Forty-four black children at two elementary schools within 0.7 mile of a battery plant had significantly higher (P < .001) concentrations of lead in their bloods (34.1 +/- 9.7, micrograms per 100 milliliters, mean +/- standard deviation) than 122 students (26.3 +/- 7.1) at seven schools 1 to 3 miles distant; 5 months later there was a comparable difference between red cell lead values (54.1 +/- 18.5 versus 37.4 +/- 12.6). BACKGROUND: Atrial lead perforation may lead to pneumopericardium or pneumothorax within a few days of device implantation.
METHODS AND RESULTS: We report a case of atrial lead perforation 6 years after cardiac resynchronization therapy implantation, which resulted in pneumopericardium and pneumothorax.
CONCLUSION: Although pneumopericardium caused by atrial lead perforation can spontaneously resolve with conservative treatment, as it did in this case, treatment should be decided based on the patient's general condition and lead performance.

We present a case of total occlusion of the superior vena cava (SVC) with extensive thrombosis of the adjacent large veins secondary to multiple abandoned pacemaker leads with a superimposed chronic lead infection by Corynebacterium jeikeium. A surgical lead extraction was performed with an extensive en-bloc resection of the SVC together with the right subclavian vein and the right innomate vein. No venous reconstruction was required because of an unobstructed runoff via a well-developed azygos system.
