Postoperative follow-up of fibromuscular subaortic stenosis.

OBJECTIVES: This study attempted to determine whether early subaortic resection at lower levels of obstruction reduces the rate of recurrence of subaortic stenosis or reduces secondary damage to the aortic valve, or both. BACKGROUND: Fibromuscular subaortic stenosis is a progressive condition, and at present it is unclear whether early operation reduces the recurrence rate along with decreasing the incidence of aortic insufficiency. METHODS: Thirty-seven patients with fibromuscular subaortic stenosis and no other significant cardiac abnormality who underwent open subaortic resection were eva

Results of total correction of the tetralogy of Fallot. Long-term haemodynamic evaluation at rest and during exercise.

Postoperative haemodynamic evaluation was performed on 15 patients 7 to 14 years (mean, 11 years) after total intracardiac repair of Fallot's tetralogy. The average age of the patients at the time of postoperative catheterization was 20 years (range 12 to 29) and they were all asymptomatic. Before operation 11 patients were severely cyanosed and disabled and 4 had mild cyanosis with anoxic spells. The cardiac output and right heart pressures were measured at rest and after 5 minutes of submaximal exercise on a bicycle ergometer. It was shown that the degree of residual obstruction of the pulmo

Echocardiographic predictors of reoperation for subaortic stenosis in children and adults.

OBJECTIVES: Subaortic stenosis (SAS) can present as various types of obstruction of the left ventricular outflow tract (LVOT) below the level of the aortic valve. Even though corrective surgery has been identified as the most effective treatment, SAS more frequently reoccurs requiring reoperation in a significant proportion of the patients. Previous studies have focused on predictors of recurrence in various subgroups of patients with SAS, but rarely in the overall population of patients with SAS. The aim of this study was to determine the predictors of recurrence of SAS after initial correcti

Prenatal diagnosis and postnatal outcomes of foetal situs abnormalities: a single tertiary centre experience.

BACKGROUND: Foetal situs abnormalities, including situs inversus totalis (SIT), right atrial isomerism (RAI) and left atrial isomerism (LAI), are associated with substantial variation in cardiac anatomy and postnatal outcomes. This study aimed to compare prenatal characteristics, associated abnormalities, perinatal outcomes, postnatal management and survival among foetuses with SIT, RAI and LAI. METHODS: This retrospective cohort study included 73 foetuses prenatally diagnosed with SIT (n = 12), RAI (n = 24) or LAI (n = 37) at a tertiary foetal cardiology and perinatology centre between 2021

Using preoperative cardiac computed tomographic conduction axis prediction to avoid damage in congenital left ventricular outflow tract and aortic valve surgery.

OBJECTIVE: Postoperative high-grade atrioventricular block is prevalent following congenital aortic valve, root and left ventricular outflow tract (LVOT) surgery. We aimed to evaluate the application of presurgical cardiac computed tomography (CT) estimation of the conduction axis in mitigating this issue. METHODS: Patients with congenital aortic valve, root and LVOT disease evaluated and operated in our center from February 2022 to August 2025 who underwent presurgical CT with intact central fibrous body were included. Anatomical landmarks were used to estimate the atrioventricular node (poi

Fontan pump: Advanced preclinical development.

OBJECTIVE: Late Fontan failure and attrition is an increasingly prevalent problem for which there is no primary therapy. A Fontan circulation is a circulation that lacks a subpulmonary pump. We are developing a technology that can safely and reliably restore subpulmonary pump function in a Fontan circulation to reverse the Fontan paradox and maintain normal circulatory health. METHODS: A rotary pump scaled for adolescent and adult use was developed for implantation to physiologically augment flow in the total cavopulmonary connection. Design objectives included 30/70% superior vena cava/infer

Clarifying the cardiovascular morphology and associated abnormalities in patients with tricuspid Atresia using multidetector computed tomography angiography.

The present study sought to assess the intracardiac morphology and associated cardiovascular anomalies in patients with tricuspid atresia using multidetector CT angiography. CT angiography datasets of all patients diagnosed with tricuspid atresia at our institution between November 2014 and December 2021 were retrospectively reviewed. The type of tricuspid atresia and associated cardiovascular structural anomalies detected on CT angiography were evaluated. Tricuspid atresia was identified in 404 patients (276 males [68.3%]; mean age at diagnosis: 4.54 ± 6.61 years [range: 9 months-40 years]).

Spinal Ultrasound Assessment of Correlation Between Intraventricular Hemorrhage Severity and Cerebrospinal Fluid Volume in Preterm Infants.

OBJECTIVES: Intraventricular hemorrhage (IVH) affects >15% of preterm infants. Severe IVH disrupts cerebrospinal fluid (CSF) flow dynamics, causing post-hemorrhagic ventricular dilation (PHVD) and further brain injury. Although lumbar puncture (LP) may reduce CSF volume and intracranial pressure, its effectiveness depends on brain-to-spine CSF flow dynamics and spinal CSF volume, which remain underexplored. This study correlates IVH severity with spinal CSF volume of preterm infants using spinal ultrasound (SUS). METHODS: This prospective study enrolled infants (<33 weeks gestational age [GA]

Noninvasive Assessment of Right Ventricular-Pulmonary Arterial Coupling in Repaired Tetralogy of Fallot by Magnetic Resonance Imaging.

BACKGROUND: Right ventricular (RV) to pulmonary arterial coupling (RVPAc) quantifies RV contractility in relation to its afterload but traditionally requires high-fidelity catheter derived pressure-volume loops. We sought to evaluate a noninvasive volume-based RVPAc parameter in children and adults with repaired tetralogy of Fallot in relation to pulmonary regurgitation (PR), RV outflow tract obstruction (RVOTO), and exercise capacity. METHODS: We retrospectively studied 92 pediatric and 105 adult patients with repaired tetralogy of Fallot who had cardiovascular magnetic resonance imaging and

Use of balloon-expandable valves for the surgical treatment of mitral valve disease in the setting of severe mitral annular calcification.

OBJECTIVE: The treatment of mitral valve disease complicated by severe mitral annular calcification poses a significant challenge, with increased risk of morbidity and mortality. We describe the outcomes after surgical mitral valve replacement in patients with severe mitral annular calcification using a balloon-expandable valve. METHODS: From 2015 to 2023, 82 consecutive patients received mitral annular calcification using a balloon-expandable valve. No patients were excluded from analysis. Patient records were retrospectively reviewed for baseline characteristics, echocardiography results, i

Accuracy of Three-Dimensional Neo Left Ventricular Outflow Tract Simulations With Transcatheter Mitral Valve Replacement in Different Mitral Phenotypes.

BACKGROUND: Transcatheter mitral valve replacement (TMVR) is emerging in the context of annular calcification (valve-in-MAC; ViMAC), failing surgical mitral annuloplasty (mitral-valve-in-ring; MViR) and failing mitral bioprosthesis (mitral-valve-in-valve; MViV). A notorious risk of TMVR is neo left ventricular outflow tract (neo-LVOT) obstruction. Three-dimensional computational models (3DCM) are derived from multi-slice computed tomography (MSCT) and aim to predict neo-LVOT area after TMVR. Little is known about the accuracy of these neo-LVOT predictions for various mitral phenotypes. METHOD

Burden of reintervention after tetralogy of Fallot repair: A joint pediatric and adult congenital experience over 30 years.

OBJECTIVE: There is a high burden of reintervention after repair of tetralogy of Fallot (TOF). We compare procedural burden and late outcomes in valve-sparing repair (VSR) and transannular patch (TAP) cohorts over 30 years. METHODS: Patients undergoing TOF repair (1990-2021, excluding complex TOF) were included in this study (n = 1239) with subsequent comparisons between TAP (n = 550) and VSR (n = 648) cohorts. Descriptive statistics, cumulative incidence frequencies, survival analysis, and propensity matching (n = 425) were used to analyze reintervention burden and survival. RESULTS: Overal

Residual pulmonary stenosis and right ventricular contractility in repaired tetralogy of Fallot.

OBJECTIVES: The impact of residual pulmonary stenosis (rPS) or right ventricular (RV) outflow tract obstruction on prognosis after surgical pulmonary valve insertion) in repaired tetralogy of Fallot patients with pulmonary regurgitation (PR) remains controversial. rPS assessment is partially dependent on RV contractility. We investigated the impact of rPS according to RV ejection fraction (RVEF). METHODS: In this multicentre, retrospective study, 117 repaired tetralogy of Fallot patients who underwent surgical pulmonary valve insertion for more than moderate PR between 2003 and 2021 were exam

Transcatheter Mitral Valve Replacement With the Transseptal EVOQUE System.

OBJECTIVES: The aim of this study was to examine the initial experience with a novel transseptal transcatheter mitral valve replacement (TMVR) system. BACKGROUND: Transseptal TMVR may offer a less invasive option than surgery for mitral regurgitation (MR) with greater efficacy and fewer anatomic limitations than transcatheter repair. METHODS: Patients were treated with the EVOQUE TMVR system from September 2018 to October 2019. Key inclusion criteria were moderate or greater MR, New York Heart Association functional class ≥II, and high or prohibitive surgical risk. The primary outcome was te

Patient-Specific Approach to Mitral Valve Replacement in Infants Weighing 10 kilograms or less.

BACKGROUND: Mitral valve replacement (MVR) is the only option for infants with severe mitral valve disease that is not reparable; however, previously reported outcomes are not always favorable. Our institution has followed a tailored approach to sizing and positioning of mechanical valve prostheses in infants requiring MVR in order to obtain optimal outcomes. METHODS: Outcomes for 22 infants ≤10 kg who have undergone MVR in Sydney, Australia, from 1998 to 2016, were analyzed. Patients were at a mean age of 6.8 ± 4.1 months (range: 0.8-13.2 months) and a mean weight of 5.4 ± 1.8 kg at the time

Identifying the Transcriptome Signatures of Calcium Channel Blockers in Human Induced Pluripotent Stem Cell-Derived Cardiomyocytes.

RATIONALE: Calcium channel blockers (CCBs) are an important class of drugs in managing cardiovascular diseases. Patients usually rely on these medications for the remainder of their lives after diagnosis. Although the acute pharmacological actions of CCBs in the hearts are well-defined, little is known about the drug-specific effects on human cardiomyocyte transcriptomes and physiological alterations after long-term exposure. OBJECTIVE: This study aimed to simulate chronic CCB treatment and to examine both the functional and transcriptomic changes in human cardiomyocytes. METHODS AND RESULTS

Causes and Clinical Outcomes of Patients Who Are Ineligible for Transcatheter Mitral Valve Replacement.

OBJECTIVES: The aim of this study was to gain insight into the causes and outcomes of patients who do not qualify for transcatheter mitral valve replacement (TMVR). BACKGROUND: Despite the increasing availability of TMVR, patients with severe mitral regurgitation may not be eligible. Thus far, no investigation has examined ineligible patients and their clinical outcomes. METHODS: A total of 203 patients (mean age 79 ± 9 years, 48% men) who were ineligible for participation in early feasibility studies of TMVR were examined. RESULTS: The ineligibility rate for TMVR was 89.0%. The most common

Open Atrial Transcatheter Mitral Valve Replacement in Patients With Mitral Annular Calcification.

BACKGROUND: Mitral valve replacement in the setting of severe mitral annular calcification remains a surgical challenge. Transcatheter mitral valve replacement (TMVR) using an aortic balloon-expandable transcatheter heart valve is emerging as a potential treatment option for high surgical risk patients. Transseptal, transapical, or transatrial access is not always feasible, so an understanding of alternative implantation techniques is important. OBJECTIVES: The authors sought to present a step-by-step description of a contemporary transatrial TMVR technique using balloon-expandable aortic tra
