Detection of residual fibrotic tissue, called ghosts, after lead extraction is a new phenomenon in cardiology. This paper aims at describing the phenomenon of ghosts and determining their characteristic features. The study group consisted of 580 consecutive patients who underwent transvenous lead extraction (TLE) due to local infection, endocarditis and a superfluous lead. Each patient was clinically examined with the application of transthoracic echocardiography and transesophageal echocardiography directly before and after TLE. In the study population ghosts were detected in 110 patients (19

BACKGROUND: The guidelines for diagnosis of periprosthetic joint infection (PJI) introduced by the American Academy of Orthopaedic Surgeons served the orthopedic community well. However, they have never been validated and do not account for newer diagnostic modalities. Our aim was to update current guidelines and develop an evidence-based and validated diagnostic algorithm.
METHODS: This multi-institutional study examined total joint arthroplasty patients from 3 institutions. Patients fulfilling major criteria for infection as defined by Musculoskeletal Infection Society were considered infec

Periprosthetic joint infection (PJI) after total ankle arthroplasty (TAA) is a serious complication, and a reliable diagnostic test to identify PJI is needed. The purpose of this study was to investigate the use of synovial α-defensin levels in identifying PJI of the ankle. Data from 33 patients were retrospectively collected between September 2015 and May 2018. Patients who had pain or suspected loosening after TAA and who had undergone joint aspiration were included in the study. Aspiration was performed in a semisterile theatre. Synovial fluid was processed in descending order for microbiol

INTRODUCTION: Reinfection after two-stage exchange arthroplasty is a difficult clinical scenario with limited data on adequate treatment algorithms. Beside the possibility of treatment failure and a new intraoperative infection at the time of reimplantation, hematogenous seeding could play an up to date underestimated crucial role as another cause of an infection after two-stage exchange. The aim of this study was to evaluate its incidence and treatment possibilities in a prospectively followed case series.
METHODS: All consecutive hip and knee periprosthetic joint infection cases (93 hips an

BACKGROUND: Two-stage exchange arthroplasty is the preferred treatment for chronic periprosthetic joint infection following total hip arthroplasty (THA). These patients are at high risk of substantial blood loss and perioperative blood transfusion. Our study aimed at determining risk factors for blood transfusion during a 2-stage exchange for infected THA.
METHODS: Medical records of 297 patients with infected THA who underwent 2-stage exchange arthroplasty from 1997 to 2016 were reviewed. Blood loss was calculated using a validated formula. Transfusion data, clinical information, and operati

BACKGROUND AND PURPOSE: The risk of complications in cirrhosis patients after orthopedic surgery is unclear. We examined this risk after total hip arthroplasty (THA) or total knee arthroplasty (TKA).
PATIENTS AND METHODS: Using Danish healthcare registries, we identified all Danish residents who underwent a THA or TKA for primary osteoarthritis in the period 1995-2011. We compared the risk of complications in patients with or without cirrhosis.
RESULTS: The surgical technique was similar in the 363 cirrhosis patients and in 109,159 reference patients, but cirrhosis patients were more likely

The outcomes of 18 primary or revision total hip (THA) and knee arthroplasties (TKA) in 9 patients with cardiac transplants were reviewed. Primary total joint arthroplasties were performed for osteonecrosis (5 hips) or osteoarthritis (5 hips, 4 knees). There were no infections in any of these patients. Final Harris Hip Scores were 71.8 for patients with osteonecrosis and 88.6 for osteoarthritis. Eight of 10 hips were pain-free at final follow-up. Two of the 10 primary THAs required late revision at 7 and 10 years after the index arthroplasty. One patient (2 hips and 1 knee) had chronic bilater

Fever after transcatheter aortic valve implantation (TAVI) is common and may result in extensive workup, treatment with broad-spectrum antibiotics, and prolonged hospitalization. Despite these consequences, the prevalence and nature of fever after TAVI and whether cases of fever could be attributed to an infectious origin have not been studied thoroughly to date. We conducted an observational retrospective analysis of 148 consecutive patients undergoing percutaneous transfemoral TAVI at the Tel-Aviv Medical Center. All patients were treated with antibiotic prophylaxis using first- or second-ge

The fungus Cryptococcus neoformans can cause common opportunistic infection in acquired immune deficiency syndrome (AIDS) patients. But other conditions can be associated with sarcoidosis. Meningoencephalitis is the most common manifestation of this disease. One of the most important neurological complications is the development of intracranial hypertension (ICH), which may result in high morbidity and mortality. We report the case of a patient harboring a ventriculoperitoneal shunt, and having contracted a cryptococcal meningitis as a risk factor for pulmonary sarcoidosis. Brain MRI showed ar

BACKGROUND: Total shoulder arthroplasty (TSA) has seen a dramatic rise in utilization, particularly in the elderly population. While indications have expanded beyond glenohumeral osteoarthritis (OA) to include proximal humerus fractures (PHFs), robust evidence comparing outcomes between these groups remains limited. We sought to compare the rate of surgical and medical complications of TSA (reverse or anatomic) performed for PHFs versus OA in elderly patients at 1 and 3 years post-operatively.

Polyetheretherketone (PEEK) has been regarded as a prospective alternative orthopedic implant, because of its elastic modulus approximately equal to human cortical bone. However, the poor antibacterial and angiogenic properties of bioinert PEEK have become a limitation to its clinical application. Therefore, a biomimetic metal organic framework acid-controlled release nanocarrier consisting of Zeolitic Imidazolate Framework-8 (ZIF-8) nanoparticles loaded with gentamicin and modified with silk fibroin nanospheres for PEEK implants is prepared. In this study, acidity-responsive PEEK implant can
