform lung expansion

High-frequency oscillatory ventilation with volume guarantee: a single-centre experience.

OBJECTIVE: High-frequency oscillatory ventilation (HFOV) is widely used in neonatology. The Dräger Babylog VN500 ventilator offers volume-guaranteed HFOV (HFOV-VG) mode when the high-frequency tidal volume (VThf) to be delivered can be set. We investigated how HFOV-VG maintains VThf in the short and longer term and how it affects other ventilator parameters and blood gases. METHODS: We downloaded ~3.2 million seconds (36.7 days) of ventilator data from 17 infants ventilated using HFOV-VG during clinical care with 1 Hz sampling rate. To process and analyse the data, we used the Python computer

A comparison of high-frequency jet ventilation and synchronised intermittent mandatory ventilation in preterm lambs.

PURPOSE: Synchronised intermittent mandatory ventilation (SIMV) and high-frequency jet ventilation (HFJV) are accepted ventilatory strategies for treatment of respiratory distress syndrome (RDS) in preterm babies. We hypothesised that SIMV and HFJV both facilitate adequate oxygenation and ventilation but that HFJV is associated with less lung injury. RESULTS: There were no differences in arterial oxygenation or partial pressure of carbon dioxide despite lower mean airway pressure during SIMV for most of the study. There were no consistent significant differences in end systolic and end diasto

Use of bilevel positive pressure ventilation in patients with bronchiolitis.

RATIONAL: This study aims at describing the use of bilevel positive airway pressure (BiPAP) in infants with severe bronchiolitis. WORKING HYPOTHESIS: The use of BiPAP in infants with bronchiolitis may be associated with a worst outcome. STUDY DESIGN: A single-center retrospective study performed from October 2013 to April 2016. METHODOLOGY: All infants from 1 day to 6 months of age admitted in the pediatric intensive care unit (PICU) were included if they had a clinical diagnosis of bronchiolitis and if they required any type of noninvasive ventilation (NIV), including high flow nasal cannu

Effects of positive and negative pressure ventilation on cerebral blood volume of newborn infants.

The effects of intermittent positive airway and continuous negative extrathoracic pressure ventilation on cerebral blood volume in preterm infants were studied using near infrared spectroscopy. In 12 infants continuous negative extrathoracic pressure caused a median decrease in cerebral blood volume of 0.14 ml/100 ml brain (95% confidence intervals (CI) 0.035-0.280) compared with no respiratory support. Oxygenated and deoxygenated haemoglobin also decreased, implying increased venous drainage as the main effect. In 17 infants intermittent positive pressure ventilation also caused a median redu

Polythene predicament.

BACKGROUND: Hypoxemia complicating care during ventilation is a common problem. OBJECTIVE: To describe an unusual cause of hypoxemia with fluctuating airway pressures in an invasively ventilated, organophosphate-poisoned patient. CASE REPORT: A 40-year-old man being treated for organophosphate poisoning developed episodes of high airway pressure during mechanical ventilation. These episodes initially settled spontaneously. Detailed evaluation failed to reveal any patient-, airway-, or ventilator-related cause for the high airway pressures. On the fourth hospital day, one such episode of high

Mechanical heart-lung interaction in the adult respiratory distress syndrome.

We have discussed the complex hemodynamic changes that occur throughout the respiratory cycle during spontaneous and mechanical ventilation. In patients, many cumulative factors account for the total hemodynamic burden of inspiration and expiration. Thus, the change in pleural pressure, ventilation rate, mode of ventilation, amount of spontaneous ventilation, underlying lung and cardiac disease, and intravascular volume status all affect cardiocirculatory equilibrium. In addition, in the patient with ARDS, various immune mediators that are released into the circulation will affect LV function.

[Optimal positive and expiratory pressure in adult respiratory distress syndrome (author's transl)].

We defined a new optimal positive end expiratory pressure (PEEP) in the adult respiratory distress syndrome (ARDS). The optimal PEEP is the one which allows to obtain a PaO2 greater than or equal to 400 mmHg and/or an intrapulmonary shunt less than or equal to 15 p. cent, the cardiac output being held constant. 14 cases of ARDS have been treated by this method with encouraging results. The earlier optimal PEEP was applied, the more effective it was.

Early intravenous indomethacin prolongs respiratory support in very low birth weight infants.

Infants weighing 1500 g at birth requiring either intermittent positive pressure ventilation or continuous positive airway pressure by 12 hours of age were entered in a randomized double blind controlled trial to test the efficacy of early intravenous indomethacin therapy in preventing chronic pulmonary disease of prematurity. Of the 30 newborns enrolled, 15 were treated with indomethacin and 15 were treated with placebo at 12, 24 and 36 hours of age. The groups were similar for birth weight, gestational age, sex, hyaline membrane disease and intracranial hemorrhage. Infants in the placebo gro

Nasal HFOV versus nasal IPPV as a post-extubation respiratory support in preterm infants-a randomised controlled trial.

Early and successful extubation prevents several morbidities in preterm newborns. Several secondary non-invasive respiratory modalities exist but with their merits and demerits. Given the benefits of nasal high-frequency oscillatory ventilation (nHFOV), we tried to examine whether nHFOV could reduce reintubation rates compared to nasal intermittent positive pressure ventilation (NIPPV) during the post-extubation phase in preterm infants. Stratified randomisation based on gestational age was done for 86 mechanically ventilated preterm infants between 26 and 36+6 weeks of gestation within 2 week
