Tunnel infection is one of the most dreaded infections, post phacoemulsification. Most cases reported till now are caused by Gram-positive and Gram-negative bacteria or fungi and very few cases by the acid-fast organisms. Ocular infections by Nocardia usually have poor prognosis. Here, we report a case of tunnel infection, its management and microbiological identification of cause behind it, that is, Nocardia cyriacigeorgica a rare acid-fast microorganism.

PURPOSE: To compare the outcomes of active-fluidics based torsional phacoemulsification in diabetics and nondiabetics using a balanced tip.
METHODS: Two hundred and forty-eight patients undergoing senile cataract surgery using torsional phacoemulsification on an active-fluidics-based platform from December 2016 to August 2017 were included in this prospective, nonrandomized, interventional cohort study; of the 248 patients, 54 were controlled diabetics and 194 were nondiabetics. Intraoperative parameters such as cumulative dissipated energy (CDE), total ultrasound time, torsion usage time, to…

PURPOSE: To evaluate a novel gonioscopy score as a potential predictor for intraocular pressure (IOP) reduction after cataract surgery.
MATERIALS AND METHODS: This was a prospective study that included consecutive patients with or without glaucoma, either with open or narrow angles but without peripheral anterior synechiae, who underwent phacoemulsification. Eyes with intraoperative complications and secondary glaucoma were excluded. A preoperative gonioscopy score was created, summing the Shaffer gonioscopy grading in 4 quadrants (range, 0 to 16). To determine variables associated with IOP c…

PURPOSE: This study aimed to determine the reasons behind the failure of laser capsulotomy (LC) performed for significant posterior capsular opacification (PCO).
METHODS: Eighty-eight eyes of 88 patients referred for LC at a tertiary care center were retrospectively analyzed. The data recorded included the cause of cataract, visual acuity, duration of PCO, location of PCO, intraocular lens (IOL) position, IOL type, and lens capsule status. These data were later analyzed for determining the requirement of high pulse energy during LC and the success rate of primary LC.
RESULTS: The mean age of

PURPOSE: To describe the tearing pattern during continuous curvilinear capsulorhexis in a patient with anterior lenticonus due to Alport syndrome.
METHODS: A patient with bilateral anterior lenticonus underwent successful bilateral phacoemulsification and implantation of a single-piece intraocular lens.
RESULTS: The capsule tore in an atypical cogwheel-like tearing pattern during capsulorhexis. This tearing pattern likely correlates to the known ultrastructural characteristics of thinning and periodic dehiscence and breaks in the anterior capsule.
CONCLUSIONS: Knowing this tearing pattern p…

SETTING: Department of Ophthalmology, Clinical Center, University of Debrecen, Debrecen, Hungary.
METHODS: 52 patients with cataract were randomly assigned to longitudinal ultrasound and torsional mode group. All surgeries were performed through a 2.2 mm clear corneal incision, the method employed being divide and conquer. The endothelial morphometry such as cell density (ECD)…

We present the combined surgery of non-penetrating deep sclerectomy with insertion of the implant in the supraciliary space as an effective and safe surgery for patients with both cataract and primary open angle glaucoma. This study included 20 eyes of 16 patients who were followed up during 12 months. We found a significant intraocular pressure reduction, changing from a preoperative mean of 23 ± 5 mmHg to a postoperative mean of 18 ± 3 mmHg (p<0.002). Similarly, a significant reduction in the number of glaucoma drugs needed was observed, varying from 2.5 ± 0.9 drops per patient to 0.7 ± 0.9

Angle closure glaucoma is characterized by an optic neuropathy with a glaucomatous visual field defect with a white painless eye and mimics primary open-angle glaucoma. Acute angle closure attack is more symptomatic, with a red painful eye and elevated intraocular pressure. In both cases, iridocorneal angle closure is due to an increase in the thickness of the natural lens, which with age has a tendency to close a narrow angle. The rational approach to treating these situations is lens extraction. With today's improved lens extraction techniques, this surgical treatment should be proposed more

PURPOSE: To assess the time course of corneal biomechanical parameters after phacoemulsification with intraocular lens implantation.
METHODS: We examined 54 eyes of 33 consecutive patients undergoing cataract surgery. We quantitatively assessed the values of corneal biomechanics characterized by corneal hysteresis (CH) and corneal resistance factor (CRF) using an Ocular Response Analyzer before and 1 day, 1 week, 1 month, and 3 months after surgery. We also investigated the relationship between these biomechanical parameters and central corneal thickness 3 months after surgery.

PURPOSE: To assess the frequency and results of cataract surgery with concurrent vitrectomy due to zonule weakness in eyes with pseudoexfoliation (PXF).
SETTING: Private practice, Boston, Massachusetts, USA.
METHODS: This retrospective nonrandomized study assessed consecutive eyes with PXF having cataract surgery. High risk was defined as preoperative phacodonesis, iridodonesis, or lens subluxation (subgroup 1); preoperative anterior chamber depth or angle-depth asymmetry between eyes confirmed by slitlamp biomicroscopy or gonioscopy, respectively (subgroup 2); or complicated cataract extrac

SETTING: Two urban public county hospitals.
DESIGN: Case series.
METHODS: Phacoemulsification cataract surgeries performed by residents between January 2005 and April 2008 were retrospectively reviewed. The cataract risk score was calculated retrospectively. Intraoperative complications included posterior and anterior capsular tears, vitreous prolapse, dropped nucleus, and conversion to manual extracapsular cataract extraction.

BACKGROUND: Phacoemulsification and intraocular lens (IOL) implantation has resulted in early visual rehabilitation, increased wound stability and improved refractive results. Viscoelastic substances (VES) are important adjuncts in this type of surgery. Use of VES in phacoemulsification can be associated with adverse effects, the most common and potentially dangerous of which is the transient rise in intraocular pressure (IOP) in post-operative period. To evaluate the effects of viscoelastic substances on post-operative IOP after phacoemulsification with implantation of intraocular lens, a cro…

BACKGROUND: Healon5, the first viscoadaptive agent introduced in ophthalmic surgery, has been judged to be superior to Healon GV in protecting corneal endothelial cells. The purpose of this study was to compare the endothelial protective effects of I-Visc Phaco, a newer viscoadaptive agent, with those of Healon5 in cataract phacoemulsification.
METHODS: A total of 96 unselected patients scheduled to undergo cataract surgery at a community-based hospital in St. Catharines, Ont., were assigned to receive I-Visc Phaco. This group was compared with 112 patients who had received Healon5 in a previ…

This technique manages vitreous prolapse associated with posterior capsule rupture during phacoemulsification by performing small-gauge pars plana vitrectomy with a sutureless, self-sealing incision performed in a closed chamber, maintaining normal intraocular pressure. A high-speed cutter exerts minimal traction on the vitreous. Accessibility to the vitreous through the pars plana is better, ensuring more complete removal of the vitreous and restoration of normal anatomy.
