Intravascular versus surface cooling for targeted temperature management after out-of-hospital cardiac arrest: an analysis of the TTH48 trial.

BACKGROUND: The aim of this study was to explore the performance and outcomes for intravascular (IC) versus surface cooling devices (SFC) for targeted temperature management (TTM) after out-of-hospital cardiac arrest. METHODS: A retrospective analysis of data from the Time-differentiated Therapeutic Hypothermia (TTH48) trial (NCT01689077), which compared whether TTM at 33 °C for 48 h results in better neurologic outcomes compared with standard 24-h duration. Devices were assessed for the speed of cooling and rewarming rates. Precision was assessed by measuring temperature variability (TV), i.

Performance of life support breathing apparatus for under-ice diving operations.

INTRODUCTION: Single-hose scuba regulators dived in very cold water may suffer first- or second-stage malfunction, yielding complete occlusion of air flow or massive freeflow that rapidly expends a diver's air supply. PURPOSE: This study, conducted in Antarctica, evaluated the under-ice performance of a sampling of commercially available regulators. METHODS: Seventeen science divers logged a total of 305 dives in -1.86°C seawater under 6-meter-thick Antarctic fast-ice over two field seasons in 2008 and 2009. Dive profiles had an average depth of 30 msw and dive time of 29 minutes, including

Should early amputation impact initial fluid therapy algorithms in burns resuscitation? A retrospective analysis using 3D modelling.

AIMS: While the epidemiology of amputations in patients with burns has been investigated previously, the effect of an amputation on burn size and its impact on fluid management have not been considered in the literature. Fluid resuscitation volumes are based on the percentage of the total body surface area (%TBSA) burned calculated during the primary survey. There is currently no consensus as to whether the fluid volumes should be recalculated after an amputation to compensate for the new body surface area. The aim of this study was to model the impact of an amputation on burn size and predict

Computer-aided measurement of psoriatic lesion area in a multicenter clinical trial--comparison to physician's estimations.

BACKGROUND: There is a great need for accurate and informative outcome measures in clinical trials. In psoriasis, therapeutic effect is usually assessed by physician's estimation of psoriatic area and other signs. These measures were found to be un-reproducible. Several automated procedure for more reproducible measurement of psoriatic area were developed, but were not suitable for large-scale trials. OBJECTIVES: In a multicenter clinical trial, we tested a method where the advantage of accurate computerized measurement of the area on the digital photograph was combined with physician's profi

Capsular elasticity and joint volume in recurrent anterior shoulder instability.

It is commonly claimed that unstable shoulders have an altered capsular elasticity and increased joint volume. To find out if there is a correlation between anterior instability and increased joint volume, 20 patients were examined while undergoing arthroscopic surgery of the shoulder. There were four anterior subluxators, 11 anterior dislocators, and five bilateral dislocators, and in all patients the contralateral shoulder was also examined. During continuous saline infusion, a pressure/volume curve was recorded within the range of 0-180 mm Hg. Joint volume was correlated with body surface a

Postoperative functioning of the remaining stomach after pyloric sphincter preserving gastrectomy.

Pyloric sphincter preserving gastrectomy (SPG) has been used in 120 cases and the results were compared with those obtained in 73 cases who underwent the Billroth-I operation (B-I). No significant differences were observed in postoperative complications or anastomotic dysfunction. SPG patients, however, showed a longer gastric emptying time than the B-I patients, whereas the B-I patients showed a markedly greater increase in duodenogastric reflux than the SPG patients.

Zinc- and copper-replacement therapy--a must in burns and scalds in children?

The author describes experiences gained in the zinc and copper serum concentration estimations in burned children. Without added substitution therapy the serum zinc concentration rises by 19.9% and the serum copper concentration by 5.8% in the course of time. If only zinc is substituted the values rises by 83.6% for zinc, but the value for copper decreases by 18.8%. If the serum zinc and copper concentration is low on admission and if both added zinc and copper are given, the concentration for zinc rises by 102.5%, but the concentration of copper only rises by 25% and does not reach normal val

Incidence and strategies for preventing sustained hypothermia of crash victims during prolonged vehicle extrication.

BACKGROUND: Vehicle extrication of crash victims is a highly-demanding challenge, due to the frequently life-threatening injuries of entrapped occupants. In this phase, crash victims are often exposed to the outdoor-temperature, with the risk of sustained hypothermia. Hypothermia can significantly raise the morbidity and mortality rates of crash victims. Therefore, we have correlated the incidence of severe car accidents with entrapped patients, the outdoor conditions, and expenditure of time for extrication. Furthermore, different warming strategies have been evaluated regarding their integra

Active body surface warming systems for preventing complications caused by inadvertent perioperative hypothermia in adults.

BACKGROUND: Inadvertent perioperative hypothermia is a phenomenon that can occur as a result of the suppression of the central mechanisms of temperature regulation due to anaesthesia, and of prolonged exposure of large surfaces of skin to cold temperatures in operating rooms. Inadvertent perioperative hypothermia has been associated with clinical complications such as surgical site infection and wound-healing delay, increased bleeding or cardiovascular events. One of the most frequently used techniques to prevent inadvertent perioperative hypothermia is active body surface warming systems (ABS

Characterization of pulsatile secretion and clearance of plasma cortisol in premature and term neonates using deconvolution analysis.

Pulsatile secretion of cortisol (F) has not been documented in the newborn infant. Using repeated blood sampling and deconvolution analysis, we investigated F secretion and elimination dynamics in a group of five premature (gestational age, 24-34 weeks) and five term neonates. These infants had required placement of an umbilical arterial cannula for monitoring respiratory status, but were otherwise clinically stable. Blood samples were obtained at 15-min intervals for a 6-h period. All plasma F determinations were 58 nmol/L (2.1 micrograms/dL) or more, and pulsatile F secretion was observed in

Psoas muscle density predicts elective colorectal surgical outcomes more accurately than psoas muscle area or indexed area.

BACKGROUND: CT measurements of psoas muscle density (PMD) and area (PMA) (with or without indexing to height-squared or body-surface-area) are used interchangeably as sarcopenia measures - it is unknown which best correlates with surgical risk. OBJECTIVES: 1.Determine the correlation between psoas muscle density, area, and indexed area; 2.Identify which psoas measures most strongly associated with surgical outcomes DESIGN: The University Hospital Geelong Colorectal database included all patients who underwent elective colorectal surgery from 2007 to 2014 (minimum five-years follow-up). Echocardiographic evaluation of left and right ventricular function in mild hypertension.

Thirty-one cases of untreated 'mild hypertension' and equal number of age and sex matched controls with 'normal' blood pressure were evaluated by echocardiography. Patients with mild hypertension had significantly increased left ventricular mass index, concentric remodeling, and diastolic dysfunction. Thickness of right ventricular anterior wall, flow velocities across tricuspid and pulmonary valves were also significantly higher in hypertensives. Pulmonary flow acceleration time was significantly less in hypertensives. On multiple regression analysis, mitral valve 'A' wave velocity alone corr

3D PED BURN app: A precise and easy-to-use pediatric 3D burn surface area calculation tool.

BACKGROUND AND AIMS: Calculating the precise total burn surface area is crucial when treating burn patients, particularly children. The Lund and Browder chart and Rule of Nines, 2-dimensional diagrams that are widely used, are subject to high interrater variance, and they can severely overestimate the burn area. Previously, the adult 3-dimensional burn area calculation mobile application was developed. Aiming to improve accuracy, a 3-dimensional pediatric burn surface area calculation mobile application ("3D PED BURN") was developed to overcome the limitations of the conventional methods. MET

Tar burns in the southwest.

The burns which result from contact of human skin with hot tar may be quite serious in proportion to the body surface area involved. Although tending toward partial thickness burns, patchy areas of full thickness skin loss are commonly observed. The use of petrolatum-based ointments on the burn initially to dissolve the tar into the dressings seems like the most efficient and humane method of tar removal. Subsequently, care of the wound is like that of any other burn. Tar burns involving greater than 10 per cent of the body surface area are likely to be the most serious and require intravenous

Exercise Training Improves Microvascular Function in Burn Injury Survivors.

INTRODUCTION: Vasodilator function is impaired in individuals with well-healed burn injuries; however, therapeutic interventions that lessen or reverse this maladaptation are lacking. The purpose of this study was to test the hypothesis that a 6-month community-based exercise training program would increase microvascular dilator function in individuals with well-healed burn injuries, irrespective of the magnitude of the injured body surface area. Further, we hypothesize that macrovascular dilator function would remain unchanged posttraining. METHODS: Microvascular function (forearm reactive h

Bergmann's rule is a "just-so" story of human body size.

Carl Bergmann was an astute naturalist and physiologist. His ideas about animal size and shape were important advances in the pre-Darwinian nineteenth century. Bergmann's rule claims that that in cold climates, large body mass increases the ratio of volume-to-surface area and provides for maximum metabolic heat retention in mammals and birds. Conversely, in warmer temperatures, smaller body mass increases surface area relative to volume and allows for greater heat loss. For humans, we now know that body size and shape are regulated more by social-economic-political-emotional (SEPE) factors as

A National Evaluation of Time to Surgery Following Burn Center Admission Utilizing the Burn Care Quality Platform Data.

Burn injuries are a major cause of morbidity in the United States, affecting 600 000 individuals annually. Early surgical intervention improves outcomes and reduces complications; however, access to specialized burn care is limited, with only 135 burn centers nationwide. Timely surgery, particularly for large burns, is critical for preventing complications such as hypertrophic scarring and infection while optimizing recovery. This study evaluates surgical timing across pediatric and adult burn patients to inform evidence-based practices. We analyzed data from the American Burn Association's Bu
