BACKGROUND: Although assisted ventilation has reduced the mortality rate of premature infants, pulmonary disease is still the major cause of morbidity and mortality in very low birth weight infants. We designed this study to evaluate the efficacy and safety of high-frequency oscillatory ventilation (HFOV) in premature infants and to compare the outcome for early intervention with HFOV versus conventional ventilation (CV).
METHODS: From January, 1997, to June, 1998, we analyzed premature infants with respiratory failure who required mechanical ventilation and supplemental oxygen to support ade

There are three forms of high frequency ventilation, high frequency jet ventilation (HFJV, up to 400/min), high frequency oscillation (HFO, up to 40 Hz), and high frequency positive pressure ventilation (HFPPV, rates between 60 and 150/min). The first two forms of ventilation are still experimental and have been used only in critically ill children where respiratory failure has been unresponsive to more conventional therapy. Unfortunately, however, HFJV has already been associated with a high incidence of tracheal lesions. High-frequency positive pressure ventilation, on the other hand, using

High-frequency ventilation (HFV), currently under investigation in three categories: high-frequency positive pressure ventilation (HFPPV), high-frequency jet ventilation (HFJV), and high-frequency oscillation (HFO), is a new form of mechanical ventilation that employs small tidal volumes in relation to dead space and extremely rapid rates, ranging from 1 to 40 Hz. It has a number of theoretical advantages when compared with current methods of conventional ventilation, and provides adequate gas exchange using minimal proximal airway pressure with little circulatory interference. Reports of succ

During ventilation, the lungs may serve as accessory pumps for forward blood flow. We have noted significant hemodynamic improvement in several patients during short periods of vigorous hand ventilation with the Ambu bag and tried to reproduce the hemodynamic improvement in the management of four patients with profound low cardiac output syndrome secondary to right ventricular (RV) failure following open heart surgery. High frequency, high volume (HFHV) ventilation resulted in improved hemodynamics in these patients, as evidenced by pulsatile pulmonary blood flow and increased cardiac output.

By virtue of applying small tidal volumes, high-frequency ventilation is advocated as a method of minimizing ventilator-induced lung injury. Lung protective benefits are established in infants, but not in other patient cohorts. Efforts to improve and extend the lung protection potential should consider how fundamental modes of gas transport can be exploited to minimize harmful tidal volumes while maintaining or improving ventilation. This research investigates different models of gas transport during high-frequency ventilation and discusses the extent to which the gas transport mechanisms are c

OBJECTIVES: High-frequency jet ventilation (HFJV) is a convenient method for providing ventilation during fiberoptic bronchoscopy. We describe an incipient approach of high-frequency jet ventilation via the working channel of a flexible bronchoscope for nonintubated patients who suffer from hypoxemia during bronchoscopy. The aim of this study was to test the efficacy of this incipient approach and determine the possible complications related to it.
MATERIALS AND METHODS: Sixteen patients who had oxygen saturation below 70% that did not resolve with nasal oxygen for 20 s during interventional

AIM: To assess the use of nasal high-frequency ventilation (HFV) to provide noninvasive ventilatory support for very low birthweight (VLBW) infants.
STUDY DESIGN: VLBW infants, >7 days of age on nasal continuous positive airway pressure (CPAP), were placed on nasal HFV for 2 h using the Infant Star high-frequency ventilator (Mallinckrodt, Inc., St. Louis, MO, USA). Mean airway pressure was set to equal the previous level of CPAP, and amplitude was adjusted to obtain chest wall vibration. Capillary blood was sampled before starting HFV and after 2 h to determine change in pH and partial pressu

Despite improvements in respiratory care, ventilator-induced lung injury remains an important cause of morbidity and mortality in neonates who require assisted ventilation. Animal data clearly demonstrate that high-frequency ventilation can be used successfully to reduce lung injury in experimental models of acute lung injury. These models and human research show that the efficacy of high-frequency ventilation is dependent on optimizing functional residual capacity and avoiding lung overinflation. When used with a strategy that promotes lung recruitment, high-frequency ventilation effectively

With the addition of appropriate logic circuitry and a single-board microcomputer, a thermistor flowmeter becomes a clinically useful ventilation monitor. The microcomputer calculates upper and lower reference (voltage) levels from the baseline (normal) flow signal generated by the thermistor bridge. If the flow signal fails to pass repetitively through one or both of these reference levels, visual and audio alarms are generated. Tests performed with both high-frequency (3 Hz) and low-frequency (0.2 Hz) ventilator systems show the usefulness of this system in sensing a variety of malfunctions,

BACKGROUND: Studies that investigate the use of high-frequency percussive ventilation (HFPV) are scarce and outdated. HFPV is being used in some facilities primarily as a salvage modality on patients with acute respiratory distress syndrome (ARDS) when a conventional mechanical ventilator has failed to improve outcomes. There still are arguments whether HFPV is an effective ventilation strategy for adult patients with ARDS.
OBJECTIVES: The purpose of this study was to analyze and compare pulmonary gas exchange, as measured by arterial blood gas (ABG) values, between patients with ARDS who wer

Ten randomly selected patients were ventilated for defined periods with 2 ventilatory modes: (a) high frequency positive pressure ventilation (HFPPV) (frequency 66-70 min; tidal volume 1-3 ml/kg body weight), (b) conventional IPPV (frequency 16/min; tidal volume (VT) 10-15 ml/kg). This was done successfully using conventional ventilators, and keeping other variables (FIO2, PEEP) constant. Various ventilatory and hemodynamic variables were measured and compared during both modes of ventilation. The most prominent finding was a considerable reduction of cardiac output (CO) and stroke volume (SV)

In this issue of Critical Care, Bollen and colleagues present the results of a multicentre randomised controlled trial, comparing high-frequency oscillatory ventilation with conventional ventilation as the primary ventilation mode for adults with acute respiratory distress syndrome. The study was stopped early after recruiting only 61 patients because of declining enrolment, and although no differences were detected in any primary or secondary endpoint, this trial only had sufficient power to detect extreme differences in outcomes between groups. This editorial attempts to put these results in

INTRODUCTION: High-frequency percussive ventilation (HFPV) is a ventilation mode characterised by high-frequency breaths. This study investigated the impact of HFPV on gas exchange and clinical outcomes in acute respiratory failure (ARF) patients during spontaneous breathing, noninvasive ventilation (NIV) and invasive mechanical ventilation (iMV).
METHODS: This systematic review included randomised and nonrandomised studies up to August 2023. Inclusion criteria focused on adult ARF patients, HFPV application, comparisons with other ventilation modes, and outcomes related to oxygenation and cl

BACKGROUND: The optimal role of high frequency jet ventilation (HFJV) in lung protective stabilization of congenital diaphragmatic hernia (CDH) remains uncertain. We aimed to describe our center's experience with HFJV as both a rescue (following failed stabilization with CMV) and primary ventilation mode in the management of CDH.
METHODS: Liveborn CDH patients treated from 2013 to 2021 in a single institution were reviewed. We compared 3 groups based on their primary and last ventilation mode prior to surgery: CMV (Group 1); HFJV (Group 2); and CMV/HFJV (Group 3). Outcomes included a composit

INTRODUCTION: High frequency jet (HFJV) and oscillatory (HFOV) ventilation were used to rescue newborns with congenital diaphragmatic hernia (CDH), who failed conventional mechanical ventilation (CV). Changes in ventilator settings and pulmonary gas exchange were evaluated following transition to high frequency ventilation (HFV).
METHODS: Records of patients with CDH rescued with HFV prior to surgical intervention between 2006 and 2015 were reviewed. Mean airway pressure (Pāw) and arterial blood gases during CV and those obtained within the first hour of HFV were compared. A composite repeate

High frequency oscillatory ventilation (HFOV) has emerged over the past 20 years as a safe and effective means of mechanical ventilatory support in patients with acute respiratory failure. During HFOV, lung recruitment is maintained by application of a relatively high mean airway pressure with superimposed pressure oscillations at a frequency of 3 to 15Hz, creating adequate ventilation using tidal volumes less than or equal to the patient's dead space volume. The physiologic rationale for the application of HFOV in the clinical arena comes from its ability to preserve end-expiratory lung volum

In recent years, the usefulness of high frequency ventilation (HFV) has been clinically reassessed as an alternative to conventional mechanical ventilation (CMV). HFV has often been combined with or in some cases even completely replaced CMV in the attempt to reduce iatrogenic injury. High frequency percussive ventilation (HFPV) is a specific mode of HFV that has been successfully applied in the treatment of acute respiratory failure after smoke inhalation; it has also been more widely used in pediatric than in adult patients. This article gives an introduction to and a description of the basi
