Vascularized Versus Non-vascularized Bone Grafting for Scaphoid Non-union: A Meta-Analysis.

Non-union after scaphoid fracture is a common complication. Controversy exists regarding the efficacy of vascularized bone graft (VBG) versus non-vascularized bone graft (NVBG) for scaphoid non-union. In the current meta-analysis, a comprehensive search of PubMed and the Cochrane Library database was done from inception to June 2024. We included randomized controlled trials (RCTs) and prospective comparative studies that reported outcomes following scaphoid non-union comparing VBG versus NVBG. Two reviewers independently extracted data and assessed the risk of bias. Any discrepancy was resolve

Surgical Technique: "Perforate and Fill" Technique of Bone Grafting for Scaphoid Fracture Fixation.

PURPOSE: We introduce the "Perforate and Fill" technique for bone grafting of scaphoid fracture with delayed union and nonunion, which preserves the cartilage shell and does not break open the fibrous nonunion. This article describes the technique and reports the experience in 11 scaphoid fractures. METHODS: The records of 11 patients whose scaphoid fractures were managed surgically with this bone grating technique from our institution from July 2017 to June 2024 were reviewed retrospectively. Patient and fracture factors, radiographic measurements of bone defect, postoperative films, and sub

Concentrated autologous bone marrow injection in the surgical treatment of scaphoid non-union.

New surgical techniques for the treatment of scaphoid non-union, developed in the last two decades, now enable a healing rate of 80-90%. However, no consensus exists for the surgical treatment of non-union. On the other hand, regenerative medicine techniques have enriched the therapeutic armamentarium for non-union, especially in the lower limbs, with the use of autologous concentrated bone marrow injection using autologous osteogenic precursors to create a favorable microenvironment for bone healing. This technique has the advantages of being inexpensive, easily accessible and applicable in s

Normal ranges for measurements of the scaphoid bone from sagittal computed tomography images.

This study aimed to determine normal values of three parameters commonly used to determine malunion by investigating intact scaphoids on sagittal computed tomography images from healthy individuals. We analysed 62 normal scaphoids and found the mean height-length ratio, lateral intrascaphoid angle and dorsal cortical angle to be 0.58, 27° and 128°, respectively. These measurements had good-to-excellent, poor-to-moderate and moderate-to-good inter- and intra-rater reliabilities, respectively. This study provides information on normal parameters of the scaphoid that may inform clinical decision

Time-dependent changes in bone healing capacity of scaphoid fractures and non-unions.

The scaphoid is the most frequently fractured carpal bone and prone to non-union due to mechanical and biological factors. Whereas the importance of stability is well documented, the evaluation of biological activity is mostly limited to the assessment of vascularity. The purpose of this study was to select histological and immunocytochemical parameters that could be used to assess healing potential after scaphoid fractures and to correlate these findings with time intervals after fracture for the three parts of the scaphoid (distal, gap and proximal). Samples were taken during operative inter

Interfragmentary Compression Forces Vary Based on Scaphoid Bone Screw Type and Fracture Location.

BACKGROUND: The objective of this study was to determine the interfragmentary compression forces generated in a foam model as a function of headless compression screw type (fully threaded and central threadless) and fracture location. METHODS: Eighty-eight polyurethane foam models were fixed across a simulated transverse fracture with either a fully threaded screw or a central threadless screw. The location of the transverse fracture varied along the length of the foam model in 2 mm increments for 11 fracture locations. The force generated at the fracture site upon fixation was utilized to de

Treatment of avascular proximal pole scaphoid nonunions with vascularized distal radius bone grafting.

PURPOSE: To investigate the outcomes of vascularized distal radius pedicled bone grafting secured with K-wires for scaphoid nonunions with small avascular proximal fragments. METHODS: We included patients with scaphoid nonunions and small, avascular proximal fragments that were too small for screw fixation. The mean size of the proximal pole fragments was 21% (range, 9% to 28%) of the entire scaphoid, based on quantitative radiographic measurement. All patients had distal radius bone grafting based on the 1,2-intercompartmental supraretinacular artery pedicle and fixation with K-wires. There

Clinically suspected scaphoid fracture: a comparison of magnetic resonance imaging and bone scintigraphy.

59 patients with clinical suspicion of scaphoid fracture, but negative radiographs at presentation and review, were examined by magnetic resonance imaging (MRI) and bone scintigraphy (BS). The diagnoses were compared with subsequent follow-up. There were four scaphoid fractures, 10 other fractures and three significant ligamentous injuries. All scaphoid fractures were identified by MRI and BS. MRI showed better interobserver agreement for scaphoid injury than BS and fewer false positive reports. Significant ligamentous injury and carpal instability diagnosed by MRI were not evident on scintigr

Wrist arthroscopy for the treatment of scaphoid delayed or nonunions and judging the need for bone grafting.

This study reports outcomes of arthroscopy in the treatment of delayed or nonunions of 25 scaphoids (25 patients). The surgery was performed between 8 and 43 weeks after injury. Intraoperatively, 11 fractures were deemed stable to probing and underwent percutaneous screw fixation only; 14 were unstable and received arthroscopic bone grafting with percutaneous screw fixation. All fractures united. At a mean follow-up of 21 months (range 12-48), the mean Mayo wrist score was 96, and patient-rated wrist evaluation was 4, and the flexion-extension arc was 90% of the contralateral wrist. We conclud

Bone graft in the treatment of nonunion of the scaphoid with necrosis of the proximal pole: a literature review.

Scaphoid fractures are the most common fractures of the carpal bones, corresponding to 60%. Of these, 10% progress to nonunion; moreover, 3% can present necrosis of the proximal pole. There are various methods of treatment using vascularized and non-vascularized bone grafts. To evaluate and compare the rate of scaphoid consolidation with necrosis of the proximal pole using different surgical techniques. The authors conducted a review of the literature using the following databases: PubMed and BIREME/LILACS, where 13 case series were selected (ten with use of vascularized bone grafts and three

Deceptive appearance of a normal variant of scaphoid bone in a teenage patient: a diagnostic challenge.

Scaphoid fractures are a common injury in late teens and mid twenties with a peak period in skeletally immature children at about 15 years of age, although considered to be rare in first decade of life, its exact incidence in early teen age remains to be a subject of debate. We report an unusual case of anatomical variation of scaphoid bone at the level of waist which could potentially cause diagnostic confusion. A 14-years-old boy presented in the fracture clinic 2 weeks after injury to his Right wrist which was managed in a scaphoid cast. X-ray examinations, both at the time of injury and la…

Scaphoid nonunions in children and adolescents: surgical treatment with bone grafting and internal fixation.

PURPOSE: Scaphoid nonunions are extremely uncommon injuries in children. On account of the rarity of the injury, there is no agreement about the standard method of treatment. The purpose of this study was to evaluate the clinical and radiologic outcomes after surgical treatment with bone grafting and internal fixation. METHODS: We retrospectively analyzed 23 patients aged 18 years or less with unstable scaphoid nonunions who underwent bone grafting and internal fixation. Mean age at the time of surgery was 15.1 years and the average duration of clinical follow-up was 5.2 years (range, 1-17 y)

Vascularised bone grafting in the management of scaphoid non-union - a review of 34 cases.

We reviewed the outcomes of 34 patients who had undergone vascularised bone grafting for a chronic scaphoid non-union. Mean age was 27 years (range 16-46 years). The dominant hand was involved in 17 cases. Eleven patients were smokers. In 18 cases the fracture involved the proximal and in 16 cases the middle third of the scaphoid. In 26 patients the proximal scaphoid fragment was deemed avascular. Sixteen patients had previously undergone scaphoid fixation and non-vascularised bone grafting. At a follow-up of 1 to 3 years (mean 1.6 years), 15 of the 34 scaphoid non-unions had united. Injury to

Vascularised bone graft from the base of the second metacarpal for refractory nonunion of the scaphoid.

A vascularised bone-graft procedure from the base of the second metacarpal was performed in 14 patients with nonunion of the scaphoid. There were 11 men and three women with a mean age of 22 years. In eight patients, who had dorsiflexed intercalated segment instability (DISI), an open wedge was formed at the site of nonunion, and the vascular pedicle was grafted from the volar side. In the six patients without DISI, transplantation was carried out through the same dorsal skin incision. Complete bony union was obtained in all patients after a mean post-operative period of 10.2 weeks, and DISI w

A comparison of bone scintigraphy and MRI in the early diagnosis of the occult scaphoid waist fracture.

OBJECTIVE: To determine the accuracy of MRI in the assessment of the radiographically occult scaphoid fracture. DESIGN: This prospective study compared the sensitivity and specificity of MRI for detection of radiographically occult scaphoid fractures with bone scintigraphy (BS), the currently accepted imaging modality of choice. Consecutive patients with clinical signs of a scaphoid injury but no evidence of fracture on plain radiographs at presentation and after 7-10 days were evaluated by MRI and BS. All images were reported in masked fashion and the sensitivities and specificities of the i
