Application of Surgery-First Approach in Treating Skeletal Dentofacial Asymmetry with Orthognathic Surgery.

BACKGROUND: Skeletal dentofacial asymmetry decreases patient's attractiveness by deteriorating symmetry of facial appearance which is of great significance to individuals. Surgery-first orthognathic approach manifests its advantages of shortening treatment time and improving patient's quality of life. However, current literature on surgery-first approach mainly focuses on treating prognathism, overlooking its efficacy in improving facial symmetry of skeletal dentofacial asymmetry patients. This study aimed to assess SFA's efficacy in improving facial appearance by analyzing morphological featu…

Treatment Effect of Combined Surgical Maxillary Expansion and Mandibular Setback in Skeletal Class III.

The purpose of this case report is to describe and discuss a combined surgical and orthodontic technique for the management of transverse maxillary deficiency and mandibular prognathism in the treatment of skeletal Class III malocclusion in a mature patient. Skeletal Class III malocclusion can present with maxillary deficiency or retrognathism, mandibular excess or prognathism, or a combination. The maxillary arch is narrow and often requires expansion. A 25-year-old patient presented with a constricted maxilla, a skeletal Class III malocclusion with a large mandible, Angle's Class III maloccl…

Diffuse polymicrogyria associated with an unusual pattern of multiple congenital anomalies including turribrachycephaly and hypogenitalism.

A newborn male infant born to consanguineous parents was found to have diffuse polymicrogyria associated with an unusual pattern of congenital anomalies including microbrachycephaly, turricephaly, blepharophimosis, microstomia with maxillary retrusion and mandibular prognathism, micropenis with cryptorchidism, camptodactyly and adducted thumbs, and a progeroid appearance. The combination of manifestations in our patient represents a unique form of polymicrogyria with congenital anomalies, probably representing a new syndrome.

[Placental chorioangioma and fetal abnormalities].

We report on a postpartally decreased girl with several malformations: turricephalus, deep root of the nose, prognathism, choanal atresia, low-set dysplastic ears, fourth additional blood vessel originating from the thoracic aorta. Nearly 70% of the placenta consisted of multiple chorioangiomas. A possible correlation between fetal malformations and placental chorangiomas is discussed. There is no valuable reference to a hereditary component in the development of chorioangiomas.

Partial trisomy-5p.

Two sibs with partial trisomy-5p are reported. Their father is the carrier of a balanced translocation 46,XY,t(4q+;5p-). Twelve cases of partial trisomy-5p--including our two patients--have been reported. The most common abnormalities found were mental retardation, short stature, dolichocephaly, prominent nasal bridge, prognathism, seizures, hypotonia, ear abnormalities, increased ulnar loops on the fingertips, and cryptorchidism in affected males.

Anodontia of the permanent dentition: fourteen years after initial diagnosis.

The congenital absence of teeth, total anodontia, may involve both the primary and permanent dentitions. This paper updates the dental history of a twenty-eight-year-old Greek-American woman first reported in 1977 by Herman and Moss, when she was fourteen. Five complete sets of dentures had been made for her in thirteen years. Treatment was more complex due to an underdeveloped maxilla and a prognathic mandible.

Modified facial bipartition.

Orbital hypertelorism is a craniofacial abnormality that arises on its own or as part of a number of syndromes. It is not uncommon to find the condition in association with maxillary hypoplasia. This manuscript reports an uncommon case of Tessier 2, 12 with orbital hypertelorism and dentoskeletal maxillary prognathism. To correct the condition, the first stage procedure was a modification of facial bipartition, according to the need of the case, followed by correction of alar cleft by Denonvilliers technique.

Mandibular deformity associated with Paget's disease. Case report.

Paget's disease (osteitis deformans) and osteoporosis represent the commonest systemic bone disorders. Clinically Paget's disease may be mono-, or polyostotic, and is characterized by excessive and disorganized formation and resorption of bone. It may affect any of the facial bones, most notably the skull and the jaws. A case is described in which the only symptomatic manifestation of Paget's disease was mandibular prognathism. The pathogenesis, aetiology, and management are discussed.

Addressing Acromegaly-Related Malocclusion With Surgery-First Orthognathic Surgery: A Clinical Case Report.

Angle's class III malocclusions are characterized by the anterior positioning of the mandible in relation to the maxilla. The discrepancy can be caused by an anterior deficiency of the maxilla, excessive mandibular prognathism, or a combination of both. Acromegaly is a dysfunction caused by the excessive production of growth hormone (GH), which leads to systemic changes and orofacial manifestations. In acromegaly caused by a pituitary adenoma, which secretes an excessive amount of GH, disproportionate mandibular growth may occur, leading to skeletal class III malocclusion in adulthood. Excessi…

Technical modification of the sagittal split mandibular ramus osteotomy.

The sagittal split mandibular ramus osteotomy is a versatile surgical option for the correction of mandibular prognathism, retrognathism, and asymmetry. At the same time, it enables the use of internal rigid fixation. However, surgical complications including injury to the inferior dental neurovascular bundle and unfavorable fracture can occur. A technical modification and a concept in instrumentation is suggested with the view of minimizing such complications.

Tongue movements in patients with skeletal Class II malocclusion evaluated with real-time balanced turbo field echo cine magnetic resonance imaging.

INTRODUCTION: The aim of this study was to evaluate the deglutitive tongue movements in patients with skeletal Class II malocclusion. METHODS: Fifty-nine patients (26 male, 33 female) with skeletal Class II relationship were divided into 3 groups according to cephalometric analysis. Group 1 (n = 19) had mandibular retrognathism, group 2 (n = 20) had maxillary prognathism, and group 3 (n = 20) had both mandibular retrognathism and maxillary prognathism. Twenty-two skeletal Class I patients (10 male, 12 female) were also included as the controls. RESULTS: In the mandibular retrusion group, the…

Long-term follow-up of patients with a skeletal Angle Class III relationship treated with chin caps.

AIM: To evaluate the long-term result of chin-cap treatment in patients with a skeletal Angle Class III relationship due to mandibular prognathism. MATERIAL AND METHODS: Records of nine patients (five girls and four boys) with a mean age of 10.8 ± 1.6 years at the start of treatment were retrieved from the archives. The records comprised lateral cephalograms, intra- and extraoral photographs, and dental casts. The subjects were recalled at 6.1 ± 1.5 years after the completion of active orthodontic treatment. Cephalometric measurements from the treatment's beginning (T1), end (T2), and follow-…

Anatomical similarity between the Sost-knockout mouse and sclerosteosis in humans.

Sclerosteosis, a rare autosomal recessive genetic disorder caused by a mutation of the Sost gene, manifests in the facial skeleton by gigantism, facial distortion, mandibular prognathism, cranial nerve palsy, and, in extreme cases, compression of the medulla oblongata. Mice lacking sclerostin reflect some symptoms of sclerosteosis, but this is the first report of the effect on the facial skeleton. We used geometric morphometrics (GMM) to analyze the deformations of the murine facial skeleton from the wild-type to the Sost gene knockout. Landmark coordinates were obtained by surface reconstruct…

Two-stage mandibular setback employing modified Dingman's mandibular body osteotomy with step for large antero-posterior dentofacial deformity correction: a report of two cases.

Mandibular prognathism is a well-known skeletal discrepancy with varying prevalence rates demographically. Patients with severe prognathism may experience difficulties speaking and chewing food as well as problems with psychosocial adjustment, indicating the need for surgical correction. In cases of single-stage larger mandibular setbacks, complications such as reduction in the pharyngeal airway and instability can be encountered. Therefore, two-stage orthognathic surgery could be recommended. Two patients (17-year-old female and 16-year-old female) reported with large mandibular antero-poster…

Kleefstra Syndrome-Dental Manifestations and Needs: A Case Report with a Literature Review.

Kleefstra syndrome (KS) is a rare genetic disorder (prevalence < 1/1 000 000) characterised by autistic spectrum disorder (ASD), childhood hypotonia, and seizures. A typical facial appearance includes microcephaly, arched eyebrows, synophrism, hypertelorism, a short nose, midface hypoplasia, prognathism, and tongue protrusion. This case report presents a 19-year-old female with KS from a dentist's perspective. The patient demonstrates limited mouth opening with a slight deviation of the mandible on the left side. The mandibular prognathism and anterior open bite are visible. A generalised infl…
