Minimally invasive transforaminal lumbar interbody fusion using bone cement-augmented pedicle screws for lumbar spondylolisthesis in patients with osteoporosis. Case series and review of literature.

BACKGROUND: Instrumentation in patients with osteoporosis is challenging. Bone cement-augmented fenestrated pedicle screw fixation is a new procedure for fixation in the osteoporotic bone; and, applying minimally invasive techniques to the above is a challenging and novel concept. AIMS: To evaluate the clinical and radiological outcome of minimally invasive spine surgery transforaminal lumbar interbody fusion (MIS-TLIF) in patients with spondylolisthesis and poor bone quality, performed with rigid instrumentation using bone cement [poly(methylmethacrylate)]-augmented fenestrated pedicle screw

Anatomical Feasibility and Morphometric Analysis of Cortical Bone Trajectory Screw Fixation in the Pediatric Lumbar Spine Using Computed Tomography.

AIM: The cortical bone trajectory (CBT) screw fixation technique has been widely adopted in adult spinal surgery due to its minimally invasive nature and biomechanical advantages. However, systematic research on its anatomical feasibility and morphometric parameters in the pediatric lumbar spine remains lacking. This study aimed to evaluate the anatomical feasibility of CBT screw fixation in the pediatric lumbar spine, and to characterize the variations in screw dimensions and insertion angles across different age groups and lumbar levels, providing an individualized reference for pediatric lu

The MRI-based vertebral bone quality score is a predictor of pedicle screw loosening following instrumented posterior lumbar fusion.

OBJECTIVE: To investigate the predictive ability of the MRI-based vertebral bone quality (VBQ) score for pedicle screw loosening following instrumented transforaminal lumbar interbody fusion (TLIF). METHODS: Data from patients who have received one or two-level instrumented TLIF from February 2014 to March 2015 were retrospectively collected. Pedicle screw loosening was diagnosed when the radiolucent zone around the screw exceeded 1 mm in plain radiographs. The T1-weighted MRI sagittal images were used for calculation of the VBQ score. The score was calculated with the median signal intensity

Value of different preoperative bone evaluation methods in predicting intraoperative screw insertion torque: a prospective clinical comparative trial.

BACKGROUND CONTEXT: Establishing good screw-bone structural stability is conducive to reducing the risk of postoperative screw loosening. Screw insertion torque is an objective index for evaluating screw-bone structural stability. Therefore, accurate prediction of screw insertion torque can improve the preoperative evaluation of patients, optimize the surgical plan, and improve the surgical effect. At present, the correlation between different bone assessment methods and screw insertion torque is unclear. PURPOSE: The aim of this study was to evaluate the correlation between different bone as

Real-world clinical accuracy of long cortical bone trajectory screw placement using a patient-specific template guide.

BACKGROUND: Cortical bone trajectory (CBT) screws can be very reliable anchors if inserted precisely anterior to the vertebral body; however, their trajectory is narrow, and malpositioning of the screw is not rare, especially for surgeons who are not familiar with the CBT screw. Patient-specific template guides are a solution to this problem; however, their accuracy and usefulness in clinical settings remain unclear. The aim of the present study was to evaluate the accuracy of long CBT placement using a patient-specific screw-guide system. METHODS: This research involved a retrospective clini

Novel press-fit technique of patellar bone plug in anterior cruciate ligament reconstruction is comparable to interference screw fixation.

PURPOSE: Conventional press-fit technique for anterior cruciate ligament reconstruction (ACLR) is performed with extraction drilling of the femoral bone tunnel and manual shaping of the patellar bone plug. However, the disadvantages of this technique include variation in bone plug size and, thus, the strength of the press-fit fixation, bone loss with debris distribution within the knee joint, potential heat necrosis, and metal wear debris due to abrasion of the guide wire. To overcome these disadvantages, a novel technique involving punching of the femoral bone tunnel and standardized compress

Three-Dimensional Printed Porous Titanium Screw with Bioactive Surface Modification for Bone-Tendon Healing: A Rabbit Animal Model.

The interference screw fixation method is used to secure a graft in the tibial tunnel during anterior cruciate ligament reconstruction surgery. However, several complications have been reported, such as biodegradable screw breakage, inflammatory or foreign body reaction, tunnel enlargement, and delayed graft healing. Using additive manufacturing (AM) technology, we developed a titanium alloy (Ti6Al4V) interference screw with chemically calcium phosphate surface modification technology to improve bone integration in the tibial tunnel. After chemical and heat treatment, the titanium screw formed

Central screw use delays implant dislodgement in osteopenic bone but not synthetic surrogates: A comparison of reverse total shoulder models.

Adequate glenoid baseplate fixation in reverse total shoulder arthroplasty (rTSA) is important to achieve, but may prove challenging in the context of glenoid bone loss or osteopenia. Current rTSA testing standards rely upon synthetic bone surrogates, but it is unclear if these models accurately recapitulate the mechanics of osteoporotic bone. Additionally, it also unknown if the use of a central screw effectively provides resistance to micromotion in the milieu of poor quality bone. The purpose of this experiment was to create a novel cyclic load test protocol that elicited clinically relevan

Radiological Evaluation of the Initial Fixation between Cortical Bone Trajectory and Conventional Pedicle Screw Technique for Lumbar Degenerative Spondylolisthesis.

STUDY DESIGN: Retrospective study. PURPOSE: To compare initial fixation using the cortical bone trajectory (CBT) technique versus conventional pedicle screws (PS) in radiographs of postsurgical lumbar degenerative spondylolisthesis. OVERVIEW OF LITERATURE: Few reports have documented the holding strength of CBT technique for spondylolisthesis cases. METHODS: From October 2009 to June 2014, 21 cases of degenerative spondylolisthesis were surgically treated in our institution. Ten were treated with conventional PS technique and 11 of with CBT technique. Mean lumbar lordosis and percent slippa

Titanium integration with bone, welding, and screw head destruction complicating hardware removal of the distal radius: report of 2 cases.

Increasingly, surgeons treat distal radius fractures with locking plate systems. Recent case reports have focused on technical insertion errors resulting in removal difficulties: poor drilling orientation or cross-threading, destruction of the screw head, and filling of the screw recess with tissue. We report 2 complications of titanium locked plate removal secondary to in vivo reactions including titanium integration with bone and mechanical binding between the titanium screw and plate. We clarify and discuss terminology relevant to implant removal, including cold-welding, galling, fretting,

[Lengemann suture versus bone screw osteosynthesis in treatment of ulnar osseous collateral ligament rupture of the thumb metacarpophalangeal joint].

Out of 95 ligamentous lesions of the thumb metacarpophalangeal joint, those with an isolated bony rupture of the ulnar collateral ligament were chosen. Of these, 15 patients treated with Lengemann suture and 16 with screw fixation were followed-up on average 27.3 and 26.4 months, respectively, after operative treatment. Under only "good" and "very good" clinical results, the statistical analysis of the objective and subjective parameters showed no significant difference, except for the measurement of the soft-tissue circumference (p < 0.01) between either procedure. Although both procedures ha

Quantifying digital volume correlation strain uncertainty at the bone-pedicle screw interface: an ex-vivo zero-strain analysis.

Pedicle screws are essential for postoperative construct stability in spinal fusion surgeries. However, screw loosening remains a common clinical complication, often linked to excessive loading that induces damaging trabecular bone strains surrounding the pedicle screw. Toward improving pre-clinical assessment of pedicle screw fixation, this study combines micro-CT imaging, U-Net deep learning segmentation, and digital volume correlation (DVC) techniques to quantify the errors associated with measuring full-field strains at the screw-bone interface under nominal zero-strain conditions. Pedicle

A 3D-printed hybrid vertebroplasty, kyphoplasty and screw augmentation teaching model simulating high and low bone mineral densities.

INTRODUCTION: Vertebral fractures are highly prevalent, particularly among the elderly and patients with osteoporosis. Current surgical training for vertebral augmentation procedures is largely confined to the operating room, where opportunities are inherently limited in both duration and scope. RESEARCH QUESTION: To develop a realistic, accessible, and cost-efficient hybrid training model for fluoroscopy-guided vertebroplasty, kyphoplasty, and pedicle screw augmentation, replicating both high- and low-density vertebral bone. MATERIAL AND METHODS: Additive manufacturing techniques were appli

Distinct regional bone quality patterns in L5-S1 isthmic and degenerative spondylolisthesis: implications for pedicle screw loosening risk.

BACKGROUND: Screw loosening remains a major concern after pedicle screw fixation for lumbar spondylolisthesis. Although dual-energy X-ray absorptiometry (DXA) is widely used to assess systemic bone status, it may not fully capture regional bone quality at the pedicle, which is directly involved in screw purchase. This study aimed to compare DXA-based systemic bone status and CT-derived regional Hounsfield unit (HU) values among patients with single-level L5-S1 isthmic spondylolisthesis (IS), patients with degenerative spondylolisthesis (DS), and normal controls, and to evaluate their associati

Biomechanical impact of cortical bone vs. traditional pedicle screw trajectories: a finite element study on lumbar spinal instrumentation.

BACKGROUND: Pedicle screw fixation using the cortical bone trajectory (CBT) enhances stability by engaging cortical bone, offering a valuable alternative to the traditional pedicle screw trajectory (TT). This study used finite element analysis to compare L4-5 instrumentation with CBT and TT screws, investigating whether the increased cortical bone engagement in CBT improves stability but makes it more susceptible to fatigue failure. METHODS: A L3-sacrum model was generated using anonymized CT patient data, validated against existing studies, showing consistent ROM (range of motion) values. A

The impact of bone graft volume on risk of pseudarthrosis, screw loosening, and rod breakage in surgical correction of adult spinal deformity.

BACKGROUND: Allograft bone and demineralized bone matrix (DBM) are important allogeneic sources of structural and biologic graft to support successful fusion in surgical correction of adult spinal deformity (ASD). Little is known about the contribution of graft type, volume, and ratio to successful fusion. METHODS: A retrospective review of a single-institution database of ASD patients was reviewed to assess for postoperative pseudarthrosis, screw loosening, or rod breakage (PLB). Patients with ≥ 5 level fusion who had follow-up > 6 months were included in the analysis. Surgical records were
