BACKGROUND: Distal protection devices are increasingly used to prevent embolization during percutaneous coronary interventions (PCI) in saphenous vein grafts (SVG) and native coronary arteries (NV). During interventions with the Filterwire device we have observed reduced flow that is reversible following removal of the filter (filter no reflow, FNR), which might be erroneously interpreted as true no reflow and might be associated with reduced capture efficiency of the basket.
METHODS: We analyzed the incidence of FNR in 58 patients (60 lesions) at high risk of embolization undergoing PCI of e

Intraplaque hemorrhage (IPH) and ulcers are the major findings of unstable plaques. In addition, initial symptoms are associated with postprocedural complications after carotid artery stenting (CAS). The aim of this study was to determine the safety of CAS using an embolic protection device in symptomatic patients with severe carotid artery stenosis and unstable plaques such as IPH and ulcers. This retrospective study included 140 consecutive patients with severe carotid stenosis. These patients underwent preprocedural carotid vessel wall imaging to evaluate the plaque status. We analyzed the

OBJECTIVES: We hypothesized that the prophylactic administration of sodium nitroprusside (NTP) during saphenous vein graft (SVG) PCI would ameliorate the detrimental effects of distal embolization and reduce the frequency and magnitude of post-procedural myonecrosis.
METHODS: Sixty-four consecutive patients with normal preprocedural cardiac enzymes underwent SVG PCI without embolic protection devices and received prophylactic intragraft NTP before initial device activation. For each case, 2 control patients were selected in reverse chronologic order and were matched for stent use, thromboathe…

Carotid artery stenting (CAS) with proximal protection can expose occlusion intolerance (OI) due to ipsilateral cerebral hemisphere hypoperfusion. Near-infrared spectroscopy (NIRS) cerebral oximetry can monitor regional cerebral oxygenation (rSO2) in the frontal lobes, and is used during CAS to predict cerebral blood flow insufficiency. The aim of this study was to evaluate rSO2 as a predictor of OI during CAS. We retrospectively examined 146 patients who underwent CAS with proximal protection. An INVOS® NIRS oximeter was used for rSO2 measurement, which was compared with stump pressure (SP) m

BACKGROUND: Large intracoronary thrombus during primary PCI for ST-elevation myocardial infarction (STEMI) remains challenging, particularly when aspiration techniques fail. We report a novel mechanical retrieval technique using balloon-assisted locking between a guide extension catheter and a distal protection device, enabling en bloc thrombus extraction.
CASE SUMMARY: An 80-year-old man presented with inferior STEMI due to total occlusion of the proximal right coronary artery. Repeated aspiration attempts failed to retrieve the thrombus. A previously described guide extension catheter-dista

Slow-flow phenomenon is frequently observed during carotid artery stenting (CAS) with a filter embolic protection device. It results in technical difficulties and can lead to adverse neurological events. Flow impairment is thought to be caused by plaque entrapped by the filter and/or blood coagulation on the filter. Characteristics of heparin- or urokinase-treated polyurethanes were analyzed by surface plasmon resonance, and the fibrinolytic activity of the urokinase-treated filter of Angioguard XP was estimated by the fibrin plate assay. A filter membrane of Angioguard XP protection device wa

We present two cases with chronic total occlusion of the saphenous vein graft in two patients with a history of previous bypass surgery with unfavorable anatomic features for recanalization of the native coronary artery. In the first case, two dedicated attempts for recanalization of chronic total occlusion of the native artery failed and in the second case there was not an adequate visualization of the native vessel beyond the occlusion point, not even by contralateral injection. Excimer laser atherectomy was used in both cases as an adjuvant therapy during recanalization of the saphenous vei

The purpose of this study was to prospectively evaluate the incidence of retinal emboli during carotid angioplasty and stenting (CAS) and to correlate emboli with clinical findings and transcranial Doppler (TCD)-detected cerebral embolic load. Between 2001 and 2005, 33 CAS procedures in 32 patients (23 [72%] male, 19 [58%] symptomatic, mean age 72.5 years [range 54.6 to 83.9]) scheduled for CAS were included in this study. Bilateral fundoscopy with retinal photography was performed by an experienced ophthalmologist immediately before, immediately after (fundoscopy only), and 1 day after the pr

Carotid endarterectomy has been the standard of therapy for carotid occlusive disease in stroke prevention. More recently, carotid angioplasty and stenting became an important alternative in the treatment of carotid occlusive disease. The widespread use of cerebral protection devices has substantially decreased the morbidity of the procedure. As the experience with carotid stenting increases, so does the enthusiasm for the potential of this minimally invasive carotid intervention to become the main treatment option for the high risk patients, as well as for the average asymptomatic patient. BACKGROUND AND PURPOSE: A major concern during carotid artery stent placement is the potential for cerebral embolism. Diminishing the number of device manipulations across the lesion might reduce procedural stroke risk. For this purpose, we report our initial experience with carotid stent placement without the use of either balloon angioplasty or distal protection devices.
MATERIALS AND METHODS: Eighty-seven consecutive patients with 100 carotid stenoses compose this series. Ninety four of the 100 hundred stented carotid arteries were either symptomatic (58 [58%]) or had a greater than 70% st

BACKGROUND: Transfemoral carotid artery stenting (CAS) with endovascular proximal flow blockage is deemed able to reduce the cerebral embolization observed during filter-protected CAS. We evaluated clinical outcome and intraoperative embolization rates, measured by diffusion-weighted magnetic resonance imaging (DW-MRI) and transcranial Doppler monitoring, in a series of patients undergoing CAS with proximal flow blockage.
METHODS: A series of 35 consecutive patients with symptomatic or asymptomatic internal carotid artery stenosis ≥70% were included to undergo CAS with proximal flow blockage,

BACKGROUND: Stroke remains a significant complication after transcatheter aortic valve replacement (TAVR), and embolic debris generated during the procedure is a major contributor. The Sentinel Cerebral Protection System is designed to reduce periprocedural stroke and stroke-related complications, but real-world data on device-related complications remain limited.
METHODS: We performed a retrospective descriptive analysis of post-marketing surveillance data from the FDA Manufacturer and User Facility Device Experience (MAUDE) database (September 2022-January 2026) to assess Sentinel-related a
