Articulating external fixation to overcome nerve gaps in lower extremity trauma.

This report describes the use of articulating external fixation in patients with lower extremity trauma with segmental nerve defects. Four patients who would otherwise require nerve grafting underwent application of an articulated external fixator, allowing optimal positioning for end-to-end, tension-free nerve repair followed by gradual lengthening. After three weeks of immobilization, the fixator was gradually advanced through the arc of the hinge. At an average follow-up of 44 months, motor function testing revealed gastroc-soleus function in all cases with a median motor grade of M4. Senso

Three-dimensionally corrective external fixator system for proximal femoral osteotomy.

The authors introduce a technique of proximal femoral osteotomy using three-dimensionally corrective external fixator systems for treatment of Legg-Calvé-Perthes disease and slipped capital femoral epiphysis, and describe the results of 47 hips in 44 children. Postoperative casting immobilization was not performed. All osteotomies gained the correction angle as planned. All osteotomies healed with no nonunions, malunions, device failures, or avascular necrosis. Complications occurred in 7 (15%) of 47 hips. Pin tract infection occurred in six (13%) hips; one of these (2%) needed debridement. Th

The evaluation of tension in an experimental model of external fixation of distal radius fractures.

A cadavaric model was used to evaluate the previously reported methods of determining excessive tension when applying an external fixator across the wrist. An osteotomy of the distal radius was performed and tension was applied incrementally across the joint in 9 cadaveric specimens. The fingers of each specimen could fully flex to the palm at all levels of tension tested. Although the radiocarpal and midcarpal joint spaces did lengthen with incremental changes in the amount of tension, there was no statistically significant correlation that could be made about the difference between the radio

[Radio-radial external fixation for correction of malunited distal radius fracture].

For correction of malunited distal radius fractures, the use of the external fixator in radio-radial application was assessed. The device has been used for one-stage correction with corticocancellous bone blocks as well as with hemicallotasis for correction of dorsal angulation. The technical application is described and the use of the external fixator defined. Fourteen patients were treated with radio-radial application, seven of those were corrected with callus distraction. The mechanical possibilities of the fixator allowed complete correction of the malunion.

Treatment of unstable distal radius fractures with cancellous allograft and external fixation.

Unstable fractures of the distal radius continue to pose a challenge to the hand surgeon. Adjunctive bone grafting is often required to augment structural integrity and aid healing. Because of the risks inherent to bone autograft harvest, however, freeze-dried, irradiated cancellous bone allograft has been used to treat unstable distal radius fractures with severe metaphyseal comminution. Seventeen patients with such fractures (mean age, 70 years; 2 males and 15 females) were treated with bone allograft and external fixation with or without internal fixation. The outcome was evaluated using th

Salvage of contaminated fractures of the distal humerus with thin wire external fixation.

Fractures and osteotomies of the distal humerus that are contaminated or infected represent a difficult management problem. Stable anatomic fixation with plates and screws, the acknowledged key to a good result in the treatment of bicondylar fractures, may be unwise. A thin wire circular (Ilizarov) external fixator was used as salvage treatment in such complex situations in five patients. The fixator allowed functional mobilization of the elbow while allowing achievement of the primary goal of eradicating the infection or colonization. Two patients required a second operation for fixation of a

Use of a New External Fixator for the Correction of Fixed Flexion Deformity of the Fingers.

Three patients with severe fixed flexion deformity of the fingers caused by trauma were reported. As an alternative treatment, a new mini external fixator was used to correct the deformities. The use of this device has made treatment of contractures simpler and more predictable. Gradual distraction and straightening was applied to loosen and extend the joints, followed by a period of mobilisation while still on the device. In all the three patients, this experimental device and approach led to improvement of the fixed flexion deformities. With use of this device, surgical release may not be ne

Small wire external fixation of high energy tibial plateau fractures.

Open plate osteosynthesis for high energy tibial plateau fractures with dissociation between the metaphysis and diaphysis has been plagued with frequent soft tissue complications. The Harbor-University of California at Los Angeles Medical Center's experience with small wire external fixation supplemented by limited internal fixation is examined. This alternative method of adequate stable fixation offers the advantage of minimal soft tissue compromise. Twenty-four patients with Schatzker Type VI tibial fractures were treated with small wire external fixation. Supplementary limited internal fixa

Intraoperative SSEP monitoring during external fixation procedures in the lower extremities.

The efficacy of somatosensory evoked potentials (SSEPs) to detect acute peripheral nerve injury during external-fixator application in the lower extremities was evaluated in 40 children with 42 Ilizarov surgical procedures. The study included patients who were either clinically normal or who had preexisting neuropathy but consistent and reliable SSEP responses preoperatively. SSEPs were recorded from the popliteal fossa and lumbar regions after alternating stimulation of the peroneal and posterior tibial nerves at the ankle. SSEP changes due to anesthesia, Ilizarov apparatus application, and o

Fractures of the base of the middle phalanx treated by a dynamic external fixation device.

Intraarticular fractures of the base of the middle phalanx are difficult to treat, especially if the fragments are comminuted. A spring dynamic traction device is described and the results in the first 14 patients presented. Patient acceptance has been high and the results comparable to other methods of treatment.

[Retrospective analysis of consecutively treated distal radius fractures with the external fixator].

The purpose of this study was to determine retrospectively some subjective and objective parameters following the operative treatment of 32 patients with "complex" (intraarticular and/or comminuted) fractures of the distal radius using the small AO external fixation device. The mean follow-up period was 20 months (minimum 4, maximum 48) and the mean age of the subjects was 62 years (minimum 27, maximum 91). Subjective results such as "general feeling", mobility, strength and pain, expressed with a scoring system (% of maximum obtainable points), showed an overall good result and ranged between

External fixator pin placement in the forearm: how safe is it? An anatomical study.

An anatomical study was undertaken using cadaveric forearms to identify the best technique and angle of pin placement for external fixator pins. We have found that inserting pins through stab incisions risks transfixion of the underlying structures and leads to an increased morbidity. We conclude that inserting pins under direct vision is much safer and that the optimum angle of pin insertion is in the coronal plane, parallel to the plane of the metacarpals.

First metatarsophalangeal joint arthrodesis utilizing a mini-Hoffman External Fixator.

Arthrodesis was initially indicated for severe hallux valgus and hallux rigidus. Today, it is primarily a salvage procedure, which provides a painless first metatarsophalangeal joint. This manuscript reviews the various procedures for arthrodesis of the first metatarsophalangeal joint. A case report is presented utilizing arthrodesis as a salvage procedure for a painful first metatarsophalangeal joint, after previously failed procedures.

Remanipulation or external fixation after slipped Colles' fractures? An anatomical study.

A series of 50 consecutive patients with Colles' fractures which redisplaced after reduction underwent either remanipulation and a plaster cast or remanipulation and external fixation. The external fixator group achieved a significantly better anatomical result than simple remanipulation and replastering. Where plaster immobilization has failed, external fixation gives a better anatomical result than remanipulation and replastering.

Treatment of forearm fractures by Hoffman external fixation. A study of 93 patients.

Ninety-three patients with acute fractures of the diaphysis of one or both bones of the forearm were treated by a Hoffmann external fixator, using a half-frame configuration. Whenever possible, closed methods of reduction were used. The versatility of the Hoffmann external fixator allows multiple attempts and possible secondary resumption of the reduction. The half-frame configuration is stable enough, allowing the restoration and maintenance of the precise anatomy of the forearm bones and early active postoperative motion. The median length of retention of the external fixator was 13 weeks. B

[External fixation in open fractures of leg. Analysis of traps and complications of the method].

A consecutive series of 51 compound tibial fractures treated by external fixation have been reviewed. All the cases whose progress was marred by complications were analysed separately. From this analysis, the following conclusions have been reached: 1. A gap at the fracture site, due to loss of bone substance or to distraction of the fragments should be treated as soon as possible by bone grafts. 2. Inadequate reduction should be corrected at the time of primary treatment. 3. Pins inserted into the region of the fracture should be repositioned early to prevent infection along the pin tracks. 4

Fracture of the patella treated by open reduction and external compressive skeletal fixation.

Open reduction combined with external compressive skeletal fixation was used to treat twenty-seven patients who had a separated fracture of the patella. Fixation was obtained by the use of two compressive clamps applied to stainless-steel pins that were inserted just proximal and distal to the proximal and distal poles of the patella. Range-of-motion exercises for the knee were begun at two weeks and the pins were removed at three to four weeks. All of the fractures healed. Twenty-four patients regained a range of motion that was equal to that of the opposite knee. There was no evidence of chr
