Cervical laminoplasty is an established motion-preserving procedure for degenerative cervical myelopathy (DCM). However, patients with pre-existing cervical kyphosis often experience inferior outcomes compared to those with straight or lordotic spines. Limited dorsal spinal cord shift in kyphotic spines post-decompression and increased spinal cord tension may contribute to poor neurological recovery and spinal cord injury. This study aims to quantify the biomechanical impact of cervical sagittal alignment on spinal cord stress and strain post-laminoplasty using a validated 3D finite element mo

PURPOSE: Intervertebral disc degeneration (IVDD) is a major cause of chronic spinal pain and increases with age. Although spinal deformities contribute to asymmetric IVDD, vascular asymmetry within the intervertebral disc (IVD) remains unclear. This study investigated the relationship between differences in vascularity between the concave and convex sides of the IVD and the coronal curvature angle.
METHODS: The study comprised two parts: Study A and Study B. Study A included 15 IVDs (C7-T1, T1-T2, and T7-T8; five discs per level) obtained from five cadavers (two females). Study B included 53

Background: A precise radiographic evaluation of adolescent idiopathic scoliosis (AIS) is essential for effective treatment planning and follow-up. The pelvic axial rotation (PAR) and horizontal balance of the pelvis are critical factors to consider throughout the treatment and monitoring of AIS. While some previous studies have examined spinal curvature in relation to PAR direction and the direction of the major curve (DMC) in AIS patients, this study aims to explore the relationship between scoliosis morphology, pelvic axial rotation (PAR), and the direction of the major curve in patients wi

Lumbar disc degeneration is a major contributor to low back pain and disability. Lumbar total disc replacement (LTDR) has been introduced as a motion-preserving alternative to spinal fusion, and its biomechanical performance is strongly influenced by prosthesis geometry. This study investigated the effects of two key geometric parameters, curvature radius and radial clearance, on the biomechanical behavior of a ball-and-socket LTDR. Three-dimensional finite element models of the L3-L5 lumbar spine were developed, incorporating prostheses with curvature radii ranging from 7 to 10 mm and radial

STUDY DESIGN: Retrospective comparative cohort.
OBJECTIVE: The aim of this study was to determine whether spinal growth is restricted by brace treatment in patients with juvenile idiopathic scoliosis (JIS).
SUMMARY OF BACKGROUND DATA: Spinal fusion can negatively affect spinal growth if performed before the growth spurt. Brace treatment is often given in this young population to control the spinal deformity while allowing spinal growth. It is unknown whether the applied pressure of brace treatment on spine results in growth restriction. The aim of the study is to evaluate spinal growth in br

BACKGROUND: We hypothesized that axial-loaded magnetic resonance imaging (MRI), modified with the use of a cushion placed behind the lower back (i.e. BS-MRI method), would simulate the standing position more accurately than an axial-loaded MRI without a cushion back support (BS).
PURPOSE: To determine whether the BS-MRI method demonstrated similar morphologies on intervertebral disc (IVD), dural sac, and spinal curvature as those detected on 90° standing MRIs in individuals with suspected spinal stenosis.
MATERIAL AND METHODS: Twenty-five subjects underwent a BS-MRI, as well as axial-loaded

A common problem in long spinal fusions is caudal end fixation failure. To address sacral screw loosening, fixation may be extended to the pelvis using iliac screws. Iliac screws provide strong anchor strength, but loosening is common due to high strain. A novel 'Eiffel Tower' (E-T) construct is introduced to reduce iliac screw strain. A saw-bone spine model was instrumented with pedicle screws and rods from T9 to S1 as native fixation. L5 and S1 screw strain was measured with application of 25Nm loading in flexion, extension, lateral bending and rotation. Strain was measured for L5, S1 and il

Multiple System Atrophy (MSA) is a progressive neurodegenerative disorder characterized by autonomic dysfunction, parkinsonism, and cerebellar signs. Among its motor manifestations, camptocormia-defined as an abnormal forward flexion of the trunk-often emerges early in the disease course and significantly compromises patients' quality of life. Aside from physical therapy, the treatment of camptocormia remains particularly challenging, especially in MSA, where surgical approaches such as deep brain stimulation and spinal cord stimulation are generally contraindicated. This underscores the urgen

OBJECTIVE: Pelvic incidence (PI) is traditionally considered a constant anatomic parameter in adult spinal deformity (ASD) surgery. However, emerging evidence suggests that PI may change postoperatively, potentially influencing sagittal balance and mechanical complications. This study aims to systematically review the literature on postoperative PI variations in ASD patients, identifying potential radiologic and surgical predictors.
METHODS: Papers describing postoperative PI changes in ASD patients were included in the analysis. The rate of PI changes, in different subgroups of patients was

PURPOSE: This article aims to provide an overview of how spinal deformities can alter normal spine and thoracic cage growth.
METHODS: Some of the data presented in this article are gathered from studies performed in 1980 and 1990, and their applicability to populations of different ethnicity, geography or developmental stage has not yet been elucidated. In the present article, older concepts have been integrated with newer scientific data available to give the reader the basis for a better understanding of both normal and abnormal spine and thoracic cage growth.
RESULTS: A thorough analysis

We present a new analysis tool for cervical flexion-extension radiographs based on machine vision and computerized image processing. The method is based on semiautomatic image segmentation leading to detection of common landmarks such as the spinolaminar (SL) line or contour lines of the implanted anterior cervical plates. The technique allows for visualization of the local curvature of these landmarks during flexion-extension experiments. In addition to changes in the curvature of the SL line, it has been found that the cervical plates also deform during flexion-extension examination. While e

Early-onset scoliosis (EOS) is defined as any spinal deformity that is present before 10 years old, regardless of etiology. Deformity must be evaluated based on the intercorrelation between the lungs, spine, and thorax. Curvatures of early-onset have increased risk of progression, cardiorespiratory problems, and increased morbidity and mortality. Progression of the deformity may produce thoracic insufficiency syndrome, where a distorted thorax is unable to support normal respiratory function or lung growth. Management and treatment of EOS should pursue a holistic approach in which the psycholo

The natural history, associated anomalies, and response to operative and nonoperative treatment were reviewed in 102 patients with neurofibromatosis and spine deformity. Eighty patients were found to have curvatures associated with dystrophic changes in the vertebrae and ribs. The presence of dystrophic changes such as rib penciling, spindling of the transverse processes, vertebral scalloping, severe apical vertebral rotation, foraminal enlargement, and adjacent soft-tissue neurofibromas was found to be highly significant in prognosis and management. Brace treatment of dystrophic curves was un

This chapter reviews the biochemical, hormonal, and hematological factors in the onset and development of adolescent idiopathic scoliosis (AIS), an orthopedic entity of unknown etiology. Briefly, AIS is defined as a lateral curvature of the spine combined with vertebral rotation that occurs in patients of 10 years of age or older until bone maturity (18-20 years of age). AIS is predominant in females. If untreated, the curvature could evolve with negative long-term prognosis including psychosocial impact, back pain, pulmonary compromise, cor pulmonale, and even death due to respiratory failure

In recent years there has been spectacular progress in the approach to various disorders of the spinal column. Owing to improved methods of osteosynthesis there is no longer so much need for long periods of postoperative bed rest. Of all the scolioses, idiopathic scoliosis is most common. The vast majority of these cases are not clinically significant. What is seen in the remaining cases if left untreated is a progression in the curvature during growth. Progressive idiopathic scoliosis can be effectively treated using conservative methods. Screening at school is an important part of this proce

Spinal muscular atrophy (SMA) indicates a set of inherited autosomal recessive genetic disorders, where, specifically, the anterior horn cell motor neurons in the brain and spinal cord are affected, leading to a severe form of hypotonia and muscle weakness. The incidence is exceptionally rare, commonly manifesting as slowly progressive muscular weakness and atrophy of lower limbs. As per our existing knowledge, this is the first case of SMA associated with hyperlordosis in a patient. Hyperlordosis is a deformity in spinal curvature characterized by an excessive forward spinal curve in the regi

Adult spinal deformity is a multifaceted condition increasingly encountered in aging populations, often complicated by coexisting hip and knee osteoarthritis. It is important to present an integrated review of biomechanical, surgical, and technologic advances shaping modern adult spinal deformity management. First, the hip-spine-lower extremity relationship and how lower limb pathology impairs compensatory mechanisms, alters spinopelvic alignment, and influences surgical outcomes should be examined. Next, the evolution of classification systems and alignment philosophies-ranging from the Scoli
