Photon-counting detector computed tomography for temporal bone: does higher resolution matter?

PURPOSE OF REVIEW: Photon-counting detector computed tomography (PCD-CT) represents a major technological advancement in computed tomography, offering improved spatial resolution, reduced image noise, and increased dose efficiency compared with conventional energy-integrating detector CT (EID-CT). Because many clinically relevant structures within the temporal bone are submillimeter in size, the temporal bone represents an ideal region for determining whether these technical improvements translate into meaningful clinical benefits. This review evaluates the emerging literature on PCD-CT in tem

Impact of manual crimping on stapedotomy outcomes.

BACKGROUND: The impact of tight stapes crimping on hearing is a matter of debate. Several studies postulate that tight crimping is essential for lifelong success, whereas others have debated whether firm attachment leads to incus necrosis. Several types of prostheses with different coupling mechanisms have been developed, and manual crimping remains the most frequently used technique. This study investigates whether tightness really does affect hearing outcome. METHODS: The hearing results of patients who underwent primary stapedotomies using three different titanium pistons were analysed. Th

Design Study of a Round Window Piezoelectric Transducer for Active Middle Ear Implants.

This report describes the design of a new piezoelectric transducer for round window (RW)-driven middle ear implants. The transducer consists of a piezoelectric element, gold-coated copper bellows, silicone elastomer (polydimethylsiloxane, PDMS), metal cylinder (tungsten), and titanium housing. The piezoelectric element is fixed to the titanium housing and mechanical resonance is generated by the interaction of the bellows, PDMS, and tungsten cylinder. The dimensions of PDMS and the tungsten cylinder with output characteristics suitable for compensation of sensorineural hearing loss were derive

Endoscopic Stapedotomy: A Systematic Review.

OBJECTIVES: Stapes surgery has evolved from its origins in 1956. Microscopic assisted stapedotomy remains the most common technique but the introduction of endoscopic ear surgery has led to some units using this new approach. The endoscope delivers a wide angled, contextual view of the stapes, and associated pathology. This systematic review provides a critical analysis of the current published endoscopic data, allowing comparison to the established microscopic technique. DATA SOURCES: Six databases (PubMed, Medline, Cochrane database, AMED, EMBASE, and CINAHL) were searched for studies withi

A New Stapes-Head Coupler for the Vibrant Soundbridge System.

INTRODUCTION: The Vibrant Soundbridge (MED-EL Medical Electronics, Austria) is an active middle ear implant with a floating mass transducer (FMT) for patients with conductive, sensorineural, or mixed hearing loss. While the FMT is vertically aligned above the stapes head (SH) with the current Vibroplasty Clip coupler (MED-EL Medical Electronics), the new SH coupler was developed to mount the FMT on the inferior side of the stapes and to fit in the reduced middle ear space after canal-wall-down mastoidectomy. METHODS: Using 11 human cadaveric temporal bones (TBs), placements of the new SH coup

Multi-operated cholesteatoma: when two surgeries are not enough.

PURPOSE: Most studies regarding residual and recurrent cholesteatoma focus on single relapse. This study examines patients who had to undergo at least three surgeries for complete eradication of their cholesteatoma, with the aim of bringing to light risk factors and assessing the functional impact of multiple surgeries on hearing. METHOD: We include 27 patients who underwent 3 consecutive surgeries for cholesteatoma between 2006 and 2016. This population represented 3.1% of all cholesteatoma operated on during that same period (868 patients). RESULTS: Cases of multi-residual and/or recurrent

[The effect of one-stage tympanoplasty for stapes fixation with tympanosclerosis].

Objective:To evaluate hearing outcome and complications of one-stage tympanoplasty in patients with stapes fixation due to tympanosclerosis. Method:59 patients(sixty-one ears) underwent one-stage tympanoplasty for stapes fixation due to tympanosclerosis were retrospectively analyzed. Stapes fixation due to tympanosclerosis were proved during the surgery in these patients diagnosed with chronic otitis media. For all the patients, tympanosclerotic plaques around stapes were removed for stapes mobilization. Then the ossicular chain was rebuilt by autogenous incus or PORP. The pre-and post-operati

Treatment of conductive hearing loss with ossicular chain reconstruction procedures.

Middle ear infections and ossicular disorders can cause conductive hearing loss. Ossicular disorders include fibrous unions in which scar tissue replaces the long process of the incus; disarticulations caused by previous surgical procedures or cholesteatomas; fixation from otosclerosis, cholesteatomas, or congenital malformations; and dislocations caused by trauma. Surgical reconstruction procedures, with the insertion of partial or total ossicular replacement prostheses, help restore conductive hearing. Patients who undergo these delicate procedures have significant hearing deficits and requi

[Predictive value of high frequency audiometry in otosclerosis].

A study was carried out to evaluate the value of high-frequency audiometry in stapes surgery in cases of otosclerosis. The hearing function was measured pre- and postoperatively by means of conventional and high-frequency audiometry (Demlar 20k). The operative findings of the gradation of otosclerosis were compared with the pre and postoperative audiometrical measurements. The results of this study point out that a clear relation exists between the preoperative high tone audiogram and the gradation of otosclerosis. In conclusion, we state that high-frequency audiometry can predict the state of

Congenital conductive hearing loss.

Congenital conductive hearing loss due to ossicular deformities can be treated by either rehabilitation with a hearing aid or surgical reconstruction. We present the results of exploratory tympanotomy performed in a large paediatric otolaryngology centre in 67 patients with non-serous congenital conductive hearing loss. Forty-two children had malformation of one or more ossicles without fixation of the stapes, and 19 had fixed stapes. In 26 cases, the surgeon decided not to perform surgical correction. Seven operated patients were lost to follow-up. As a group, 47 per cent of the patients who

Implantability of endaurally insertable active vibratory middle-ear implants - an anatomical study.

Background: Hearing loss is often treated with an acoustic hearing aid. However, distortion and insufficient gain may cause problems. Active non-acoustic vibratory middle-ear implants (AMEI) may contribute to solve this problem. We recently developed an AMEI which is to be implanted completely through the patient's external auditory canal. The device uses a light-emitting diode (LED) in the external auditory canal that stimulates a photovoltaic sensor, placed in the middle ear, through the intact tympanic membrane. This results in activation of a vibratory miniaturized piezoelectric displaceme

Medical and Audiological Indications for Implantable Auditory Devices.

Implantable auditory devices (IADs) are a viable hearing restoration option for patients with hearing loss. Conditions such as chronic otitis externa, congenital aural atresia, and chronic otitis media can be treated with a variety of implants. Progressive disease are also amenable to restoration with IADs, providing stabilized hearing. When considering the best rehabilitative options, the patient's preference, ease of surgery, ease of device use, quality of life, and the traditional alternatives (such as ossiculoplasty, hearing aids, and cochlear implants) need to be considered. Patients with

Laser-assisted fixation of a nitinol stapes prosthesis.

BACKGROUND AND OBJECTIVE: Otosclerosis is an inner ear bone disease characterized by fixation of the stapes and consequently progressive hearing loss. One treatment option is the surgical replacement of the stapes by a prosthesis. When so called "smart materials" like nitinol are used, prosthesis fixation can be performed using a laser without manual crimping on the incus. However, specific laser-prosthesis interactions have not been described yet. The aim of the present study was to elucidate the thermo-mechanical properties of the NiTiBOND® prosthesis as a basis for handling instructions for
