Comparison of oscillometry with lung function parameters between patients with stable bronchial asthma with airflow obstruction and chronic obstructive pulmonary disease.

BACKGROUND: Lung oscillometry is an emerging investigation for assessing patients with obstructive airway disease. Comparison of oscillometry parameters with bronchodilator responsiveness (BDR) between patients with bronchial asthma and chronic obstructive pulmonary disease (COPD) with comparable severity of airflow obstruction has not been previously investigated. This study aimed to compare oscillometry parameters and bronchodilator responsiveness with other lung function parameters between stable bronchial asthma patients with airflow obstruction and COPD patients with a similar severity of

Small airways obstruction and its risk factors in the Burden of Obstructive Lung Disease (BOLD) study: a multinational cross-sectional study.

BACKGROUND: Small airways obstruction is a common feature of obstructive lung diseases. Research is scarce on small airways obstruction, its global prevalence, and risk factors. We aimed to estimate the prevalence of small airways obstruction, examine the associated risk factors, and compare the findings for two different spirometry parameters. METHODS: The Burden of Obstructive Lung Disease study is a multinational cross-sectional study of 41 municipalities in 34 countries across all WHO regions. Adults aged 40 years or older who were not living in an institution were eligible to participate

Generational changes in lung function in adults from several world regions: results from the Burden of Obstructive Lung Disease study.

This study estimated birth cohort effects on lung function using BOLD (Burden of Obstructive Lung Disease) data from 28 569 adults born between 1902 and 1976 across 41 sites in 34 countries. Forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1) and the FEV1/FVC increased across successive birth cohorts in both high-income countries (HICs) and low- and middle-income countries (LMICs), with mean values rising steadily with later birth year. In fully adjusted models, FVC increased by 21.4 mL, FEV1 by 24.6 mL and FEV1/FVC by 0.25% per birth year. These positive associations were cons

Oscillometry and pulmonary MRI measurements of ventilation heterogeneity in obstructive lung disease: relationship to quality of life and disease control.

Ventilation heterogeneity is a hallmark finding in obstructive lung disease and may be evaluated using a variety of methods, including multiple-breath gas washout and pulmonary imaging. Such methods provide an opportunity to better understand the relationships between structural and functional abnormalities in the lungs, and their relationships with important clinical outcomes. We measured ventilation heterogeneity and respiratory impedance in 100 subjects [50 patients with asthma, 22 ex-smokers, and 28 patients with chronic obstructive pulmonary disease (COPD)] using oscillometry and hyperpol

[Value of ipratropium (Atrovent) in the treatment of chronic obstructive lung diseases in myasthenic patients treated with anticholinesterase agents. Apropos of a case].

We report a case of myasthenia gravis occurring in a 70-year old patient with old chronic bronchitis. Anticholinesterase therapy led to an increase in respiratory symptoms, but relief was obtained with inhaled ipratropium. The activity of this drug was confirmed by spirography. This clinical finding is compared with similar data found in the literature.

[Scientific connotation of spleen-tonifying and lung-replenishing therapy in chronic obstructive pulmonary disease based on "gut-lung axis" theory].

Chronic obstructive pulmonary disease(COPD) characterized by persistent airflow limitation is a heterogeneous lung condition prone to exacerbations and progression. Recent studies have revealed that gut microbiota dysbiosis can trigger metabolic disorders, immune abnormalities, and inflammatory responses, contributing to the pathogenesis and progression of COPD through the gut-lung axis. The spleen-tonifying and lung-replenishing therapy originates from the five elements theory of TCM, specifically the principle of tonifying the source in the case of deficiency. Enhancing the function of the s

The Safety and Efficacy of Secondary Spontaneous Pneumothorax Surgery: Especially in Interstitial Lung Disease and Chronic Obstructive Pulmonary Disease.

OBJECTIVES: To clarify the surgical indications for secondary spontaneous pneumothorax (SSP), we compared the outcomes between interstitial lung disease (ILD)- and chronic obstructive pulmonary disease (COPD)-associated SSP using propensity score matching (PSM) and identified the risk factors for recurrence, prolonged air leakage (PAL), and postoperative complications. METHODS: We retrospectively analysed 138 patients who underwent SSP surgery for the first time. Outcomes were 3-year overall survival (OS), recurrence-free survival (RFS), and short-term outcomes (PAL ≥2 days; postoperative com

Global Initiative for the Diagnosis, Management, and Prevention of Chronic Obstructive Lung Disease. The 2020 GOLD Science Committee Report on COVID-19 and Chronic Obstructive Pulmonary Disease.

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic has raised many questions about the management of patients with chronic obstructive pulmonary disease (COPD) and whether modifications of their therapy are required. It has raised questions about recognizing and differentiating coronavirus disease (COVID-19) from COPD given the similarity of the symptoms. The Global Initiative for Chronic Obstructive Lung Disease (GOLD) Science Committee used established methods for literature review to present an overview of the management of patients with COPD during the COVID-19 pande

Study design and rationale for investigating phosphodiesterase type 5 inhibition for the treatment of pulmonary hypertension due to chronic obstructive lung disease: the TADA-PHiLD (TADAlafil for Pulmonary Hypertension associated with chronic obstructive Lung Disease) trial.

In patients with chronic obstructive pulmonary disease (COPD), moderate or severe pulmonary hypertension (COPD-PH) is associated with increased rates of morbidity and mortality. Despite this, approaches to treatment and the efficacy of phosphodiesterase type 5 inhibition (PDE-5i) in COPD-PH are unresolved. We present the clinical rationale and study design to assess the effect of oral tadalafil on exercise capacity, cardiopulmonary hemodynamics, and clinical outcome measures in COPD-PH patients. Male and female patients 40-85 years old with GOLD stage 2 COPD or higher and pulmonary hypertensio

Genetic, host, and environmental interactions in a 19 year old with severe chronic obstructive lung disease; observations regarding the pathophysiology of airflow obstruction.

A case of a 19-year-old with severe chronic obstructive pulmonary disease is presented. This case illustrates genetic (severe alpha-1 antitrypsin deficiency) and host factors (such as developmental diaphragmatic hernia and the innate response to injury), and environmental (high oxidative stress and lung injury) interactions that lead to severe chronic obstructive lung disease. The development of chronic lung disease was caused by lung injury under high oxidative and inflammatory conditions in the setting of a diaphragmatic hernia. In the absence of normal alpha-1 antitrypsin levels, a pro-elas

Butyrate Regulating Inflammatory Responses in Chronic Obstructive Pulmonary Disease via the Gut-Lung Axis.

Chronic obstructive pulmonary disease (COPD) poses a major global health challenge, yet current pulmonary-directed therapies offer only symptomatic relief without modifying the underlying systemic inflammatory pathology. The gut-lung axis has emerged as a pivotal cross-organ regulatory circuit in COPD progression, and butyrate-a key microbial short-chain fatty acid derived from dietary fibre fermentation-mediates pleiotropic immunomodulatory effects that bridge intestinal homeostasis and pulmonary inflammation. This narrative review synthesises preclinical and limited clinical evidence to eluc

A rare case of lung abscess caused by Mycoplasma pneumoniae in an adult with chronic obstructive pulmonary disease.

BACKGROUND: Lung abscess is a severe pulmonary infection usually caused by anaerobic or pyogenic bacteria. Mycoplasma pneumoniae (MP), a common cause of community-acquired pneumonia, is only rarely implicated in lung abscess formation. Available reports of MP-associated lung abscess have been largely confined to pediatric patients, and adult cases have not been clearly documented to date. CASE PRESENTATION: We report a 74-year-old male with chronic obstructive pulmonary disease (COPD) who presented with chest pain, productive cough, and dyspnea. Chest CT revealed a cavitary lesion with an air-

[The research progress of neutrophil-to-lymphocyte ratio level in chronic obstructive pulmonary disease with lung cancer].

The neutrophil-to-lymphocyte ratio (NLR), as a simple and reliable biomarker of systemic inflammation, has garnered significant attention in the clinical management of various inflammatory diseases and malignancies. By quantifying the relative counts of neutrophils and lymphocytes, this parameter reflects the body's immune-inflammatory status. This article provides a comprehensive review of the current research on the role of NLR in chronic obstructive pulmonary disease (COPD) concomitant with advanced lung cancer (LC) and aims to offer insights into novel diagnostic approaches, prognostic eva

Comorbid depression and autonomic dysfunction reduce lung function in patients with chronic obstructive pulmonary disease.

The high comorbidity rates of depression and chronic obstructive pulmonary disease (COPD) have garnered widespread attention. As a refractory disease, its long-term stress effects exacerbate the coexistence of depression. Depression is linked to a decline in lung function in patients with COPD through reduced heart rate variability, increased inflammatory cytokines, dysregulation of the hypothalamic-pituitary-adrenal axis, and the interplay of various biological and psychological factors. Sole reliance on biomedical treatment cannot fully counteract these negative effects, which are detrimenta

A first-in-kind MAPK13 inhibitor corrects stem cell reprogramming and muco-obstructive lung disease.

UNLABELLED: The stress kinase MAPK13 (aka p38delta-MAPK) is an attractive entry point for therapeutic intervention because it regulates the structural remodeling that can develop after epithelial injury in the lung and likely other tissue sites. However, a selective, safe, and effective MAPK13 inhibitor is not yet available for experimental or clinical application. Here we identify a first-in-kind MAPK13 inhibitor using structure-based drug design combined with a screening funnel for cell safety and molecular specificity. This inhibitor (designated NuP-4A for intravenous or Nu4-B for inhaled d
