Treatment of Herpes simplex encephalitis with the helicase-primase inhibitor pritelivir in an immunocompromised patient.

Pritelivir, a novel helicase-primase inhibitor, has successfully completed a global Phase 3 trial for refractory herpes simplex virus (HSV) infections in immunocompromised patients. This case report is the first clinical use of pritelivir in Herpes simplex encephalitis (HSE). An allogeneic hematopoietic cell transplant recipient developed HSE and was treated with acyclovir. After 1.5 months, while still on valacyclovir prophylaxis, HSE relapsed, confirmed by Magnetic Resonance Imaging (MRI) and HSV1 DNA detection in cerebrospinal fluid (CSF). Following transient response to acyclovir and fosca

[Human herpesvirus-6 pneumonitis following autologous peripheral blood stem cell transplantation].

A-46-year-old man was diagnosed with peripheral T cell lymphoma, not otherwise specified. He achieved a complete remission after pirarubicin, cyclophosphamide, vincristine, and prednisolone (THP-COP) therapy and successful autologous peripheral blood stem-cell transplantation (AutoSCT). However, 6 months post AutoSCT, he complained of fever. Chest computed tomography of the patient displayed bilateral interstitial pneumonitis. Human herpesvirus-6 (HHV-6) DNA was detected in his bronchoalveolar lavage fluid. Therefore, the patient was confirmed for HHV-6 pneumonitis. The treatment with foscarne

Phenotypic and genotypic characterization of acyclovir-resistant varicella-zoster viruses isolated from persons with AIDS.

Phenotypic and genotypic analyses were done on 17 varicella-zoster virus (VZV) isolates recovered from 10 persons with AIDS (mean CD4 cell count, 16.4/mm3) who had chronic VZV lesions. Eleven acyclovir-resistant isolates were recovered from 10 patients after a mean of 20.1 weeks of therapy. Six susceptible isolates were recovered before acyclovir treatment (n = 1), early during therapy (n = 4; mean time, 4.2 weeks), or after discontinuation of acyclovir (n = 1). Acyclovir-resistant VZV isolates were deficient in thymidine kinase (TK) or induced a TK with altered substrate specificity; all isol

Inhibition of Fosfomycin Resistance Protein FosB from Gram-Positive Pathogens by Phosphonoformate.

The Gram-positive pathogen Staphylococcus aureus is a leading cause of antimicrobial resistance related deaths worldwide. Like many pathogens with multidrug-resistant strains, S. aureus contains enzymes that confer resistance through antibiotic modification(s). One such enzyme present in S. aureus is FosB, a Mn2+-dependent l-cysteine or bacillithiol (BSH) transferase that inactivates the antibiotic fosfomycin. fosB gene knockout experiments show that the minimum inhibitory concentration (MIC) of fosfomycin is significantly reduced when the FosB enzyme is not present. This suggests that inhibit

Human Herpesvirus 6B in the Transplant Recipient: When to Worry, When to Act.

Human herpesvirus 6B (HHV-6B) is a ubiquitous pathogen that infects most individuals before the age of three years. HHV-6B reactivates in approximately 40% of transplant recipients where it has been associated with a number of important outcomes, especially in allogeneic transplant recipients. This article will review the epidemiology, clinical manifestations, diagnosis, and treatment of HHV-6B infection.

[Quantitative cytomegaloviral leukocytic antigenemia. Clinical aspects].

Cytomegalovirus infection, commonly observed in immunodepressed patients, raises a number of problems both in clinical practice (early rapid diagnosis, therapeutic options) and biological research (resistant strains, mutations). The classical methods of diagnosis do not provide specific information for decision making in each particular clinical situation. We report a case of cytomegalovirus infection in a renal transplant recipient in which quantitative cytomegalovirus antigen level in leukocytes provided rapid early diagnosis of the primary infection and two episodes of recurrent infection.

How we diagnose and manage refractory and resistant herpes simplex virus mucocutaneous infection after haematopoietic cell transplantation.

BACKGROUND: Herpes simplex virus (HSV) infection is a clinically significant complication in haematopoietic cell transplant (HCT) recipients. Refractory and resistant (R/R) HSV infections may occur in this patient population, particularly after prolonged exposure to anti-HSV agents. OBJECTIVES: This study aims to provide a comprehensive review of the diagnostic approach and the treatment options for R/R HSV mucocutaneous infections in HCT recipients and to highlight future treatment strategies. SOURCES: We searched the PubMed Central Database and Embase to identify published studies on R/R H

Cutaneous presentation of disseminated cytomegalovirus infection in a non-transplant patient with hematological malignancy: A case report.

RATIONALE: Cytomegalovirus (CMV) disease is relatively uncommon in nontransplant hematological patients. Moreover, cutaneous manifestations of CMV diseases have scarcely been reported and are probably under-recognized. PATIENT CONCERNS: We describe a patient with large B-cell lymphoma who developed a band-form, erythematous lesion over his left abdomen soon after the second course of rituximab, cyclophosphamide, doxorubicin, vincristine, prednisolone chemotherapy. DIAGNOSES: The lesion was initially mistaken for bacterial cellulitis or herpes zoster and was histologically confirmed as cutane

[Current treatment modalities of immunocompromised patients with cytomegalovirus infection. II. Therapeutic options and management strategies].

Although cytomegalovirus is one of the most prevalent viral pathogens on the globe, in immunocompetent individuals infected with cytomegalovirus usually no specific antiviral therapy is required. In the case of impaired T-cell mediated immunity, however, latent infection can reactivate and occasionally a viral disease with organ involvement develops. The number of actually available anti-cytomegalovirus drugs is low, for prophylaxis or treatment ganciclovir, valganciclovir, foscarnet or cidofovir can be administered. The clinical use of these drugs is primarily hampered by their toxicity. In s

Chronic verrucous varicella zoster virus skin lesions: clinical, histological, molecular and therapeutic aspects.

The outbreak of HIV infection introduced a new phenomenon in varicella zoster virus (VZV) pathology, namely the long-standing wart-like skin lesions that are frequently associated with resistance to thymidine kinase (TK)-dependent antiviral agents. This paper reviews the clinical, histological, and molecular aspects and the therapeutic management of these verrucous lesions. The majority of lesions are characterized by chronically evolving, unique or multiple wart-like cutaneous lesions. The main histopathological features include hyperkeratosis, verruciform acanthosis and VZV-induced cytopathi

Systemic antivirals in herpesvirus infections.

Over the last few years we have seen the advent of effective antiviral drug therapy, the major ramifications being in the area of herpes infections. Acyclovir has become an established part of clinical practice, and other compounds are being increasingly used in the immunocompromised host. An effective vaccine is available for chickenpox, but herpes simplex and cytomegalovirus vaccines are still in early development. Thus, antiviral agents will continue to play a major role in the management of all herpesvirus infections for the foreseeable future. Current work includes the development of incr

Disseminated Ganciclovir-Resistant Cytomegalovirus in a Nontransplant Pediatric ALL Patient: A Case Report.

BACKGROUND: Cytomegalovirus (CMV) infection is an underrecognized complication in children with acute lymphoblastic leukemia (ALL). While CMV disease is well documented in transplant recipients, its clinical course in nontransplant settings remains poorly defined. Disseminated ganciclovir-resistant CMV infection is a rare complication in nontransplant children with ALL, and its optimal management remains uncertain. OBSERVATIONS: We describe an 8-year-old patient with T-ALL receiving maintenance chemotherapy who developed high levels of CMV viremia without initial end-organ disease and subsequ

Identifying HHV-6 Induced Cytopathic Effects in Bronchoalveolar Lavage Fluid Preparations to Enhance Care in Immunocompromised Patients.

Human Herpesvirus 6 (HHV-6) is a ubiquitous virus that remains latent after primary infection but can reactivate in immunocompromised patients, leading to severe complications. Early detection of HHV-6 in respiratory samples is critical for timely intervention. We describe a 61-year-old immunocompromised woman who presented with pneumonitis and encephalopathy. Initial testing confirmed COVID-19, and worsening respiratory symptoms prompted bronchoalveolar lavage (BAL). Cytologic analysis of the BAL revealed epithelial cells with viral cytopathic effects, including multinucleation, chromatin mar

Detection of Pre-clinical Involvement of the Second Eye in Viral Acute Retinal Necrosis Using Optical Coherence Tomography.

PURPOSE: To report the detection of retinitis in the second eye of a patient with viral acute retinal necrosis (ARN), before the appearance of clinical change, using swept-source optical coherence tomography. RESULTS: A 63 year-old male developed right-sided varicella-zoster virus (VZV) ARN, confirmed with aqueous sampling. High-dose intravenous aciclovir caused renal impairment and was suspended for two-days. One day later, left eye macular SS-OCT revealed focal retinal thickening and disruption of retinal architecture without clinically detectable retinitis. The patient was asymptomatic. Aq

[Transplantation-associated thrombotic microangiopathy confirmed by renal biopsy].

An eight-year-old girl with myelodysplastic syndrome (refractory cytopenia) received a bone marrow transplant (BMT) from an unrelated donor because of immunosuppressive therapy failure. Following administration of foscarnet for cytomegalovirus reactivation at day40 post-BMT, serum creatinine increased, and proteinuria, hematuria, and hypertension gradually exacerbated and became prolonged. However, neither schistocytosis nor other organ damage was evident. At six months post-BMT, renal biopsy revealed diffuse glomerular damage with glomerular lobulation, a double contour of the glomerular base

A fatal case of cytomegalovirus ventriculoencephalitis in a mycosis fungoides patient who received multiple umbilical cord blood cell transplantations.

Cytomegalovirus (CMV) infection is latent in the majority of adult humans. The reactivation of CMV causes pneumonia and gastrointestinal disease in severely immunosuppressed patients, who consequently suffer very high mortality due to CMV central nervous system disease. We report here a case involving a 28-year-old female patient with mycosis fungoides who underwent umbilical cord blood transplantation three times and developed CMV ventriculoencephalitis. The patient's CMV viremia was successfully preempted with ganciclovir (GCV) as indicated by undetectable CMV antigenemia; despite this succe
