Effectiveness of Cognitive Enhancement Programs in Parkinson's Disease Patients with Mild Cognitive Impairment: A Narrative Review.

OBJECT: Parkinson's disease (PD) is a chronic progressive neurodegenerative disease. Although PD has been associated mainly with the onset of motor symptoms, cognitive impairment is an equally important feature of the disease. The high prevalence of cognitive impairment in PD makes the implementation of cognitive enhancement programs particularly important. AIM: This narrative review evaluates the effectiveness of cognitive enhancement programs in Parkinson's disease patients with mild cognitive impairment (PD-MCI) according to the results of English language studies that have been published …

What transcranial direct current stimulation intensity is best for cognitive enhancement?

In a recent study published in The Journal of Neurophysiology, Ehrardt et al. (Ehrhardt SE, Filmer HL, Wards Y, Mattingley JB, Dux PE. J Neurophysiol 125: 385-397, 2021) report that moderate intensity (1 mA/25 cm2) transcranial direct current stimulation (tDCS) is optimal for improving performance on a stimulus-response matching task, as opposed to a lower 0.7 mA/25 cm2 or higher 2 mA/25 cm2 dose. This result suggests that behavioral effects of tDCS do not follow a linear dose-response curve. Potential neurobiological and neurocognitive implications of these findings, as well as suggested dire

[Review of cognitive enhancement techniques based on the combination of cognitive training and transcranial direct current stimulation].

Cognitive enhancement refers to the technology of enhancing or expanding the cognitive and emotional abilities of people without psychosis based on relevant knowledge of neurobiology. The common methods of cognitive enhancement include transcranial direct current stimulation (tDCS) and cognitive training (CT). tDCS takes effect quickly, with a short effective time, while CT takes longer to work, requiring several weeks of training, with a longer effective time. In recent years, some researchers have begun to use the method of tDCS combined with CT to regulate the cognitive function. This paper…

Problems in the search for cognition enhancers.

Drugs which enhance cognition may benefit the two largest groups of psychiatric patients who have been bypassed during the psychopharmacological revolution of the past 35 years: those with development mental disabilities and those with brains ravaged by senile dementias. But who among us would not benefit from such a drug, if indeed it could enhance normal performance? Establishing suitable animal models for developing such drugs has been a major problem. Lesioning experiments dictate the answers as well as the questions, delimiting the type of drug to remedy the defect. Other lesions which ar

Dissociable neural mechanisms of cognitive enhancement through transcranial stimulation and behavioral training.

BACKGROUND: Transcranial direct current stimulation (tDCS) and adaptive working memory (WM) training are promising cognitive enhancement approaches; however, their neural mechanisms and potential synergies remain poorly understood. OBJECTIVE: We directly compared how tDCS and WM training modulate neural oscillations during WM performance and examined whether combining both interventions produces additive effects. METHODS: We randomized 112 healthy adults into four groups: control (sham tDCS + non-adaptive 1-back), tDCS-only (active tDCS + non-adaptive 1-back), training-only (sham tDCS + adap…

Cognitive control enhancement in attention deficit hyperactivity disorder (ADHD) and neurotypical individuals.

Cognitive control, which has been localized to the right inferior frontal gyrus (rIFG) based on functional imaging and brain lesion studies, is impaired in patients with ADHD. The present study aims to investigate whether transcranial direct current stimulation (tDCS) over the rIFG might improve cognitive control in ADHD subjects. We hypothesized poorer performance in a cognitive control task, but not in a control language task, in the ADHD subjects. Crucially, following tDCS, we expected the ADHD group to improve their cognitive control. In a double-blind randomized control trial, 42 particip

Caffeine in Aging Brains: Cognitive Enhancement, Neurodegeneration, and Emerging Concerns About Addiction.

This narrative review examines the effects of caffeine on brain health in older adults, with particular attention to its potential for dependence-an often-overlooked issue in geriatric care. Caffeine acts on central adenosine, dopamine, and glutamate systems, producing both stimulating and rewarding effects that can foster tolerance and habitual use. Age-related pharmacokinetic and pharmacodynamic changes prolong caffeine's half-life and increase physiological sensitivity in the elderly. While moderate consumption may enhance alertness, attention, and possibly offer neuroprotective effects-esp

Application of Noninvasive Brain Stimulation for Sleep Quality Enhancement and Cognitive Improvement.

Sleep is a fundamental process for maintaining our physical and mental health by adjusting brain homeostasis and repairing axons to refresh memories. Due to its essentiality, sleep disorders and insufficiency can cause both physiological and behavioral risks. This report diverges from traditional medical treatments and focuses mainly on physiotherapy-based neuroregulatory techniques for sleep treatment. Noninvasive brain stimulation (NiBS) techniques have been developed to enhance patients' sleep and memory, including transcranial electrical stimulation (tES), transcranial magnetic stimulation

Impact of COMT val158met on tDCS-induced cognitive enhancement in older adults.

BACKGROUND: Previous studies suggest that genetic polymorphisms and aging modulate inter-individual variability in brain stimulation-induced plasticity. However, the relationship between genetic polymorphisms and behavioral modulation through transcranial direct current stimulation (tDCS) in older adults remains poorly understood. OBJECTIVE: Link individual tDCS responsiveness, operationalized as performance difference between tDCS and sham condition, to common genetic polymorphisms in healthy older adults. METHODS: 106 healthy older participants from five tDCS-studies were re-invited to don

Cognitive Enhancement as Transformative Experience: The Challenge of Wrapping One's Mind Around Enhanced Cognition via Neurostimulation.

In this paper, the authors explore the question of whether cognitive enhancement via direct neurostimulation, such as through deep brain stimulation, could be reasonably characterized as a form of transformative experience. This question is inspired by a qualitative study being conducted with people at risk of developing dementia and in intimate relationships with people living with dementia (PLWD). They apply L.A. Paul's work on transformative experience to the question of cognitive enhancement and explore potential limitations on the kind of claims that can legitimately be made about individ…

Meta-analysis of randomized controlled trials of galantamine in schizophrenia: significant cognitive enhancement.

Cognitive impairments are core features of schizophrenia and the best predictor of functional outcome. Cholinergic system and alpha-7 nicotinic acetylcholine (α7nACh) receptors are strongly implicated in the pathophysiologic mechanisms associated with cognitive impairments in schizophrenia. Galantamine is not only a reversible, competitive inhibitor of acetylcholinesterase but also a type I positive allosteric modulator of α7nACh receptors. The objective of this meta-analysis was to examine the efficacy of galantamine for cognitive symptoms of schizophrenia. In the meta-analysis that included

Cognitive enhancement with Salience Network electrical stimulation is influenced by network structural connectivity.

The Salience Network (SN) and its interactions are important for cognitive control. We have previously shown that structural damage to the S … the causal influence of this network on cognitive control and confirming it as a target to produce cognitive enhancement. Our results also highlight the importance of structural connectivity as a modulator of network to TDCS, which should guide the design and interpretation of future stimulation studies.

Retaining pathological gamblers in cognitive behavior therapy through motivational enhancement: A pilot study.

Treatment for pathological gambling is in its infancy. Several cognitive and behavioral interventions have shown promise, but high attrition and relapse rates suggest that gamblers requesting treatment are not uniformly committed to change. This article describes an exploratory study with 9 severe pathological gamblers--in their majority horse race bettors--who were recruited from a community treatment center. The gamblers were treated with a hybrid intervention consisting of motivational enhancement and cognitive behavior therapy. All gamblers were retained in treatment and during a 12-month

From Treadmill to Textbook: Neurovascular and Immune Mechanisms of Exercise-Induced Cognitive Enhancement.

It has been well established that physical exercise is associated with augmented cognitive function, but the exact mechanisms by which peripheral activity affects the plasticity of the brain remain mechanistically elusive. This narrative review synthesises evidence from mouse models and human studies to clarify how physical exercise may enhance hippocampus-dependent cognition through systemic, neurovascular, immune, and metabolic mechanisms instead of direct "brain training" alone. This distinction is supported by the limited far transfer shown in many cognitive training models, wherein improv

Neuronal ketone body utilization couples exercise and time-restricted feeding to cognitive enhancement.

Ketogenesis and ketone body metabolism are linked to brain health benefits, including delaying age-related cognitive decline and neurodegeneration. Exercise, particularly when combined with an overnight fast, stimulates ketogenesis and ketone body turnover as well as improves brain metabolism and cognition. Yet, whether ketone metabolism is obligatory for this response is unknown. Here, we use chronic exercise via voluntary wheel running plus time-restricted feeding (VWR+TRF, fasting from ZT10.5-18.5) to explore whether ketone bodies are a potential mediator of exercise-induced brain health be
