Minimally invasive technology for mitral valve surgery via left thoracotomy: experience with forty cases.

BACKGROUND: Recent evolution of minimally invasive technology has expanded the application of the right thoracotomy approach for mitral valve surgery. These same technological advances have also made the left posterior minithoracotomy approach attractive in complex mitral procedures. METHODS: From 1996 to 2003, 921 isolated mitral valve procedures were performed without sternotomy; 40 (4.3%) of these were performed via left posterior minithoracotomy. In the left posterior minithoracotomy group, ages ranged from 18 to 84 years; 36 patients had had previous cardiac surgery (9 on > or =2 occasio…

Perioperative myocardial ischaemia, heart rate and arrhythmia in patients undergoing thoracotomy: an observational study.

We have studied myocardial ischaemia, heart rate and arrhythmia in 82 patients undergoing elective thoracotomy. Myocardial ischaemia was detected using a microprocessor-based surveillance system programmed to record leads V2 and V5. Patients were monitored on the day before and for up to 72 h after surgery. The total monitoring time was 5158 h. The incidence of silent myocardial ischaemia before operation was 11% (nine of 82). This increased to 24% (20 of 82) after operation. Postoperative myocardial ischaemia was associated with preoperative myocardial ischaemia in six patients. Before operat…

Automatic implantable cardioverter-defibrillator implantation without thoracotomy using an endocardial and submuscular patch system.

The automatic cardioverter-defibrillator lead system is implanted by a thoracotomy procedure that may result in atelectasis, pleural effusion, cardiac tamponade and lengthy convalescence. A new defibrillator lead system that allows selection of different defibrillating current pathways is implanted without a thoracotomy. Ten patients requiring a cardioverter-defibrillator for recurrent sustained ventricular tachycardia (five patients) or aborted sudden cardiac death (five patients) were evaluated for implantation of this lead system. A lead configuration with a bidirectional defibrillating cur…

Metabolic studies following thoracotomy for lung cancer with particular reference to postoperative atrial fibrillation.

Atrial fibrillation has proved to be a frequent complication after thoracotomy for lung cancer. In order to study the possibility that metabolic changes are the trigger mechanism of these arrhythmias, free fatty acids (FFA), glycerol, triglycerides, serum insulin and glucose were determined pre- and postoperatively in patients both with and without this postoperative complication. In the primary series of patients, FFA were found to be increased immediately after the start of the atrial fibrillation, partly in relation to the morning values and partly to the 24-hour curve. This is a possible e…

Impact of Thoracotomy-Based Left Ventricular Assist Device Implantation on Right Ventricular Functional Preservation.

Right ventricular (RV) dysfunction remains a critical determinant of outcomes following left ventricular assist device (LVAD) implantation. We used a doubly robust inverse probability weighting based on propensity scores to evaluate the association between surgical approach and clinical outcomes in 187 consecutive patients (thoracotomy: n = 55; sternotomy: n = 132). The primary endpoint was RV systolic function in the early postoperative period, assessed by tricuspid annular plane systolic excursion (TAPSE). Despite comparable baseline RV performance and hemodynamics, the thoracotomy-based app…

A technical pitfall in resuscitative thoracotomy: Delayed mechanical myocardial injury from a narrow pericardiotomy.

BACKGROUND: Mechanical complications related to the configuration of the pericardiotomy during resuscitative thoracotomy (RT) have not been previously documented. No prior report has visually demonstrated a mechanism by which an insufficient pericardiotomy leaves a sharp margin that directly abrades the beating heart. CASE PRESENTATION: A 62-year-old man in traumatic cardiac arrest underwent left anterolateral RT and right thoracotomy for hemorrhage control. Pericardiotomy was performed anterior to the phrenic nerve, followed by internal cardiac massage. After temporary closure, he developed …

Single-center experience of minimally invasive ascending aortic surgery via right anterior mini-thoracotomy.

BACKGROUND: The right anterior mini-thoracotomy (RAMT) has emerged as a well-established minimally invasive surgical approach in the field of cardiac surgery and is increasingly being utilized for valve-related interventions. However, high-quality clinical evidence supporting its application in ascending aorta surgery remains limited. This study aims to evaluate the safety and feasibility of ascending aortic operations performed via the RAMT approach. METHODS: A retrospective analysis was conducted on 41 patients who underwent ascending aortic surgery using the RAMT technique at the Cardiac Va…

Is Definitive Closure Always Needed After Open-Window Thoracotomy for Post-Lung Resection Empyema?

INTRODUCTION: Open-window thoracostomy is a recognized treatment for postresection empyema, especially when complicated by bronchopleural fistula (BPF). However, the necessity of definitive closure surgery remains unclear. METHODS: We retrospectively analyzed 24 patients who developed empyema after anatomical lung resection and underwent open-window thoracostomy at our institution between April 2010 and March 2024. We evaluated factors associated with spontaneous closure of BPF and the empyema cavity, as well as the clinical indications for definitive surgical closure. RESULTS: BPF was prese

An alternative surgical approach to redo pulmonary valve replacement: the vertical left axillary thoracotomy.

Redo cardiac surgery is often needed for right ventricular outflow tract complications after tetralogy of Fallot repair. Common approaches include redo sternotomy or anterior thoracotomy. We report a case of pulmonary valve replacement performed via vertical left axillary thoracotomy, which effectively minimises cardiac injury risk during reentry and offers a safe, cosmetic alternative to conventional thoracotomy approaches.

Right anterior mini-thoracotomy for isolated aortic valve replacement: An international and multicenter study.

BACKGROUND: Right anterior mini thoracotomy (RAMT) is a sternum-sparing approach for aortic valve replacement (AVR). Single-center studies have found RAMT AVR to be safe; however, there is a paucity of large, multicenter, all-comer, and real-world data for this surgical strategy. METHODS: This retrospective study investigated the outcomes of isolated RAMT AVR at 6 centers in the United States, Canada, and Europe. Primary outcomes were death and disabling stroke within 30 days of surgery. Secondary outcomes included rate of conversion to sternotomy, prosthetic used, operative times, residual p…

Outcomes for Initially Hemodynamically Stable Pediatric Patients With Thoracic Trauma Undergoing Emergency Department Thoracotomy.

BACKGROUND: Resuscitative thoracotomy, performed in only 10%-15% of pediatric thoracic trauma cases, yields a dismally low survival rate of 3.4%. As such, an investigation into the mechanisms of such high mortality associated with this emergent procedure is warranted. OBJECTIVE: This study aims to evaluate the clinical outcomes of initially hemodynamically stable pediatric patients requiring an emergency department thoracotomy (EDT) at pediatric trauma centers (PTC), combined adult/PTCs (CTC), and adult-only trauma centers (ATC). METHODS: The American College of Surgeons-Trauma Quality Impro…

Partial Atrioventricular Septal Defect Repair through Right Thoracotomy in A 73-Year-Old Woman.

INTRODUCTION: While surgical treatment for partial atrioventricular septal defect (AVSD) is commonly performed in pediatric patients, its application is relatively rare in older patients. CASE PRESENTATION: A 73-year-old woman was referred to our hospital for evaluation of shunt blood flow caused by a partial atrioventricular septal defect and left atrioventricular valve regurgitation that caused right heart overload and cardiomegaly. She underwent minimally invasive atrial septal defect closure and left atrioventricular valve repair through right-sided thoracotomy. CONCLUSIONS: We report a …

Effects of mobilization within the first 4 h following anatomical lung resection with thoracotomy.

Thoracic incisions are very painful and complicated incisions. Many analgesic drugs including opioids are used to relieve pain in patients. We hypothesized that early mobilization and patient-centered strategy after lung resection reduce the incidence of perioperative complications and postoperative pain. We conducted a retrospective study on patients who underwent lung resection via thoracotomy and were mobilized in the first 4 h postoperatively. Mobilization was defined as standing and walking at least 100 m from the bed. If orthostatic hypotension occurred, mobilization was postponed for 30…

Delayed massive hemothorax after blunt thoracic trauma requiring thoracotomy by VATS: a case report.

Delayed hemothorax after thoracic trauma is a rare type of thoracic injury that may require angioembolization and surgical treatment. We report a case of a 59-year-old man with a delayed massive hemothorax from a fall-induced blunt thoracic trauma, causing multiple right lower rib fractures. The patient fell from a chair while standing on and working from it. He was diagnosed with right 7th-11th rib fractures, scanty hemothorax and liver contusion. The patient was hospitalized and received conservative treatment, and a delayed massive hemothorax was present on the fourth day after the injury.

Left ventricular assist device implantation via lateral thoracotomy: A systematic review and meta-analysis.

BACKGROUND: Left ventricular assist device (LVAD) implantation via lateral thoracotomy can offer similar effectiveness to conventional approaches with less perioperative adverse events. We performed a systematic review and meta-analysis to determine the potential benefits of lateral thoracotomy (LT) for LVAD implantation compared to median sternotomy. METHODS: We searched MEDLINE and Embase databases for studies comparing continuous-flow LVAD implantation using LT with conventional sternotomy. Main outcomes were perioperative mortality and complications. RESULTS: Twenty-five observational st…
