Four patients with coronary artery disease and chronic marked left axis deviation, defined as a frontal QRS axis more negative than -45 degrees, were studied with epicardial mapping during coronary bypass surgery. All patients had normal right ventricular and inferior left ventricular epicardial breakthrough sites and activation sequence. Normal breakthrough in the basal anterolateral left ventricular epicardium was absent in all four patients. Two patients had breakthrough in the apical region of the anterolateral left ventricle. In the other two this region was activated from wave fronts eme…

BACKGROUND: Stress perfusion cardiovascular magnetic resonance (CMR) in the presence of atrial fibrillation (AF) has long been challenging due to electrocardiogram (ECG) mis-triggering. However, non-invasive ischemia imaging is important due to an increased risk of myocardial infarction in patients with AF, which has been attributed to underlying microvascular dysfunction. Myocardial blood flow (MBF) in patients with AF is poorly understood, and few studies have attempted to quantify this through non-invasive imaging.
METHODS: Patients were recruited for stress perfusion CMR using a research

BACKGROUND: Atypical atrial flutter (AFL) is common in patients with postsurgical atrial scar, with macro- or microscopic channels in the scar acting as substrate for reentry. Heterogeneous atrial scarring can cause varying flutter circuits, which makes mapping and ablation challenging, and recurrences common.
AIM: We hypothesize that dynamically adjusting voltage thresholds can identify heterogeneous atrial scarring, which can then be effectively homogenized to eliminate atypical AFLs.
METHODS: We studied consecutive patients who presented to Electrophysiology laboratory for atypical AFL ab

BACKGROUND: Idiopathic premature ventricular complexes (PVCs) that originate from the left fascicular system have rarely been reported.
OBJECTIVE: The present study aimed to investigate the electrophysiological characteristics and the ablation strategy for this category of PVCs.
METHODS: Of 648 patients with idiopathic PVCs, 27 were enrolled in this study. All the PVCs presented with a relatively narrow QRS complex and right bundle branch block morphology. The mean QRS duration was 117.4 ± 8.6 ms. Radiofrequency catheter ablation was applied at the site recording the earliest fascicular pote

Although radiofrequency ablation has revolutionized the management of tachyarrhythmias, the rate of arrhythmia recurrence is a large drawback. Successful substrate identification is paramount to abolishing arrhythmia, and bipolar voltage electrogram's narrow field of view can be further reduced for increased sensitivity. In this report, we perform cardiac mapping with monophasic action potential (MAP) amplitude. We hypothesize that MAP amplitude (MAPA) will provide more accurate infarct sizes than other mapping modalities via increased sensitivity to distinguish healthy myocardium from scar ti

BACKGROUND: Concomitant epicardial ablation of ventricular tachycardia (VT) remains a clinical challenge in cardiac surgery due to the need for intra-operative mapping. A preoperative patient-tailored epicardial guide could provide an intraoperative 'blueprint' of arrhythmogenic target substrate, thereby facilitating the workflow. Thus far, no 3D printing material or technique has fully met the requirements for this application. This study explores the feasibility of high-consistency rubber (HCR) silicone molding to produce low-cost, customized guides for VT scar ablation.

UNLABELLED: A 23-year-old woman presented with 200,409 heart beats/day due to an atrial tachycardia (AT) lasting one year and tachycardia-induced cardiomyopathy. None of the antiarrhythmic or rate control drugs terminated the AT or sufficiently reduced the heart rate during the AT. Thus, she underwent radiofrequency catheter ablation of the AT. Ultra-high-resolution mapping suggested that the AT originated from the epicardial left atrial appendage (LAA), and endocardial radiofrequency ablation failed to eliminate the AT, although acceleration and transient termination of the AT during ongoing

INTRODUCTION: Catheter ablation is a therapeutic option to suppress ventricular tachycardia (VT) in the setting of dilated-phase hypertrophic cardiomyopathy (DHCM). However, the characteristics of the arrhythmogenic substrate and the ablation outcome are not fully illustrated.
METHOD: A total of 23 ablation procedures for drug-refractory sustained monomorphic VTs in 13 DHCM patients (60 ± 11 years, one female, the left ventricular [LV] ejection fraction 39% ± 9%, the LV mass index 156 ± 39 g/m2 ) were performed. The distribution of VT substrate as endocardial or epicardial/intramural was base

INTRODUCTION: Inappropriate sinus tachycardia (IST) is characterized by increased heart rate out of proportion to normal physiologic demand. IST ablation is challenging for the electrophysiology community due to the epicardial location of the sinus node and the risk of phrenic nerve (PN) injury during catheter ablation. In this study, we investigated the safety and efficacy of a minimally invasive thoracoscopic surgery for elimination of IST.
METHODS: Patients with IST who failed medical therapy or endocardial ablation underwent minimally invasive thoracoscopic epicardial ablation. Epicardial

PURPOSE: Epicardial ablation and mapping are critical adjuncts to the electrophysiologist's approach to arrhythmias; however, ablation within the epicardial space requires the avoidance of coronary arteries (CA). We aimed to evaluate the feasibility and performance of a novel-stabilizing ablation sheath housing an intracardiac echocardiography (ICE) catheter to (1) obtain Epicardial Echocardiography (EE) images, (2) visualize CAs, and (3) enable targeted delivery of radiofrequency energy away from visualized CAs.
METHODS: We designed a sheath that could enclose a regular ICE catheter. This sh

BACKGROUND: Type 2 diabetes (T2D) and obesity are associated with nonalcoholic fatty liver disease, cardiomyopathy, and cardiovascular mortality. Both show stronger links between ectopic and visceral fat deposition, and an increased cardiometabolic risk compared with subcutaneous fat.
OBJECTIVES: This study investigated whether lean patients (Ln) with T2D exhibit increased ectopic and visceral fat deposition and whether these are linked to cardiac and hepatic changes.
METHODS: Twenty-seven obese patients (Ob) with T2D, 15 Ln-T2D, and 12 normal-weight control subjects were studied. Subjects u

Quinidine and verapamil are widely used as antiarrhythmic agents and their combination is often used in the treatment of supraventricular tachycardia. This study was undertaken to clarify, whether these drugs exert proarrhythmic effects on the ventricles in therapeutic concentrations and whether possible arrhythmogenic effects might be enhanced by combination. Isolated rabbit hearts perfused according to the Langendorff technique were treated with increasing concentrations of quinidine (0.05 to 3.5 microM) or verapamil (5 to 50 nM) or of their combination (70:1 or 10:1, quinidine:verapamil) co

Left ventricular outflow tract (LVOT) ventricular arrhythmias comprise one-third of outflow tract ventricular arrhythmias. The LVOT comprises the aortic cusps, infracuspal left ventricle, aortomitral continuity, great cardiac vein, intramural myocardium, and the left ventricular (LV) summit, each providing potential arrhythmogenic sites. Electrocardiographic features provide important initial clues to differentiate LVOT from RVOT origins and further localize arrhythmia sources within the LVOT. However, definitive localization relies on systematic electroanatomic mapping. Electroanatomic mappin

Ablation strategies for almost all types of ventricular tachycardias have now been established. The optimal ablation strategy for ventricular tachycardia is determined by the site of origin and the electrophysiological mechanisms. Electrocardiograms, an understanding of the common sites of basic disease, and identification of the scar site using imaging modalities might be helpful for predicting the originating location. Electrophysiological activation mapping is the gold standard for identification of the ventricular tachycardia substrate. However, when activation mapping of scar-related vent

The purpose of this study was to develop a surgical treatment for atrial flutter using intraoperative activation sequence mapping to minimize the surgical procedure necessary to ablate the flutter. A canine model (n = 10) of left atrial enlargement was developed by creating a shunt from the left subclavian artery to the left superior pulmonary vein. Sustained atrial flutter was easily induced in this model. The flutter consisted of a single reentrant circuit that rotated around one or two anatomic obstacles linked by a region of functional block. Epicardial templates, consisting of 252 bipolar
