A lesion of endodontic origin misdiagnosed as a globulomaxillary cyst.

This report presents a case of a lesion originating from a pulpless central incisor misdiagnosed as a globulomaxillary cyst. The initial diagnosis of globulomaxillary cyst was made solely from radiographs by observing an almond-shaped radiolucency between the lateral incisor and cuspid. No pulp testing was performed before surgical removal of the lesion. The final diagnosis of an endodontic lesion caused solely by a pulpless central incisor was complicated by the initial lack of a pulpal response from the lateral incisor next to the surgical site. By delaying the start of endodontic treatment,

A new method for the quantitative analysis of endodontic microleakage.

The aim of this in vitro study was to evaluate the apical seal obtained with three commonly used root canal sealing cements: Sealapex, AH Plus or Topseal, and Sealite, using a new method based on the quantitative analysis of 125I-radiolabeled lysozyme penetration. One hundred thirteen teeth with straight single root canals were instrumented to master apical point #25/30. These were divided into three groups: (i) negative control (4 roots) covered with two layers of nail polish, (ii) test group (105 roots) obturated by laterally condensed guttapercha with the three cements; and (iii) positive c

Dentinogenesis imperfecta: endodontic implications. Case report.

Dentinogenesis imperfecta is a hereditary disorder resulting in defective dentin in both the primary and secondary dentitions. The complications of dentinogenesis imperfecta are difficult to manage and provide a challenge to the dentist. This case report concerns treating an African American patient with dentinogenesis imperfecta who appeared for treatment with endodontic pathosis. It illustrates the need for appropriate and timely restorative treatment to prevent pulpal pathosis. Also demonstrated is the difficulty of endodontically treating dentinogenesis imperfecta teeth because of pulpal o

Local and systemic therapeutics for the control of endodontic pain.

Endodontic pain is best managed by eliminating the source as completely as possible. When it is determined that drugs are required, judicious use of local anesthetics and nonopioid analgesics can be beneficial and provide the first course of action. When it is determined that the pain will not be managed by these measures alone, the addition of an opioid analgesic may be considered with the understanding that more adverse side effects attend their use.

Orthodontic-endodontic treatment planning of traumatized teeth.

Occasionally, an orthodontic patient will accidentally traumatize a maxillary anterior tooth before or during orthodontic treatment. In some situations, the trauma will be substantial and avulse the tooth. In other accidents, the tooth may not avulse, but the pulp becomes nonvital. If the pulp is devitalized, and the root has not fully formed, the apex of the root canal may be wide. In this situation, the endodontist may recommend apexification procedures to help close the apex before conventional obturation of the root canal. If the patient is currently undergoing orthodontic movement of the

Problems attaining local anesthesia with endodontic emergencies.

The attainment of profound local anesthesia in the management of pulpal and periradicular pain often eludes even the most skilled clinician; various reasons for this include, but are not limited to, localized inflammation, osseous and neural variations, and poor technique of the operator. This paper reviews these potential impediments while focusing on recognizing occurrences, understanding mechanisms, and applying clinical techniques to effectively eliminate their impact on dental pain management.

Managing local anesthesia problems in the endodontic patient.

Root canal therapy has a poor public image related to occasional and sometimes severe pain, and to dentists' inability to obtain profound anesthesia. Patients' apprehension, in combination with tissue inflammation, significantly lowers the pain threshold, which decreases the anesthetic's effectiveness. The best and also the first approach to achieving anesthesia is to administer a conventional block or infiltration. If profound anesthesia does not occur after this attempt, use a supplemental technique such as lingual infiltrations, PDL or intrapulpal injections. Because infiltrations are gener

Comparative tissue tolerance of a new endodontic sealer.

An experimental radiopaque resin root canal sealer was tested for tissue biocompatibility in rat connective tissue. The controls were four widely used sealers: Sealapex, Kerr's sealer, AH-26, and Roth's sealer. Polyethylene tubes were filled with the sealers and allowed to set for 24 hours in 100% humidity. Each sealer was implanted in the connective tissue of Sprague-Dawley rats and removed after 3, 10, 20, 30, and 60 days, fixed, and histologically prepared for light microscope evaluation. At each time period there was practically no difference in the reaction of all the sealers. By 60 days,

Endodontic management of the avulsed tooth and tooth transplantation.

The major causes of post-transplantation tooth loss following tooth replacement after avulsion (exarticulation) and surgical replacement of impacted maxillary canines (auto-transplantation) are inflammatory root resorption and replacement resorption (ankylosis). This paper attempts to outline the treatment and management of replanted teeth using the current philosophies of splinting, endodontic therapy and the use of calcium hydroxide in the re-attachment of the periodontal ligament (PDL). Clinical research during the past 20 years has provided sufficient information for the development and es

Intracanal use of a corticosteroid solution as an endodontic anodyne.

In fifty consecutive clinical patients with endodontically involved teeth that responded with vital pulp test readings, a corticosteroid solution and a saline placebo were alternately used as an intracanal anodyne. The patients subjectively rated their pain on a scale of 0 to 100 as "none," "mild," "moderate," and "severe" (0 = none, 1 to 33 = mild, 34 to 66 = moderate, 67 and above = severe). The ratings were done before the first treatment and then at 24, 48, and 72 hours after treatment. Corticosteroid-treated and placebo-treated cases were not significantly different in terms of type of te

[Indications for the treatment of bone rarefactions of endodontic origin].

Bone rarefactions arising from dental lesions are commonly thought to be sustained by the microbiol component or by the toxins spread out into the surrounding bone by the disintegrated pulp. In thise case endodontic therapy is the only one capable of obtaining a full recovery. In our opinion the most suitable endodontic procedure is that described by Schilder: with his technique infact a great deal of substantial results can be achieved. Bone rarefactions are to be surgically treated only when endodontic therapy does not guarantee the complete removal of microrganisms and that of the tissue de

Teaching Responsible Innovation in Endodontics: A Pedagogical Framework for Long-Term Clinical Success: A Narrative Review.

This review explores the responsible innovation concept in endodontics, emphasising its educational implications, biological accountability in clinical outcomes and a framework to guide everyday practice. Responsible innovation in endodontics must transcend mere technical advancement; it demands a comprehensive critical appraisal of practices, ethical awareness and a stewardship of long-term patient outcomes. Educational frameworks that prioritise reflective practice and encourage practitioners to continuously assess and refine their methodologies are essential. Additionally, a focus on biolog…

A Psychometric Comparison of Faculty-Authored and Large Language Model-Generated Multiple-Choice Questions in Endodontics.

Background and objective Designing high-quality multiple-choice questions (MCQs) in dental education is highly resource-intensive. While generative artificial intelligence (AI) offers a rapid, scalable solution for automated item generation, there is limited student-tested psychometric evidence comparing uncurated large language model (LLM) outputs with traditional faculty-authored items in advanced dental specialties like endodontics. This study aimed to evaluate and compare the psychometric properties, specifically the difficulty index, discrimination index, distractor effectiveness, and int…
