Histological effects of intratympanic gentamicin on the vestibular organ of guinea pigs.

BACKGROUND: Transtympanic administration of gentamicin may be suitable to achieve unilateral vestibular ablation, in order to control unilateral Ménière's disease. In low doses, gentamicin appears to affect selectively the vestibular system, with relative sparing of the cochlea. An experimental study on guinea pigs was conducted to determine what single dose of gentamicin would produce a unilateral vestibular organ lesion when applied to the middle ear. STUDY DESIGN: Experimental and prospective. METHODS: Four groups of guinea pigs received different gentamicin doses (1, 5, 10 and 25 mg) adm

Vestibular nerve section versus intratympanic gentamicin for Meniere's disease.

OBJECTIVES/HYPOTHESIS: Vestibular nerve section and transtympanic gentamicin administration are procedures with proven efficacy in the treatment of vertigo associated with Meniere's disease refractory to medical management. Hearing loss is a known complication of each of these procedures; however, there has not been a report of hearing results of both treatments from a single institution. STUDY DESIGN: Retrospective review. METHODS: Review was made of 25 patients undergoing gentamicin injection and 39 patients undergoing vestibular nerve section for Meniere's disease. Effects of gentamicin on sensorineural elements of the cochlea in human temporal bones.

PURPOSE: Gentamicin is commonly prescribed for treatment of gram-negative bacterial infections. It has, however, been known to have vestibulotoxic and ototoxic effects in humans. Previous studies were confounded by the use of other types of ototoxic drugs, higher than therapeutic doses, and prolonged time periods between gentamicin administration and death. The purpose of this study is to observe the effects of parenteral gentamicin administration on the sensorineural elements of the cochlea in which the previously described factors did not exist. MATERIALS AND METHODS: Temporal bone specimen

Continuous intratympanic infusion of gentamicin via a microcatheter in Menière's disease.

Between 1995 and 1998, 11 patients with disabling Menière's disease were treated at our institution with a continuous gentamicin infusion into the middle ear via a microcatheter. The patients had frequent attacks of vertigo and vomiting (functional levels 3-5). Hearing threshold on the affected side was significantly worse than on the healthy side (stage 4+5). Gentamicin was applied by a high-precision insulin pump with a flow rate of 40 mg per day directly in front of the round window. Application was stopped as soon as signs of vestibular affection appeared. A good overall control of vertigi

Long-term follow-up of transtympanic gentamicin for Ménière's syndrome.

OBJECTIVE: Recent studies have shown that transtympanic gentamicin for Ménière's syndrome is effective. Current treatment protocols vary. One concept has been to perform a chemical ablation; the other has been to perform a chemical alteration. Ablation requires multiple injections and is effective in controlling the vertigo, but it is associated with a significant incidence of hearing loss. Chemical alteration uses a minimal dose to reduce vestibular function without affecting cochlear function. STUDY DESIGN: Prospective. SETTING: Tertiary medical center. PATIENTS: Patients had classic unil

Gentamicin pharmacokinetics in preterm infants with a patent and a closed ductus arteriosus.

BACKGROUND AND AIM: A patent ductus arteriosus (PDA) may influence renal and hepatic blood flow and hence pharmacokinetics of drugs in neonates compared to neonates with a closed ductus arteriosus (CDA). A 10-percent difference of gentamicin pharmacokinetic parameters between PDA and CDA has been reported; but its implications are unclear. The relevance of this difference relative to the variability within the neonatal population was investigated. METHODS: Twenty-four neonates (12 with a PDA and 12 with a CDA) treated with gentamicin were retrospectively included. Before closing treatment of

Intratympanic gentamicin for treatment of intractable Meniere's disease: a preliminary report.

Topical administration of aminoglycoside antibiotics in the middle ear can achieve "chemical labyrinthectomy" in patients with intractable Meniere's disease. Herein we report our results of intratympanic gentamicin therapy in 21 patients using two different dosing protocols, twice weekly and twice daily (b.i.d.). Both hearing and vertigo outcome were evaluated. Complete control of episodic vertigo was achieved initially in 20 of 21 patients (95.2%). However, 6 of 20 responders (30%) developed relapsing symptoms within 12 months. Retreatment was successful in 75% of these patients. Overall, hea

Intraocular gentamicin sulfate and postcataract anterior chamber aspirate cultures.

We cultured anterior chamber aspirates of 28 patients who had uncomplicated cataract surgery. Fourteen patients had received 80 micrograms/ml of gentamicin in balanced salt solution through irrigation of the anterior chamber throughout cataract extraction, and 14 patients had received 8 micrograms/ml of gentamicin. Cultures of anterior chamber fluid obtained at the end of surgery from all 28 patients had no growth on two nonselective media. There was no clinical evidence of ocular toxicity. This study demonstrates that antibiotic in the intraocular irrigation fluid can reduce or eliminate the

Irritative spontaneous nystagmus following intratympanic gentamicin for Menière's disease.

Intratympanic aminoglycosides were first used to treat unilateral intractable Meniere's disease over 30 years ago. Gentamicin, the current drug of choice, provides excellent vertigo control with a low incidence of hearing loss. Its salutory effect results from direct damage to both the sensory neuroepithelium and the dark cells of the labyrinth. This latter effect may reverse the degree of endolymphatic hydrops. Twelve patients with unilateral Meniere's disease were treated and prospectively studied. Although further follow-up is necessary, 10 patients have complete vertigo control, while 2 re
