OBJECTIVES: Chronic subdural hematoma (CSDH) is a common neurosurgical condition, particularly in the elderly. While traditional surgical options such as craniotomy and burr hole craniostomy remain prevalent, endoscopic evacuation may offer a minimally invasive approach. This study aimed to evaluate the safety and feasibility of a novel device, the Minimally Invasive Cortical Access System (MICAS), with flexible endoscopic evacuation of CSDH.
METHODS: A prospective study was conducted from April 2021 to April 2022, enrolling five patients with symptomatic CSDH. Patients underwent endoscopic h

BACKGROUND: Temporary vena cava filters have been used for protection from potentially fatal pulmonary embolism. However, recent reports suggested that they may be associated with serious adverse complications including filter-related thrombosis. The purpose of this study was to examine the clinical complications of temporary vena cava filter placement.
METHODS: We enrolled 40 consecutive patients from January 2006 to December 2010 who underwent percutaneous temporary vena cava filter insertion in Saitama Medical Center, Jichi Medical University.
RESULTS: Major filter complications related t

Incorrect implantation of a ventricular pacemaker (PM) lead into the left ventricle (LV) is a known problem associated with permanent pacing. The optimal management of such cases identified late has not been clearly established. Generally acceptable management options are: open-chest cardiac surgery using cardio-pulmonary bypass, chronic anticoagulation and antiplatelet-drugs therapy. Rarely, the problem is solved by percutaneous LV lead extraction. We present a case of a patient with DDD pacing and ventricular lead implanted incorrectly into the LV apex region via an atrial septal defect eigh

BACKGROUND: Left ventricular (LV) thrombectomy is a high-risk procedure, but when anticoagulation fails, it may be an important treatment option to prevent a catastrophic outcome.
CASE SUMMARY: A 13-year-old boy with congenitally corrected transposition of the great arteries developed an LV thrombus after heart transplantation despite systemic anticoagulation. Given ongoing cerebrovascular events, we elected thrombus removal via transcatheter snare thrombectomy assisted by the ONO retrieval device.
DISCUSSION: There is limited evidence on the management of LV thrombectomy if anticoagulation

During endovascular aneurysm repair (EVAR), interruption of the internal iliac arteries (IIAs) or the inferior mesenteric artery by stents or embolization is thought to cause colon ischemia. To minimize this risk, attempts have been made to preserve the IIAs using iliac branch devices or IIA revascularization. Here we present our experience of colon ischemia after EVAR in a patient with bilaterally patent IIAs without evidence of embolism. A 70-year-old man had abdominal pain and a ruptured abdominal aortic aneurysm was found. We performed EVAR with custom-made tube grafts preserving the bilat

OBJECTIVES: Carotid angioplasty and stenting procedures are associated with an obligatory release of particulate debris into the distal cerebral circulation. Although most of the emboli are small and do not result in symptomatic neurologic deficits, some may be large enough to cause stroke. For this reason, a variety of filters and balloon occlusion devices have been employed as adjuvants to decrease the risk of distal embolization during carotid stenting. Some of these devices rely on the arrest of antegrade blood flow with the use of inflow arrest. The present study was undertaken to investi

BACKGROUND: Data regarding left atrial appendage closure (LAAC) in patients with left atrial appendage (LAA) thrombus are limited. Recently published cases have mostly been guided by transesophageal echocardiography. Intracardiac echocardiography (ICE) is now widely used during LAAC procedures.
OBJECTIVE: This is the first study to report the feasibility of LAAC in patients with LAA thrombus guided by ICE.
METHODS: Patients with persistent LAA thrombus despite anticoagulation or contraindications to anticoagulation who underwent a modified ICE-guided LAAC procedure between June 2021 and Apri

OBJECTIVE: This study sought to identify patient and left atrial morphology specific factors associated with early complications for left atrial appendage occlusion with the Watchman™ (Boston Scientific, Natick, MA) device.
BACKGROUND: Oral anticoagulation (OAC) is recommended for patients with atrial fibrillation, however, long-term OAC compliance is poor. In randomized control trials, the Watchman™ device has demonstrated superiority over OAC with warfarin for all cause and cardiovascular mortality and hemorrhagic stroke. However, predictors of procedural complications have not yet been wel

BACKGROUND Despite use of optimal oral anticoagulation regimens, left atrial appendage (LAA) thrombus may develop and persist in patients with atrial fibrillation (AF). The therapeutic options in this population are limited. Percutaneous LAA closure (LAAC) is performed to reduce thrombus formation and the risk of thromboembolism. However, this approach is prohibited in patients with LAA thrombus. We report the case of a 76-year-old man with AF and persistent LAA thrombus who underwent percutaneous LAAC with the LAmbre device protected by the SENTINEL Cerebral Protection System (CPS). OBJECTIVES: The goal of this study was to compare the rate of cerebral microembolization during carotid artery stenting (CAS) with proximal versus distal cerebral protection in patients with high-risk, lipid-rich plaque.
BACKGROUND: Cerebral protection with filters partially reduces the cerebral embolization rate during CAS. Proximal protection has been introduced to further decrease embolization risk.
METHODS: Fifty-three consecutive patients with carotid artery stenosis and lipid-rich plaque were randomized to undergo CAS with proximal protection (MO.MA system, n = 26) or distal protection

BACKGROUND: Distal landing zone accuracy is critical in some intracranial aneurysms treated with the Pipeline Embolization Device (PED), and delayed opening of the distal end of the device can complicate the procedure. Here, we report a technical nuance that facilitates accurate placement of the distal end of the PED by ex-vivo, pre-implantation release of the PED Flex polytetrafluoroethylene (PTFE) sleeves.
TECHNICAL NOTE: The PED Flex is partially pushed out of the introducer sheath ex-vivo, pre-implantation until the distal PED opens entirely and the PTFE sleeves are located distal to the
