"ARVs is for HIV and cream is for HPV or precancer:" Women's Perceptions and Perceived Acceptability of Self-Administered Topical Therapies for Cervical Precancer Treatment: A Qualitative Study from Kenya.

BACKGROUND: Women in low- and middle-income countries (LMICs) bear a disproportionate burden of cervical cancer, despite being a preventable disease. Prevention efforts in LMICs are hindered in part by lack of access to cervical precancer treatment, due to weak health infrastructure and a lack of adequate human resources to deliver current provider-administered precancer treatments. Innovative strategies are urgently needed to close the cervical precancer treatment gap in LMICs, including the use of self-administered topical therapies such as 5-fluorouracil and imiquimod, for which efficacy ev

Topical erythropoietin in the management of scleral necrosis: a narrative review.

Scleral necrosis is a rare but severe complication caused by various etiologies. The main therapeutic approach is topical and systemic medical treatment. Surgical interventions may be indicated in unresponsive cases. These approaches, however, may fail to control the scleral necrosis. In addition, both medical and surgical treatment may lead to a number of ocular and systemic side effects, calling for noninvasive but effective treatment for the management of scleral necrosis. This review aims to summarize current studies investigating the role of topical erythropoietin in the treatment of scle

Evaluating Three Routes of Tranexamic Acid Administration in Total Hip and Knee Arthroplasty: A Nationwide Database Analysis in Taiwan.

BACKGROUND: Tranexamic acid (TXA) reduces perioperative blood loss, but comparative effectiveness and safety across administration routes remain uncertain. The authors evaluated topical, IV, and oral TXA in unilateral total hip and knee arthroplasty. METHODS: Using Taiwan's national health insurance database (2012 to 2021), the authors identified patients undergoing unilateral arthroplasty and evaluated perioperative TXA use, erythrocyte transfusion, and adverse events within 60 days after discharge. Conditional logistic regression and six propensity score-matched comparisons were conducted:

Topical treatment of Ginkgolide a ameliorates psoriasiform dermatitis by suppressing M1 macrophage polarization by the blockade of PI3K/AKT/c-JUN pathways.

Psoriasis (PsO) is a chronic, relapsing inflammatory skin disease. While targeted biologics have improved outcomes, limitations in long-term remission and safety concerns underscore the urgent need for cost-effective, safe, and potent topical therapies. Here, we investigated the therapeutic potential of Ginkgolide A (GA) for mild-to-moderate PsO. To maximize local efficacy and circumvent first-pass metabolism, we formulated a GA oily solution for topical administration. This administration of GA effectively alleviated IMQ-induced psoriasis-like dermatitis in mice. Furthermore, safety assessmen

Intradermal Injection of Tranexamic Acid for the Treatment of Adult Melasma: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Background: Melasma is a chronic skin pigmentation disorder, and intradermal injection of tranexamic acid (TXA) is an effective treatment option for melasma with limited comparative efficacy studies. Objectives: To compare the effectiveness of TXA injections with other treatment modalities for patients with melasma, as measured by Melasma Area and Severity Index (MASI). Methods: A total of 17 eligible randomized controlled trials were included in the meta-analysis. The MASI and the modified MASI served as the primary outcome measures of treatment effectiveness. Patient satisfaction was also ev

Mitomycin C therapy for corneal intraepithelial neoplasia.

We treated three patients who had intraepithelial neoplasia involving the visual axis and a subsequent decrease of visual acuity with topical mitomycin C 0.02% four times daily for ten to 22 days. In two of the three cases, the intraepithelial neoplasia was histologically confirmed. The intraepithelial neoplastic lesion was replaced by biomicroscopically normal epithelium within nine weeks of the beginning of drug administration and did not recur during the four to 12 months of follow-up. Adverse reactions to topical mitomycin in two patients ranged from minimal conjunctival hyperemia after 14

Trichomegaly induced by topical tacrolimus for the treatment of periorbital vitiligo: A brief report of a new adverse effect.

Trichomegaly is a known adverse effect of systemic administration of cyclosporine but is less commonly associated with systemic tacrolimus or with topical calcineruin inhibitors. In this report, we describe the first case, to our knowledge, of trichomegaly due to long-term use of topical tacrolimus for periocular vitiligo.

Osteonecrosis of the femoral head and glaucoma caused by topical corticosteroid application.

A 37-year-old man was diagnosed with bilateral osteonecrosis of the femoral head (ONFH) and glaucoma. He had no risk factors for either ONFH or glaucoma, except for 24 years' continuous use of topical corticosteroid ointments for treating atopic dermatitis. We determined that the topical steroid had caused both the pathological conditions. It is necessary to recognize that continuous topical corticosteroid therapy may cause the same pathological conditions as those caused by the systemic administration of these agents.

Cushing Syndrome Induced by Topical Corticosteroids for the Treatment of Lichen Sclerosus.

BACKGROUND: Lichen sclerosus is a chronic inflammatory dermatological condition with a predilection for the anogenital area. CASE: We describe a case of iatrogenic Cushing syndrome from the administration of high-potency topical steroids for vulvar lichen sclerosus in a 6-year-old girl. Her symptoms resolved after the cessation of topical steroid treatment. SUMMARY AND CONCLUSION: This case brings attention to iatrogenic Cushing syndrome as a potential complication when using high-potency topical corticosteroids in the anogenital region.

Topical minoxidil: review of efficacy and safety.

Topical minoxidil (Rogaine) has recently been approved by the Food and Drug Administration for treatment of androgenetic alopecia. It has been approved for such use in many other countries. This paper is a review and summary of the reported efficacy and safety of topical minoxidil in the treatment of androgenetic alopecia. The results of anecdotal and controlled clinical trials are included. Realistic appraisal of the restorative and/or preventative potentials of topical minoxidil in androgenetic alopecia is needed.

Topical application of ciclosporin ophthalmic solution containing alpha-cyclodextrin in experimental uveitis.

Ciclosporin (CS) has been shown to be a potent immunosuppressive drug. However, systemic administration of CS has been limited because of its unfavourable side effects. Thus, our attention has been focused on a topical application of CS for ophthalmic diseases. In this study, we examined the influence of a topical application of CS on experimentally induced ocular inflammation. CS was prepared by a new drug delivery system, with alpha-cyclodextrin (CD) as a solvent. Topical 0.075% CD-CS reduced ocular inflammation of rabbits induced by intravitreal injection of bovine serum albumin. The magnit

Ocular permeation and inhibition of retinal inflammation: an examination of data and expert opinion on the clinical utility of nepafenac.

BACKGROUND: The efficacy of topical nonsteroidal anti-inflammatory drugs (NSAIDs) for inflammation in the anterior segment, and pain control after cataract surgery, is well established. However, their effectiveness in the posterior segment has not been as well studied. Nepafenac ophthalmic suspension, 0.1% is a new topical NSAID pro-drug that has been approved by the US Food and Drug Administration (FDA) for the treatment of pain and inflammation after cataract surgery. Preclinical data suggest nepafenac may also provide unique efficacy in the posterior segment. SCOPE: We searched the PubMed

Early addition of topical corticosteroids in the treatment of bacterial keratitis.

IMPORTANCE: Scarring from bacterial keratitis remains a leading cause of visual loss. OBJECTIVE: To determine whether topical corticosteroids are beneficial as an adjunctive therapy for bacterial keratitis if given early in the course of infection. DESIGN, SETTING, AND PARTICIPANTS: The Steroids for Corneal Ulcers Trial (SCUT) was a randomized, double-masked, placebo-controlled trial that overall found no effect of adding topical corticosteroids to topical moxifloxacin hydrochloride in bacterial keratitis. Here, we assess the timing of administration of corticosteroids in a subgroup analysis

Successful topical treatment of oral lichen planus and leukoplakias with fenretinide (4-HPR).

Eight patients with diffuse (non-operable) pre-cancerous lesions (oral lichen or leukoplakias) were treated with fenretinidec (4-HPR) applied topically twice daily. After one month of therapy two patients had complete remission and the other six had a greater than 75% response. 4-HPR was well tolerated, and no local or distant side effects were observed. Topical treatment may be combined with oral administration at lower doses to obtain therapeutic results with less toxicity than observed with oral administration alone. A study to further evaluate the efficacy of topical treatment is in progre

A potential side effect of oral topical steroids: Central serous chorioretinopathy.

Topical corticosteroids are some of the most common drugs used in oral medicine for treating atrophic and erosive lesions that affect the mucosa. Adverse effects of these drugs include oral candidiasis with associated burning mouth and hypogeusia, hypersensitive reactions to the drug, and inhibition of the hypothalamic-pituitary-adrenal axis and secondary adrenal insufficiency. The ocular side effects of oral topical steroids are less documented. This short communication describes a case of central serous retinopathy that developed following administration of oral topical steroid.

Steroid-induced rosacea.

The excessive, regular use of topical fluorinated corticosteroids on the face often produces an array of skin complications, including an eruption clinically indistinguishable from rosacea ("iatrosacea"). Treatment involves discontinuation of the steroid and administration of oral tetracycline and nonsteroidal topical preparations. Once therapy is begun, clearing of the lesions may take several months.

Dual enkephalinase inhibitor PL265: a novel topical treatment to alleviate corneal pain and inflammation.

Ocular pain is a core symptom of inflammatory or traumatic disorders affecting the anterior segment. To date, the management of chronic ocular pain remains a therapeutic challenge in ophthalmology. The main endogenous opioids (enkephalins) play a key role in pain control but exhibit only transient analgesic effects due to their rapid degradation. The aim of this study was to explore the antinociceptive and anti-inflammatory effects of topical administration of PL265 (a dual enkephalinase inhibitor) on murine models of corneal pain. On healthy corneas, chronic PL265 topical administration did n
