Shortened Cerebral Circulation Time Predicts Resistance to Obliteration in High-Flow Brain Arteriovenous Malformations After Stereotactic Radiosurgery.

BACKGROUND AND OBJECTIVES: Treatment selection for brain arteriovenous malformations (BAVMs) is complicated by BAVM size, location, and hemodynamics. Quantitative digital subtraction angiography is used to quantify the hemodynamic impact of BAVMs on cerebral circulation. This study investigated the association between cerebral circulation time and the complete obliteration (CO) rate of BAVMs after stereotactic radiosurgery (SRS). METHODS: We analyzed the data of 143 patients who underwent SRS for BAVMs between January 2011 and December 2019 in our institute. Their pre-SRS magnetic resonance i

Intracranial Vessel Wall MRI in Cryptogenic Stroke and Intracranial Vasculitis.

INTRODUCTION: Intracranial vasculopathies easily elude classic stroke work-up. We aim in this work to show that vessel wall-MRI could prove an efficient alternative to digital subtraction angiography for the diagnosis of intracranial vasculopathies by identifying intracranial arterial vessel walls anomalies and contrast enhancement, suggestive of angiitis of the central nervous system. MATERIALS AND METHODS: Clinical and imaging characteristics of stroke patients diagnosed with primary angiitis of the central nervous system based on vessel wall-MRI were retrospectively reviewed and the clinic

CT angiography before embolization for hemorrhage in head and neck cancer.

We present a patient with advanced head and neck carcinoma and a bleeding pseudoaneurysm diagnosed by means of CT angiography; this was not apparent on conventional digital subtraction angiograms. The information provided by CT angiography facilitated rapid identification of the pseudoaneurysm and treatment with embolization. CT angiography may be helpful before embolization in cases of hemorrhage in head and neck cancer.

High-Flow Intracranial-Intracranial Interposition Bypass from A1 to M2 with Proximal Occlusion for a Large Unruptured Fusiform Aneurysm of the Middle Cerebral Artery M1 Segment: A Technical Note.

BACKGROUND: Complex fusiform aneurysms of the middle cerebral artery M1 segment are uncommon and technically demanding, characterized by circumferential dilation with frequent perforator involvement and atherosclerotic changes. When direct clipping or endovascular therapy is not feasible, intracranial-intracranial (IC-IC) bypass provides a rational revascularization option. CASE DESCRIPTION: A 55-year-old man presented with a large, unruptured fusiform aneurysm of the left middle cerebral artery M1 segment. Preoperative computed tomography angiography/digital subtraction angiography and high-

Bilateral internal carotid artery hypoplasia and multiple posterior circulation aneurysms. Importance of 3DCTA for the diagnosis.

We present a case with bilateral internal carotid artery hypoplasia and multiple posterior circulation aneurysms who was diagnosed following a subarachnoid hemorrhage. The patient was admitted to our clinic with a history of sudden and severe headache with short-term loss of consciousness and being unable to open the right eyelid five days ago. Nuchal rigidity and right partial ophthalmoplegia were found during the examination. Computed tomography revealed a subarachnoid hemorrhage. Digital subtraction angiography showed bilateral internal carotid artery hypoplasia while three-dimensional comp

Bilateral carotid and vertebral rete mirabile with aneurysm misdiagnosed as Moyamoya disease: A case report.

RATIONALE: Rete mirabile (RM) is rare in humans, normally existing in lower mammalian arteries. To study its clinical and imaging features, we report an extremely rare case that occurred in humans and review the literature. PATIENT CONCERNS: A 43-year-old female patient was admitted to our hospital because of recurrent dizziness and headache for 4 years. DIAGNOSES: The computed tomography angiography (CTA) of the head and neck indicated that the normal structure of the bilateral cervical and vertebral arteries disappeared, and the microvascular network formed, misdiagnosed as Moyamoya diseas

Blister-like aneurysms of the anterior communicating artery: a retrospective review of diagnosis and treatment in five patients.

OBJECTIVE: Blood blister-like aneurysms are small hemispherical bulges from the dorsomedial wall of the internal carotid artery that resemble berry aneurysms but differ in their clinical and surgical features. On the basis of our literature review, blister-like aneurysms have been reported to occur only at nonbranching sites of the dorsomedial internal carotid artery. In this report on our series of five patients, we describe blister-like aneurysms of the anterior communicating artery (AComA) and discuss important diagnostic and therapeutic aspects unique to them. METHODS: In our retrospectiv

Intracranial tumors examined by intraarterial DSA: a comparative angiography study.

We compared intraarterial digital subtraction angiography (DSA) studies with those of conventional angiography, performed for 24 patients who had intracranial tumors. Intraarterial DSA is more effective in evaluating tumoral blush and venous phase, but neovascularization can be judged better by conventional angiography. In most cases, intraarterial DSA can replace conventional angiography for the diagnostic and preoperative evaluation of intracranial tumors. However, conventional angiography remains the technique of choice for the evaluation of neovascularization or if the patient is not coope

Corona mortis: The leading pelvic vascular variant.

Corona mortis is a common vascular variant in the pelvis with significant clinical importance. While there is no standardised definition, the term generally refers to any anatomical situation where a vessel-either arterial or venous-crosses over the superior pubic ramus and is thus susceptible to injury. This anatomical course places it at high risk of injury in numerous clinical scenarios, including pelvic trauma and various surgical procedures. The aim of this article is to review the anatomy of arterial corona mortis using computed tomography angiography (CTA) and digital subtraction angiog

[The current role of cerebral angiography in diagnosis of cerebrovascular disease].

The role of cerebral angiography in diagnosis of cerebrovascular disease has changed considerably because of the increasing availability of neuroimaging (CT, MRI), especially MR angiography (MRA). MRA has almost entirely taken the place of cerebral angiography as a screening procedure in evaluation of the cervical carotid arteries and main trunks of intracranial arteries. However, detection of accurate morphological changes of arterial walls requires cerebral angiography, especially in cases of arteritis, arterial dissection and aneurysm of smaller than 3 mm in diameter. Cerebral angiography i

Pseudoaneurysm of the superficial temporal artery following revision of a middle cerebral artery aneurysm clipping: case report and review of the literature.

Pseudoaneurysms of the superficial temporal artery are mostly traumatic in origin. Here, a case of a superficial temporal artery aneurysm that emerged following a recraniotomy is presented. A 59-year-old woman was admitted with subarachnoid hemorrhage. She underwent a pterional craniotomy and clipping of a saccular aneurysm of middle cerebral artery bifurcation. A control digital subtraction angiography on the 3rd postoperative day revealed partial filling of the aneurysm and a revision was performed. The second control digital subtraction angiography on the 4th postoperative day of the revisi

Intrapetrous Internal Carotid Artery Pseudoaneurysm Secondary to Malignant Otitis Externa: A Case Report.

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr
