Anterior stromal puncture in the treatment of bullous keratopathy: six-month follow-up.

PURPOSE: This study was designed to evaluate the effect of anterior stromal puncture to treat patients with symptomatic bullous keratopathy. METHODS: Fourteen patients with bullous keratopathy who had decreased vision and were or were not awaiting penetrating keratoplasty were evaluated before and 1, 4, 12, and 24 weeks after anterior stromal punctures with a 25-gauge needle. The patients were questioned about pain intensity, foreign-body sensation, and photophobia. Biomicroscopy, esthesiometry, and pachymetry results were evaluated. The results before and after the punctures were compared us

Normal-tension glaucoma and central corneal thickness.

PURPOSE: To determine whether a decreased corneal thickness is a primary factor in the lower intraocular pressure readings found in normal-tension glaucoma patients. METHODS: Fourteen patients with normal-tension glaucoma were compared with 14 age- and sex-matched glaucoma patients. Corneal thickness was measured by ultrasonic pachymetry on each eye. RESULTS: The corneal thickness (mean +/- SD) in patients with normal-tension glaucoma was 0.536 +/- 0.035 mm. The corneal thickness in patients with glaucoma was 0.537 +/- 0.035 mm. The 95% confidence interval for the difference between the two

Delayed onset keratectasia following laser in situ keratomileusis.

We present a case of unilateral iatrogenic keratectasia developing 10 months after bilateral laser in situ keratomileusis (LASIK) involving enhancement surgery using a broad-beam excimer laser (Summit Apex) to treat 6.6 diopters (D) of myopia. The ectasia progressed rapidly over the subsequent 12 months. The surgeon did not measure preoperative pachymetry, but preoperative topography and corneal measurements did not reveal underlying keratoconus or forme fruste keratoconus. Corneal transplantation was required for final visual rehabilitation. Light microscopy of the button revealed no underlyi

Current concepts in crosslinking thin corneas.

Corneal cross-linking (CXL), introduced by Wollensak et al. in 2003, is a minimally invasive procedure to halt the progression of keratoconus. Conventional CXL is recommended in eyes with corneal thickness of at least 400 microns after de-epithelialization to prevent endothelial toxicity. However, most of the keratoconic corneas requiring CXL may not fulfill this preoperative inclusion criterion. Moderate-to-advanced cases are often found to have a pachymetry less than this threshold. There are various modifications to the conventional method to circumvent this issue of CXL thin corneas while

Surface and Orbscan II slit-scanning elevation topography in circumscribed posterior keratoconus.

We used an Orbscan II topography system (Bausch & Lomb) to study anterior and posterior surface abnormalities, keratometry, and topographic pachymetry in a patient with circumscribed posterior keratoconus. This system clearly showed a marked localized paracentral annular elevation in the posterior corneal surface that corresponded to an abrupt decrease in thickness and a slightly localized anterior surface bulge in the anterior float.
