Prolonged erection is a rare problem in urology but it must be treated effectively. The most common etiological factor is intracavernosal vasoactive agent injection for diagnosis or treatment of erectile dysfunction. The aim of this study was to evaluate the efficacy of intracavernosal adrenalin injection alone in the treatment of priapism. Nineteen patients with prolonged erection were evaluated. Seventeen out of the 19 prolonged erections were due to intracavernosal vasoactive agent injection and the remaining two were idiopathic. One hundred ninety-seven cases of priapism have been recorded in the medical files of Charity Hospital Louisiana in New Orleans from 1938-1982. One hundred seventy-two adults and 25 children were registered in this series. Although priapism secondary to hematologic malignancies has been well described, review of the literature reveals 75 case reports of priapism due to local corporeal involvement by solid tumors. An 18-year-old black man with known history of homozygous (SS) sickle cell disease presented with sudden onset of priapism awakening him from sleep. Priapism as an isolated manifestation, without other signs or symptoms of sickle cell crisis, is thought to be more prevalent than previously reported. The patient was taken to surgery, where 14-gauge intercaths were used to aspirate blood from his corpora cavernosa bilaterally. Complete detumescence was achieved. At a five-month follow-up visit, normal erectile capacity was reported.

An unusual case of priapism is described. The cause was unknown but the onset of the condition coincided with the start of Coumadin therapy which was given orally to a 65-year-old man who had undergone aortic valve replacement. Management of this patient was difficult. Conservative measures failed, but a corpus spongiosum--cavernosum shunt successfully resolved the problem. The authors emphasize that priapism must be treated urgently if potency is to be maintained. They also consider the various possible causes and treatment of priapism.

Due to the poor knowledge concerning etiology and pathophysiology of priapism the treatment necessarily remains largely symptomatic. There are two main therapeutic aims: 1. UNLABELLED: The authors review the literature with regard to surgery for priapism performed in 138 patients: 92 treated by uni- or bilateral sapheno-cavernous anastomosis and 46 by spongio-cavernous shunt.
